당뇨병 환자에서 미세혈관 합병증의 하나인 당뇨병성 신병증은 단백뇨와 전신혈압의 점진적 증가 및 사구체 여과율 감소를 특징으로 하며, 그 발생에 안지오텐신 전환효소(angiotensin converting enzyme, ACE) 유전자 다형성이 관여하리라는 가능성이 제시되었으며 이에 대한 연구가 활발히 전개되고있다. ACE는 내인성 심혈관 수축계인 레닌-안지오텐신계(renin-angiotensin system)에서 angiotensin I을 angiotensin Ⅱ로 전환하고, bradykinin을 불활성화 시킴으로서 혈압 및 심혈관계 질환의 발현과 전해질 조절에 중요한 역할을 한다. 17번 염색체의 장완에 위치하는 21 kilobases 크기의 ACE 유전자는 16번째 intron의 287 base pair 길이의 alu 반복서열 결손 유무에 따라 대립인자를 삽입(insertion)과 결손(deletion) 형으로 구분하여 유전자형을 DD, ID, II으로 분류할 수 있고, 각각의

유전자형에 따라 당뇨병성 신병중의 발생, 진행 및 항단백뇨 효과가 다름이 보고되었다.

ACE 억제제는 여러가지 원인의 신장질환에서 단백뇨를 감소시키는 효과가 있으나, 단백뇨 배설감소 정도에 대한 개개인의 반응은 다양하다. ACE 억제제의 항단백뇨 기전은 확실히 규명되어 있지 않으나 원심성 신 소동맥의 저항 감소와 전신혈압하강에 의한 사구체 모세혈관 압력 감소라는 혈역학적 변화로 설명할 수 있다.

당뇨병성 신병증의 발생에 DD형 또는 D 대립인자가 관여한다는 보고가 있으며, II형이 신병증 발생 빈도를 낮춘다는 보고 등 다양한 연구결과가 보고되고 있다.

그러나 아직 인슐린 비의존성 당뇨병에서 ACE 유전자 다형성과 ACE 억제제의 항단백뇨 효과의 연관성에 관하여는 연구된 바가 빈약하다. 이에 본 연구에서는 인슐린 비의존성 당뇨병에서 24시간 소변 단백 배설량이 500mg이상인 당뇨병성 신병중 환자 83명을 대상으로 3개월간 ACE 억제제(benazepril 10mg/day 또는 perindopril 4mg/day) 치료 후 항단백뇨 효과와 ACE 유전자 다형성의 관련성을 관찰하여 다음과 같은 결과를 얻었다.

1. ACE 유전자 DD: ID: II의 상대적 비율은 0.20: 0.40: 0.40, 대립인자 D와 I의 비율은 0.40: 0.60로 한국인 정상대조군과 유의한 차이는 없었다(p>0.05).

2. ACE 억제제 치료 전 ACE 유전자형에 따른 세 군간의 연령, 비만도, 당뇨병 유병기간, 혈액요질소(BUN), 혈청 creatinine, HbA^^1C, 혈압, creatinine 제거율, 24시간 소변 단백, 알부민 및 나트륨 배설량은 유의한 차이가 없었다(p>0.05).

3. ACE도 억제제(benazepril 10mg/Day 또는 perindopril 4mg/day)를 3개월간 치료 후, 세 군간의 BUN, 혈청 creatinine, HbA^^1C, 24시간 소변 나트륨 배설량, 혈압(수축기, 이완기, 평균 동맥압) 감소 및 creatinine 제거율 감소는 유의한 차이를 보이지 알았고(p>0.05),24시간 소변 단백 및 알부민 배설량의 감소율은 DD형이 ID형과 II보다 유의하게 높았다(50.9±19.2 vs 19.2±16.0, 20.2±20.4% 52.6±23.6 vs 13.5±51.8, 24.8±23.9%, p<0.05).

4. ACE 억제제 치료 전 후, 각 유전자형 군내 비교에서 BUN, 혈청 creatinine, HbA^^1C, 24시간 소변 나트륨 배설량 및 creatinine 제거율 감소는 유의한 차이를 보이지 않았고(p>0.05), 24시간 소변 단백 및 알부민 배설량의 감소율 및 혈압(수축기, 이완기, 평균

동맥압) 감소는 유의한 차이를 보였다(p<0.05).

5. ACE 억제제 종류(benazepril 10mg/day, perindopril 4mg/day)에 따른 비교에서 DD형 및 ID형 군은 치료 전 후 연령, 비만도, 당뇨병 유병기간, DUN, 혈청 creatinine, HbA^^1C, 혈압(수축기, 이완기, 평균 동맥압), creatinine 제거율, 24시간 소변 단백, 알부민 및 나트륨 배설량, 혈압(수축기, 이완기, 평균 동맥압) 감소, creatinine 제거율 감소 및 24시간 소변 단백 및 알부민 배설량의 감소율은 유의한 차이를 보이지 않은 반면(p>0.05), II 군은 당뇨병 유병 기간, 치료 전 BUN 및 creatinine 제거율, 치료 후 creatinine 제거율 감소에서 유의한 차이를 보였다(p<0.05).

이상의 결과로, 신병증이 있는 인슐린 비의존성 당뇨병 환자에서 ACE 유전자형에 따른 ACE 억제제의 항단백뇨 효과는 DD형에서 ID형 및 II보다 의의있게 높았으며, 각 유전자형 군내 ACE 억제제 종류에 따른 차이는 없었다 ACE 대립인자의 분포는 중국이나 일본과 같이 I 대립인자가 많았고, 이는 D 대립인자가 I 대립인자보다 많은 유럽이나 미국의 결과와는 다른 소견을 보여 ACE 유전자형은 인종별 차이가 있는 것으로 보이며. 또한 유전자형의 분포가 정상인과 당뇨병성 신병중 환자와 차이가 없어 당뇨병성 신병증의 발생과 ACE유전자 다형성 사이에는 의미있는 관계를 찾아볼 수 없었다. 그러나 ACE 유전자 다형성과 당뇨병성 신병증의 진행 및 ACE 억제제의 장기간 치료 효과에 대하여 어떠한 결과를 보일 것인지는 앞으로 더 많은 연구가 진행되어야 하리라 생각된다.

Diabetic nephropathy, one of the microvascular complications in diabetic patients is characterized by progressive increase in systemic blood pressure and proteinuria, and decrease in glomerular filtration rate. It has been reported that the gene polymorphism of angiotensin converting enzyme(ACE) might participate in this occur. trance.

The ACE in retrain-angiotensin system affects the cardiovascular system through angiotensin Ⅱ formation and inactivation of bradykinin. The ACE gene is located on the long arm of chromosome 17. Insertion or deletion of any 287 base pairs fragment in the 16th intron of the ACE gene may determine its genotype. These genotypes in trun are used to characterize the polymorphism as Ⅱ, ID or DD type. It has been reported that each of these genotypes may have different effects in the occurrence,

progression and reduction of proteinuria. The antiproteinuric mechanism of ACE inhibitors are thought to decrease glomerular capillary pressure by reducing both efferent renal arteriolar resistance and systemic blood pressure. Recent reports suggest that genotype DD or D allele may be involved in the diabetic nephropathy while genotype Ⅱ may lower the chance for diabetic nephropathy.

We investigated the association of ACE gene polymorphism on the nephropathy in non-insulin dependent diabetes mellitus(NIDDM) by assessing ACE genotypes and respective short-term effect of ACE inhibitors on proteinuria of 83 NIDDM patients in whom urinary protein excretion is over 500mg/day, The results were as fellows.

1. The ratio of ACE genotypes(DD:ID:II) for the diabetic nephropathy was 0.20 : 0.40 : 0.40, and that of ACE allele, D and 10.40 : 0.60. The results had no significant difference when compared with normal Korean Population(P>0.05).

2, There were no significant differences of clinical parameters such as age, body mass index, DM duration, BUN, serum creatinine, HbA^^1C, blood pressure, clearance of creatinine(Ccr) and excretion of 24-hour urinary protein, albumin and sodium

among three groups(p>0.05).

3. After the three-month treatment using ACE inhibitors, there were no significant differences in the reduction of blood pressure and clearance of creatinine among all three groups(p>0.05). However, the percent reduction of

24-hour urinary excretion of protein and albumin for DD genotype was significantly higher than that of ID and II genotypes(50.9±19.2 vs 19.2±16.0, 20.2±20.4%, 52.6±23.6 vs 13.5±51.8, 24.8±23.9%, p<0.05).

4. There was no significant difference in clearance of creatinine beforeand after ACE inhibitor treatment within each group(p>0.05). On the other hands, blood pressure and 27hour urinary excretion of protein and albumin decreased significantly after ACE inhibitor treatment within each group(p<0.05).

5. In the comparison according to ACE inhibitors(benazepril 10mg/day or perindopril 4 mg/day) before and after the treatment within each of DD and ID genotypes, there were no significant differences in blood pressure, clearance of creatinine and the percent reduction of 24-hour urinary excretion of protein and

albumin(p>0.05), and in II genotype, there were no significant differences in reduction of blood pressure and the percent reduction of 24-hour urinary excretion of protein and albumin(p>0.05), but the reduction of clearance of creatinine of

perindopril group was significantly higher than that of benazepril group(p<0.05).

The results of the present study show that the antiproteinuric effect by ACE inhibitors in DO genotype is significantly higher than that of ID and II genotypes. However, no significant differences were noted according to different ACE inhibitors within each of three genotypes. In the distribution of allele D and I, allele D is more frequent than allele I. This result corresponds with reports from China and japan, unlike those of Europe or USA, suggesting an ethnic difference. We couldn't find a significant relationship between the ACE polymorphism and the

occurrence of diabetic nephropathy since there is no significant difference in the distribution of ACE genotypes between normal persons and the patients with diabetic nephropathy in Korea. But further studies on the issues for the ACE gene polymorphism and the progression and long-term effects of ACE inhibitors in diabetic nephropathy may be needed.
