성장기 아동에 있어 Gonial angle의 변화와 IMPA의 변화에 관한 누년적 연구

Abstract

Gonial angle은 부정교합 환자의 진단과 치료계획 수립시 중요하게 이용되고 있으며, 두개 안면 복합체의 성장방향과 안모형태의 분류 그리고 하악골의 형태학적 및 기능적인 면을 연구하는데 사용되어왔다. 또한 임상적인 치료에 있어 하악 절치의 위치는 치료의

안정성과 하안면부의 심미적인 면에서 중요하며 전후방적인 절치의 위치는 치열궁 장경에 영향을 미쳐 치료계획 중 발치 결정의 중요한 기준으로 사용되고 있다.

저자는 연세대학교 치과 대학 교정과에 소장되어있는 E여자학교 사범대학 부속초등학교 졸업생중에서 전신질환이 없고 발육상태가 양호한 소년 15명, 소녀 19명을 대상으로 7세에서 만 9년간 누년적으로 촬영한 측모 두부 방사선 구격사진을 이용하여 gonial angle의 연령에 따른 변화양상과 gonial angle의 변화가 하악절치의 위치에 미치는 영향에 관한 누년적 연구를 하여 다음과 같은 결론을 얻었다.

1. 성장에 따른 gonial angle의 누년적 변화양상을 보면 25명에서는 감소를, 9명은 일정하게 유지되거나 증가하는 양상을 보였다.

2. Gonial angle이 감소한 아동에서는 IMPA가 증가하는 경향을 보였으며, gonial angle이 일정하게 유지되거나 증가한 아동에서는 IMPA가 점진적으로 감소하는 경향을 보였다.

3. Interincisal angle은 gonial angle의 변화 양상에 관계없이 감소하는 경향을 보였다.

This study was undertaken to investigate the growth change of gonial angle and interrelationship between the growth change of gonial angle and IMPA in the longitudinal data from 7 years to 15 years.

By analyzing the serial lateral cephalograms of 15 males and 19 females of 7 years old to 15 old wo had no abnomality in growth and development and no history of orthodontic treatment, the following conclusions were obtained.

1. Gonial angle gradually decreased with aging in 25 children and increased or maintained in 9 children.

2. There wsa s tendency that children who decreased gonial angle with aging showed gradual increase of IMPA nad children who increased or maintained gonial angle with aging showed decrease of IMPA.

3. There was a tendency that refardless of the change of gonial angle, interincisal angle decreased with aging.
