의료기관서비스평가제도에 대한 병원근무자의 의견

의료기관서비스평가제도의 향후 정책방향을 제시하고자 지난 4년 동안 시험평가를 실시한 165개 병원 중 60개 병원을 선정하여 병원에 근무하는 1,200명(병원별 20명)을 대상으로 1999년 4월 1일부터 5월 15일까지 구조화된 조사표를 이용하여 자기기입식 방법으로 연구자료를 수집하였다. 연구에서 분석된 표본수는 459명이었으며 주요 결과는 다음과 같다.

1. 의료기관서비스평가제도의 시행에 따라 병원서비스가 전반적으로 개선되었다. 그 중 환자편의시설, 환자편의 및 권리와 관련한 제도, 환자만족도 부문의 서비스가 크게 개선되었고, 의료기관서비스평가 수행 과정별로는 평가를 준비하는 과정에서 병원의 서비스가 크게 개선되었다.

2. 의료기관서비스평가는 병원 및 병원근무자가 중심적인 역할을 담당해야 한다는 응답자가 97%이었고, 소비자의 참여에 대해서도 긍정적인 의견이 많았으나(63%) 상대적으로 정부나 보험자의 역할과 개입에 대해서는 부정적이었다.

3. 의료기관서비스평가 주관기구는 응답자의 61%가 독립된 평가전담기구를 지지하였고 정부나 정부출연기관이 직접 평가를 담당하는 것에 대해서는 부정적이었다.

4. 평가결과는 '5등급화'하여 산출하는 방식을 가장 선호하고, 평가결과를 공표하는 것을 반대하는 의견은 약 15%이었으며, 평가결과에 대한 보상은 금전적 보상을 선호하였다.

5. 의료기관서비스평가제도와 병원표준화심사제도를 통합하여 운영하는 것이 바람직하다는 의견이 86%이었다. 서비스평가와 표준화심사제도에 차별성이 있다는 의견은 300병상 미만 병원 근무자에서 가장 많이 제기되었으며, 두 제도의 통합 필요성에 대해서는 3차진료기관 근무자가 가장 많이 지지하였다.

이상의 연구결과는 향후 의료기관서비스평가제도의 정책방향을 설정함에 있어 의료기관서비스평가의 경험이 있는 병원근무자들의 의견만을 정리한 것이므로 병원을 이용하는 소비자 의견에 대한 연구가 필요하고 여타 제도와의 관계 등에 대해 종합적으로 연구가 이루어져야 할 것이다.

이러한 결과를 근거로 향후 의료기관서비스평가제도의 확대 실시를 위한 정책방향을 제시하면 다음과 같다.

첫째, 의료기관서비스평가는 준비과정에서 서비스 개선효과가 크므로 평가기준을 사전 배포하여 충분하게 준비하도록 하되, 평가당일 과잉준비로 인한 평가의 왜곡을 방지할 수 있도록 보완책을 강구한다.

둘째, 의료기관서비스 평가와 병원표준화 심사는 그 내용에 있어서 차별성이 있으나 평가를 받는 병원의 입장을 고려할 때 장기적으로 양 제도를 통합하여 시행하되, 양 제도의 장점을 살릴 수 있는 방향으로 통합하여야 한다.

셋째, 의료기관서비스평가는 정부주도보다는 병원과 병원근무자가 주도적으로 참여하는 가운데 객관성과 신뢰성을 확보할 수 있도록 제3의 독립된 기구를 설립하여 운영하는 것이 바람직하다.

마지막으로, 평가결과는 평가대상기관간 구분이 가능하도록 5등급화 하여 산출하고, 평가결과를 공표하되 이로 인한 병원 피해 등의 문제점을 최소화할 수 있도록 보완책을 마련해야 할 것이며, 평가결과에 따른 보상제도를 도입한다면 의료보험수가체계와 연계하는

방안을 검토하는 것이 필요하다고 생각한다.

A hospital services evaluation programme was introduced in 1995 and, as a pilot project, has been applied to 165 hospitals for the last 4 years. This study aims to study the opinions of hospital employees for the policy issues on developing this programme as a nationwide accreditation system and to suggest desirable policy alternatives. 1,200 hospital employees in 60 hospitals(20 employees in each hospital) were selected and questionnaire was mailed. The total number of respondents was 459. Key results are as follows.

1. The services provided by surveyed hospitals were turned out to be better than before the survey. The improved areas are facilities related with the convenience of patients, management system for patients' right and convenience, and patients' satisfaction. The preparation period for the survey was turned out to be most useful in improving hospital services.

2. A total of 97% of respondents answered that hospitals and hospital employees should play a key role in implementing a hospital services evaluation programme and 63% of respondent agreed in consumer participation in this programme while revealing negative response in the involvement of government and insurance body.

3. A total of 61% of respondents answered that the governing body for this programme should be independent, non-profit, nongovernmental organization and showed a negative response in that government or a government sponsored body control this programme directly.

4. Respondents preferred to present the final result of survey with a five staged measure of the level of compliance with the standards and to provide financial incentives to high scoring hospitals. Only 15% of respondents disagreed in making the final result to be public.

5. A total of 86% of respondents preferred to integrate the hospital standardization programme which has been conducted by Korean Hospital Association with the hospital services evaluation programme. The difference between two programmes is more recognized by employees in 300bed size hospitals than those in

other size hospitals.

The study results are derived from the opinions of employees who have had an experience in the hospital services evaluation programme. Therefore in order to formulate a desirable policy alternative in developing hospital services evaluation programme as a nationwide accreditation system, it is necessary to study the opinions of customers and to look at the wider policy implications in relation to the other health services management policies.

Based on the study results, the followings are suggested as a desirable approaches in extending a hospital services evaluation programme to all the hospitals.

1. It is desirable that the survey is conducted with advance notice and the standards are provided to hospitals in advance in order to allow a sufficient preparation time for hospitals because the service improvement is mainly achieved during the survey preparation period. However it is necessary to develop ways of preventing over-reaction by hospitals to the survey so as to investigate hospital services as they are.

2. In the long run it is necessary to integrate the hospital standardization programme with the hospital services evaluation programme and develop a nationwide accreditation system which utilizes the merits of each programme.

3. The governing body should be independent, non-profit and non-governmental organization in order to guarantee reliability and objectiveness of the survey results while allowing active participation of hospitals and hospital employees in the board.

4. It is desirable to present the survey result with a five scaled grade and make it public while minimizing the negative impact on hospitals by public disclosure. In case of introducing a financial incentive system to high scoring hospitals, it is considered to be suitable to combine financial incentives with

medicare payment system.
