환자의 병원이용 및 만족도에 대해 병원직원의 인식과 환자의 인식간 차이를 대학병원과 종합병원으로 구분하여 비교하고자 서울과 경기도내 4개 병원의 직원 368명, 환자 485명을 대상으로 조사하였다. 연구방법은 자기기입식 설문지를 이용하여 1999년 4월 30일부터5월 10일까지 연구자료를 수집하여 통계분석하였다. 연구의 주요결과는 다음과 같다.

1. 환자의 병원인지경로에 대해 직원과 환자간 인식은 차이를 보였는데, 직원은 직접 방문을 통해 환자가 인지한다고 했으나 환자는 대중매체, 병원직원 연관, 타병원 소개로 병원을 처음 인지한다고 하였다. 대학병원과 종합병원을 구분하여 비교하면 큰 차이가 없었다.

2. 병원을 선택하는 요인은 직원과 환자간 차이가 있었는데, 직원은 근무병원이라서 병원이용을 추천한 반면 환자는 의료진, 교통편리, 친절/편의, 대학병원의 요인으로 병원을 선택하였다. 대학병원과 종합병원으로 구분하여 비교하면 큰 차이가 없었다.

3. 환자의 병원이미지에 대해 직원과 환자간 인식을 비교하면 차이가 있었는데, 대학병원의 경우 직원은 환자에 비해 대학병원과 기업연관의 이미지를 가진 반면, 실제 환자는 이용편리, 직원 친절을 병원에 대한 이미지로 가지고 있었다. 종합병원의 경우 환자는 직원

에 비해 이용편리에 대한 이미지를 가지고 있었다. 대학병원과 종합병원별로 비교시 차이가 있었는데, 병원이미지 항목 대부분에 대해 대학병원은 직원의 인식이, 종합병원은 환자의 인지가 더 높았다.

4. 환자의 병원만족도에 대해 직원과 환자간 인식 비교시 직원의 인식에 비해 실제 환자의 만족도가 더 높았고 대학병원과 종합병원별 차이는 없었다.

5. 병원인지경로와 병원만족도의 관련성을 직원과 환자별 비교시 통계적으로 유의하지 않았으나 직원은 직접방문이나 병원직원과 연관돼 환자가 병원을 인지한다고 한 경우의 만족도가 높은 반면 환자는 대중매체나 직원연관에 의한 인지시 만족도가 높았다. 병원선택요인과 만족도의 관련성을 직원과 환자별로 비교하면 직원은 시설/규모로 병원을 추천한 경우가 병원 서비스에 대해 만족하였고, 환자는 시설/규모 외에도 의료진, 친절/편의와 대학병원의 선택요인으로 내원한 경우 병원서비스에 만족하였다.

이상의 결과로 보아 환자의 병원이용 및 만족도에 대해 직원의 인식과 환자의 인식에는 차이가 있었으나, 대학병원과 종합병원으로 구분하여 비교시에는 큰 차이가 없었고 직원과 환자별 병원선택요인과 만족도는 관련성이 있었다. 결국 이 결과는 환자에 대한 직원의 인식은 실제 환자의 인식과 차이가 있어 직원의 고객지향적 태도 형성을 위한 기초자료와 직원과 환자의 만족을 위한 내·외부 마케팅 전략 수립시 중요한 마케팅 정보로 활용할 수 있을 것이다.

이 연구는 서울과 경기지역 소재 대학병원과 종합병원 각각 2개를 대상으로 하여 연구대상의 지역적 특성, 병원 설립형태별 차이 등을 감안하지 못하였다는 문제점으로 연구결과를 우리나라 전체병원에 일반화할 수 없는 제한점과 연구대상의 일반적 특성 및 라이프스타일을 고려한 병원이용과 만족도를 파악하지 못하였다. 따라서 향후 연구과제는 직원과 환자의 일반적 특성에 따라 병원이용 및 만족도에 어떤 차이를 보이는지, 직원의 인식이 환자만족에 어떤 영향을 미치는지에 대한 추가적인 연구가 필요할 것이다.

To compare the differences in the recognition on hospital utilization and satisfaction, 368 hospital employees and 485 patients were selected in four hospitals in Seoul and Kyung-gi do. The survey was done using a constructed form of checklist from Apr. 30 to May 10, 1999. And the results are as follows:

1. Gaining knowledge of hospitals available among hospital employees differed from that of patients. When finding out about sources of information concerning hospitals, direct visit to the hospital was recognized to be the main factor for the hospital employees in contrast to the patients' recognition which were mass media, personal involvement of job related workplace and recommendations from other hospitals. There was no difference between university and general hospitals.

2. The factors that concern which hospital to choose there was a difference between hospital employees and patients. Hospital employees recommended their own hospital solely based on the reason that it was their work place. On the other hand, the

patients made a choice based on the type of medical staff, transportation available and whether it was a university hospital or not. There was no difference between university and general hospitals.

3. The recognition of employees concerning hospital image of a hospital between hospital employees and patients was different. In university hospitals, the employees recognized the name value of university hospital and cooperation as most important, whereas the patients thought convenience, kindness were the main factors. Patients considered general hospitals to be more convenient. There was some difference between university and general hospitals. For university hospitals employees' recognition was higher and for the general hospital patients' recognition is higher on hospital image.

4. The recognition of employees was different from that of patients' on hospital satisfaction. The patients' satisfaction was higher than that of employees'. There was no difference between university and general hospitals.

5. Relationship between hospital perception and hospital satisfaction was not statistically significant. Gaining knowledge of a particular hospital for employees was through either direct visit or personal involvement of job related workplace whereas for patients it was through mass media or personal involvement of job related workplace. In comparison of the choice hospital related to the degree of hospital satisfaction, the employees was satisfied of hospital service depending on the type and scale of medical facilities in contrast to patients who were satisfied

based on medical facilities, medical staff, kindness and convenience.

Based on the above findings, the employees' recognition on hospital utilization and satisfaction was different from that of the patients, but there was no difference between university and general hospitals. In both groups choice of hospital was associated with satisfaction. Results showing difference between employees' and patients' recognition can be applied to implement customer-oriented attitude and be used as a baseline data for internal-external marketing planning of hospital management.

The study may be limited in that the results cannot be generalized due to its small sample size and not being able to reflect demographic variables and life style. Further studies to investigate the difference of hospital utilization and hospital

satisfaction will be necessary to define demographic characteristics and recognition of employees which influences patients' hospital satisfaction.
