가시위근의 힘줄과 힘살은 어깨 부딪힘증후근에서 가장 손상이 많이 되는 중요한 구조이며 구성요소와 지나가는 방향을 확인하는 작업은 임상적으로 매우 중요하다. 자기공명영상를 이용하여 부리봉우리인대의 가쪽에서 가시위근의 힘줄과 힘살의 위치와 구성을 알아보고자 하였다.

30개의 시체와 70명의 환자의 어깨 자기공명영상을 그 대상으로 가시위근의 위치와 구성 비율을 중심으로 관찰하였다. 시체의 T1 강조영상과 환자의 양자밀도 강조영상을 대상으로 하였다.

가시위근의 힘줄과 힘살의 위치는 다음의 다섯가지의 형태로 구분하였다. I형은 가시위근이 부리봉우리인대 아래에만 있는 경우, Ⅱ형은 가시위근의 대부분이 부리봉우리인대 아래에 있는 경우, Ⅲ형은 가시위근의 1/2은 부리봉우리인대 아래에 있고 1/2은 봉우리

아래에 있는 경우, Ⅳ형은 가시위근의 대부분이 부리봉우리인대 아래에 있는 경우. 그리고 Ⅴ형은 가시위근이 봉우리 아래에만 있는 경우로 하였다.

가시위근의 위치를 측정함에 있어서 어깨 관절의 회전 정도가 가시위근의 위치에 미치는 영향을 알아보기 위해, 위팔두갈래근의 긴갈래힘줄이 위팔뼈에 접하는 선과 어깨뼈의 관절 면을 잇는 선이 이루는 각도를 측정하였다. 이 각도를 가시위근의 위치의 형태별로

구분하여 평균을 구한 후 통계학적으로 각 형태간에 유의한 차이가 있는지 조사하였다.

70명의 환자의 경우는 임상적으로 어깨 부딪힘증후군의 유무에 따라 두가지 군으로 나누었는데, 1군은 어깨 부딪힘증후군이 있는 경우, 2군은 어깨 부딪힘증후군이 없는 경우로 하였다. 위의 두군과 가시위근의 다섯가지 형태의 상관성을 연구하였다.

또한 가시위근의 힘줄과 힘살의 비율은 다음의 네가지 형태로 구분하였다. Ⅰ형은 힘줄로만 구성되어 있는 경우, Ⅱ형은 대부분 힘줄로 구성되어 있는 경우 Ⅲ형은 힘줄과 힘살의 비율이 비슷한 경우, Ⅳ형은 대부분 힘살로 구성되어 있는 경우로 하였다.

가시위근의 힘줄과 힘살의 위치의 각 형태에 따른 빈도는 전체 100예 중 Ⅰ형이 19예, Ⅱ형이 55예, Ⅲ형이 21예, Ⅳ형이 4예, Ⅴ형이 1예 있었다.

위팔두갈래근의 긴갈래힘줄이 위팔뼈에 접하는 선과 어깨뼈의 관절면을 잇는 선이 이루는 각도에 있어서 위의 다섯가지 형태는 서로 통계학적으로 유의성있는 차이가 없어 어깨 관절의 회전 정도가 가시위근의 위치에 크게 영향을 미치지 않았다.

70명 환자의 경우, 1군은 31명, 2군은 39명이었다 1군과 2군간의 가시위근의 위치에 따른 다섯가지 형태는 차이가 없었다.

가시위근의 힘줄과 힘살의 비율은 전체 100예 중, Ⅰ형이 13예, Ⅱ형이 19예, Ⅲ형이 21예, Ⅳ형이 47예의 빈도를 보였다.

위의 결과로 볼 때, 부리봉우리인대의 가쪽에서 가시위근의 위치와 구성은 다양하였으나, 이러한 변이들이 어깨 부딪힘증후군과 큰 상관은 없었다. 그러나 이러한 변이들에 대한 이 연구의 결과는 앞으로 어깨 부딪힘증후군의 치료와 연구에 있어서 도움이 될 것이다.

In this study, variations of location and composition of the supraspinatus muscle and tendon on MR imaging were evaluated in 70 patients' and 30 cadaveric shoulders at the lateral margin of the coracoacromial arch. The purpose of this study was to demonstrate the variations of location and composition of supraspinatus muscle and tendon and to correlate them with shoulder impingement syndrome.

Oblique sagittal images of 30 cadavers and 70 patients were obtained at the lateral margin of the coracoacromial ligament. Locations of supraspinatus mucsle and tendon were categorized into 5 types: I, all of them under the acromion; Ⅱ, most of them under the acromion; Ⅲ, 1/2 uner the acromion, 1/2 under the ligament; Ⅳ, most of them under the ligament; Ⅴ, all of them under the ligament. The angle between the line of articular margin of scapula and the line of bony margin of bicepital groove was measured to evaluate how rotation of the humerus affected the

supraspinatus location. Seventy patients were categorized into 2 groups: group 1, patients who had impingement syndrome; group 2, patients who had not impingement syndrome. Five types of location were correlated above 2 groups. Compositions of muscles and tendons at the same section of MR images were evaluated by grouping into four categories: I, only the tendons were demonstrated; Ⅱ, most of them were tendon: Ⅲ, 1/2=tendon. 1/2=muscle; Ⅳ, most of them were muscle. On 100 MR images, locations of supraspinatus muscle and tendon were as follows:

type Ⅰ, 19; Ⅱ, 55; Ⅲ, 21; Ⅳ, 4; Ⅴ, 1 case. There was no significant difference between location types depending rotation angle of the humerus. Group Ⅰ patients(n=31) revealed type Ⅰ, 29.0%; Ⅱ, 51.6%; Ⅲ, 9.7%; Ⅳ, 9.7%. Group Ⅱ patients(n=39) showed type Ⅰ, 20.5%; Ⅱ, 56.4%; Ⅲ, 23.1%. No significant

difference between group Ⅰ and Ⅱ was observed. According to compositions of supraspinatus muscle and tendon there were type Ⅰ, 13; Ⅱ, 19; Ⅲ, 21; Ⅳ, 46 cases.

In conclusion, the important thing was to find that locations and compositions of supraspinatus muscle and tendon were various. However, these variations did not relate to impingement syndrome. That was different from the initial idea that I had expected. But the verification of these variations in detail may be helpful for the diagnosis and treatment of the impingement syndrome and related shoulder disorder.
