총의치 환자 만족도의 영향요인

무치악 환자의 보철적 수복에 있어 첫 번째로 선택되는 방법인 총의치 치료는 최근까지 임상적, 재료학적 발전이 이루어지고 있으나 치료 후 환자가 호소하는 불편감과 불만족은 여전히 남아있다. 따라서 총의치 환자의 만족도에 영향을 주는 요인을 규명하려는 많은 연구들이 보고되었으나 그 결과는 일관성이 없다. 본 연구에서는 환자와 치과의사를 대상으로 설문조사를 시행하여 환자가 느끼는 총의치의 만족도를 조사하고 이에 영향을 주는 요인을 규명하고자 하였다. 1998년 9월부터 1999년 8월까지 연세대학교 치과대학병원에 총의치 치료를 위하여 내원한 33명의 환자들을 대상으로 총의치 장착 4-6주 후 총의치 만족도, 사회학적 요인, 정신의학적 요인 등을 미리 선정한 설문 항목들을 이용하여 조사하고 직접 총의치 치료를 시행한 치과의사들을 대상으로 환자의 구강상태와 의치에 대한 임상적인 평가를 조사한 후 다중회귀분석을 이용해 전반적인 의치 만족도에 영향을 주는 요인들을 분석하여 다음과 같은 결과를 얻었다.

1. 환자의 사회인구통계학적 변수 중, 자식과의 관계, 경제적 상태, 주거환경, 의치에 대한 다른 사람들의 의견, 성별 등이 총의치 만족도에 영향을 준다.

2. 환자의 정신의학적 변수들 중 우울증의 경향이 높은 환자일수록 총의치에 만족하지 못한다.

3. 임상적 변수 중 구강해부학적 상태가 양호할지라도 환자는 총의치에 만족하지 못하며 의치의 질은 총의치 만족도에 영향을 주지 못한다.

결론적으로 총의치 환자의 만족도에는 임상적, 기술적인 변수인 구강해부학적 조건과 의치의 질보다는 사회인구통계학 및 정신의학적 변수들이 더 큰 영향을 주는 것을 알 수 있다. 따라서 본 연구는 치과의사들이 총의치 치료의 성공과 실패를 예측할 수 있게 함으로써 환자와 치과의사 모두에게서 총의치 만족도를 높이는 데 도움을 줄 수 있으리라 사료된다.

In spite of the progress in techniques and materials in complete denture prosthodontics, patients still complain of discomfort after the insertion of complete dentures. For the last several decades many prosthodontists tried to find the factors influencing the patient complete denture satisfaction, however the reported results became a controversy.

33 patients who visited the department of prosthodontics, dental hospital of Yonsei University, 4 to 6 weeks after the complete denture delivery, were asked to complete the questionnaires on complete denture satisfaction, social variables and psychological variables. The prosthodontists who treated the patients with complete dentures were also asked to complete the questionnaires on evaluation of patient's oral condition and technical quality of dentures. The factors influencing patients'

satisfaction with their complete dentures were analyzed using Multiple Regression Analysis.

The following are the results

1. Among the patient's sociodemographic variables, the variables of relationship with children, economic status, housing condition, other people's opinions of dentures and gender were the influential factors on patient's satisfaction with complete dentures.

2. Patients showing the symptoms of depression, one of patient's psychological variables, were dissatisfied with their complete dentures.

3. In spite of good oral condition, patients were dissatisfied with complete dentures whereas the technical quality of dentures did not influence patients complete denture satisfaction.

According to the results above, patient's sociodemographic and psychological variables rather than clinical variables including oral condition and technical quality of dentures were the influential factors on patient complete denture satisfaction. The results of this study may not only enable prosthodontists to

predict the success and failure of complete denture treatment, but also help both prosthodontists and patients be informed of the essentials of increasing satisfaction with complete dentures.
