최근두경부 편평세포암종의 치료에 있어서 바이러스를 매개체로 필요한 유전자를 이입시키는 유전자 치료법이 성공적인 결과를 보임에 따라서 유전자치료가 암치료에 있어서 또하나의 새로운 치료법으로서 가능성을 제시하고 있다. 유전자를 전달하는 바이러스는 여러 종류가 이용되고 있으나, 이들 중 헤르페스바이러스는 다른 바이러스에 비해 여러가지 장점을 가지고 있다. 우선 분열하는 세포뿐만 아니라 미분열상태에 있는 세포에도 효과적으로 감염을 일으키며 , 숙주세포에 감염되는 시간이 1시간 이내로 매우 짧고, 감염률도 타 바이러스에 비하여 매우 높다. 또한 유전자의 크기가 크기 때문에(150kb) 원하는 유전자의 이입에 제한이 적다. 본 연구에서는 헤르페스바이러스의 단백질 막을 형성하는 부분의 유전자(gH)를 삭제시킨, 유전적으로 변형된 defective infectious single cycle herpes simplex virus(DISC 바이러스)를 사용하였다. 이 바이러스는 기존의 유전자 매개체로서의 헤르페스바이러스의 장점을 그대로 지니었으며, 유전적 변형으로 인하여 숙주세포내에서 1회 분열에 그치므로 헤르페스바이러스의 단점인 신경독성 및 세포독성을 배제한 안전성이 입증된 바이러스이다. 본 연구는 첫째, 방사선(10,000 cGy)을 조사하여 분열능력을 없앤 반면, 종양세포의 항원성은 그대로 지닌 종양세포(SCCVII)에 Interleukin-2 (IL-2), 또는 granulocyte macrophage colony stimulating factor (GM-CSF)와 같은 면역조절 유전자를, DISC 바이러스를 매개로 하여 이입시킨 후, 면역조절 유전자 및 면역조절 단백질이 발현되는지 여부를 증명하고, 둘째, 이렇게 만든 종양백신을 종양세포에 특이적이며, 면역능력을 지닌 순종 쥐에 주사후 신선한 동종의 종양세포 투여시의 항암효과를 알아봄과 동시에 가장 효과적인 면역조절 유전자 조합을 선정하며, 셋째, 종양조직을 인위적으로 만들어 여기에 상기의 가장 효과적인 면역조절 유전자가 이입된 DISC 바이러스를 종양내 직접 주사 후 종양내 면역조절 단백의 발현 및 종양증식를 살펴보았으며, 다음과 같은 결과를 얻을 수 있었다.

첫깨, DISC 바이러스 벡터는 in vitro 및 in vivo에서 SCCVII 세포에 효과적으로 유전자를 이입시켰으며, 이로 인한 단백질의 발현을 나타내었다.

둘째, 동물실험에서 종양유발억제 및 성장억제 효과에 있어서 DISC-GMCSF를 이용한 종양백신(SCCVII/ GMCSP)이 DISC-IL2를 이용한 종양백신(SCCVII/IL2)보다 의미있게 우수한 결과를 보였으며, 두 종양 백신의 병합은 SCCVII/GMCSF 단독 백신 효과와 차이가 없었다.

셋째, 종양내 DISC-GMCSF의 투여는 의미있는 종양 성장 억제효과를 보였으며, 종양내 면역반응세포의 증가를 보였다.

이상의 결과로 미루어, DISC 바이러스를 이용한 유전자 치료는, 발현되지 않은 종양세포의 항원성을 유전자변형을 통해 극대화시켜 면역체계에 노출시킴으로서, 미세전이나 미세잔존암이 주된 치료 실패 원인인 두경부 편평세포암종의 새로운 치료방법으로 응용될 수 있으리라 생각한다.

The herpes simplex type 2 defective infectious single cycle virus (DISC virus) are attenuated viruses originally produced as viral vaccines but are also efficient gene transfer vehicle. The main goals of this study were to examine determinants of the gene transfer using DISC vectors for squamous cell cancer and to evaluate the efficacy of vaccination with DISC viruse carrying a combination of immunomodulatory genes (IL-2, GM-CSF) as cancer therapy in a model of squamous cell cancer in the C3H/HeJ mice. SCCVII cells transduced by DISC virus vector (MOI=10) carrying the IL-2, or GM-CSF gene, produced nanogram quantities of IL-2 or GM-CSF per 10**6 cells. Of particular interest was the observation that cells irradiated at different doses (5,000 cGy, 10,000 cGy) secreted levels of IL-2 or GM-CSF that were comparable to nonirradiated cells. In vivo vaccination using tumor cells transduced ex vivo with DISC-lL2 or DISC-GMCSF resulted in protection against subsequent tumor challenge (P<0.01). Among the multiple immunomodulatory transgenes vaccination groups, DISC-GMCSF transfected irradiated tumor cell showed the greatest suppression of tumor development and growth (p<0.001). Marked growth arrest of established tumor was also noted by direct injection of DISC-GMCSF (P<0.001). These data demonstrate that; 1) The DISC virus vector is capable of efficient gene transfer to SCCVII cells, 2) GM-CSF secreting genetically modified tumor vaccine (SCCVII/GMCSF) efficiently protects against tumor cell challenge and suppresses the tumor growth in our tumor model, 3) Intratumoral injection of DISC-GMCSF significantly suppresses the tumor growth in established tumor model and induces intratumoral increase in immune effector cells. DISC virus mediated cytokine gene transfer may prove useful as clinical therapy for head and neck cancers.
