심근경색증에서의 Thrombomodulin 유전자의 역할

Thrombomodulin(TM)은 혈관내피세포의 표면에 존재하는 당단백수용체로써 트롬빈과 결합하여 protein C를 활성화시킴으로써 혈전형성과정에 있어 항응고과정에 관여한다. 그러므로, TM 유전자변이로 인한 유전자발현의 이상 혹은 TM의 결함은 항응고 작용에 영향을 미쳐 심근경색과 같은 질환의 발생에 영향을 미칠 수 있다. TM의 유전적 이상에 대해서는 혈전성향을 가진 환자에서 보고된 바 있으며, 최근 TM 유전자변이와 심근경색과의 관련성에 대해 연구되기 시작하고 있다. 본 연구에서는 심근경색의 발생에 미치는 TM 유전자의 역할을 알아보고자 심근경색증 환자에서 비교적 흔히 관찰되는 유전자 변이를 찾아보고 이들과 심근경색증 발생과의 관련성을 분석하였다.

심근경색환자들의 혈액으로부터 추출한 DNA로 single-strand conformation polymorphism-heteroduplex 분석법을 이용하여 유전자변이에 대한 검색을 시행하여 기존에 보고된 바 있는 유전자전사조절부위, 엑손 부위에서 TM G-33A, TM Ala455val 유전자 변이가 한국인에서도 비교적 높은 빈도로 나타남을 확인하였고, 3' 비전사부위에서 TM C1922T 유전자다형성을 새로이 발견하였다. 이들 유전자변이를 allele-specific oligomer 보합결합방법을 이용하여 임상적으로 심근경색으로 진단 받은 234명과 대조군291명 (실근경색의 병력이 없으면서 혈관조영술상 정상 소견을 보인 114명, 건강검진자 중 심장 허혈의 임상 소견이 관찰되지 않았던 177명)을 대상으로 세 가지 유전형을 분석하였다. TM A1a455va1유전형의 빈도는 서양인의 보고와 유사하였으나, TM G-33A에서의 변이유전자형 빈도는 한국인에

서 훨씬 높은 빈도로 관찰되었다. 그리고, 세 유전자변이간에는 유의한 연쇄불균형이 관찰되어 연관관계가 있음을 알 수 있었다. 남자에서는 변이유전형의 빈도가 대조군에 비해 유의하게 높았으나, 여자에서는 각 유전자형의 빈도가 환자군과 대조군간에 유의한 차이

가 없었다. 심근경색의 발생에 미치는 영향을 다변량 로지스틱으로 분석한 결과 TM 455va1 및 TM 1922T 유전형은 남자에서 심근경색발생의 독립적으로 유의한 요인이었으며(각각OR[95% Cl]=1,799[1.125-2.878], p=0.014 와 OR[95% Cl]=5.624[1.019-31.025], p=0.048), TM -33A 유전형의 경우 상대위험도가 증가되어 있었으나 통계적인 유의성은 관찰되지 않았다(OR[95% Cl] =1.784[0.953-3.341], p=0.07). 여자에 있어서는 세 유전자변이 모두 심근경색발생에 있어서 유의한 독립적인 요인으로써 의의가 관찰되지 않았다. 유전자전사 조절부위에 존재하는 TM G-33A 유전자형이 유전자 발현에 미치는 영향을 알아보고자 TM G-33A 유전자형에 따른 TM promoter luciferase construct를 제작하여, HepG2, BAE, 그리고, CHO cell에 transfection후 luciferase 활성도를 비교하여 본 결과 TM G-33G 유전자형에 비해 TM -33A 유전자형인 경우 세포주에 따라 차이는 있으나, 유전자 전사조절부위의 활성도가 감소되어 있어 TM G-33A 유전자형이 TM 유전자 발현에 영향을미 침을 시사한다.

이상의 결과에서 TM 유전자내 변이유전자를 가진 경우 남자에서 심근경색의 발생위험도가 증가하며, 이는 유전자발현과 엑손 흑은 3' 비전사부위에서의 유전자변이가 기능적으로 영향을 미치기 때문으로 추정된다. 결론적으로 심근경색증의 발생에 있어서 혈액응고과정에 관련이 있는 TM 유전자는 중요한 역할을 하며, TM 유전자변이에 대한 분석은 남자에서 심근경색발생 위험도를 예측하는데 유의한 지표로 이용될 수 있을 것이다.

[영문]

Thrombomodulin(TM) is thrombin receptor present on the luminal surface of endothelial cells. Because the thrombin-TM complex acts as an anticoagulant, the functional variants or deficiency of TM may lead to increment of thrombotic tendency. In this study, we screened the genetic variants of the TM gene inpatients

with myocardial infarction(Ml) and analyed the genotype to elucidate the effects of genetic variations of TM gene on the development of the Ml. We screened a promoter region and coding sequence of the TM gene using single strand conformation polymorphism-heteroduplex analysis and identified three common genetic variants; that were TM G-33A, TM Ala455Val, and TM C1922T. The genotype frequencies were investigated in the patients with Ml (n=234) and control subjects (n=291) by the method of allele-specific oligomer hybridization. The frequencies of mutant genotypes (TM -33A, TM 455va1, and TM 1922T)were higher in patient group compared to the control subjects in men while there were no significant differences in females. In the multiple logistic regression analysis, TM 455val and TM 1922T alleles were independent risk factors for Ml(OR[95% Cl; 1,799[1.125-2.878] p=0.014 and 5.624[1,019-31,0251], p=0.048,respectively) in males. However, the genetic variations were not independent risk factors for Ml in females. There were significant linkage disequilibriums among three genetic variants. These linkage disequilibriums explain the similar effects of three genetic variants on the

development of Ml. To investigate the effect of the TM G-33A mutation on TM promoter activity, the two TM promoter constructs(pTM-355 and pTs-125, bearing TM -33G or TM -33A) containing of firefly luciferase gene were transfected into HepG2, BAE, and CHO cells, The promoter activities were higher in the promoter constructs with TM -33G compared to the constructs with TM -33A in pTM-355. These results suggest the possibility of the positive predisposing effect of TM -33A allele on Ml in males. The functional study for TM A1a455va1 and TM C1922T should be followed to elucidate the genotype effects of these mutations on the development of Ml.

In this study, we identified three genetic variants of TM gene and showed the significant associations between genetic variants and Ml in males. These results proposed that TM gene is an attractive candidate for genetic risk factor for Ml in Koreans.
