관상동맥질환에서 혈장 TF 및 TFPI 변화와 말초혈액 단구의 TF발현 유도

Tissue factor(TF)는 제 Ⅶ 또는 Ⅶ^^a 응고인자와 결합하여 외인계 옹고과정을 활성화시키는 옹고인자이다. Tissue factor pathway inhibitor(TFPI)는 TF가 작용하는 외인계 옹고 과정의 조절인자로서 옹고 억제 작용을 나타낸다. TF는 정상적으로 혈관내피세포와 말초혈액 단구에서는 발현되지 않으나 여러 염증성 매개체, 면역복합체, 내독소 등으로 자극하면 TF가 발현될 수 있다. 한편 관상동맥질환의 죽상동맥경화 병변내 TF가 검출되며 죽상동맥경화판이 파열되면 병변내 존재하는 TF가 순환혈액에 노출되면서 혈전 형성에

기여할 것으로 추측되고 있다.

본 연구에서는 관상동맥질환에서 임상적 진단 및 혈관협착에 따른 혈장 TF와 TFPI 농도 차이를 조사하고, 관상동맥질환자의 혈장에 의해 말초혈액단구에서 TF가 발현되는지를 알아보고자 하였다. 관상동맥협착의 병변이 확인된 65명의 관상동맥질환자와 22명의 정상

인에서 TF와 TFPl의 혈중 농도를 효소면역법으로 측정하였다. 또한 관상동맥질환자의 혈장으로 말초혈액 단구를 반응시켜 TF 발현정도를 유세포 분석기와 Northern blot으로 측정하였다. 아울러 혈장 칼슘재첨가법으로 응고활성도(procoagulant activity:PCA)를 검사

하였다.

혈장 TF의 농도는 정상대조군(187.3±108.7 pg/7m1)에 비해 관상동맥질환군(303.6±134.1 pg/m1)이 의미있게 놓았고(p=0.001), 환자군에서 임상적 진단에 따른 TF의 농도는 불안정형 협심증(360.9±166.9 pg/m1), 급성심근경색증(288.5±131.4pg/m1), 안정형 협심증(262.5±77.2 pg/ml)순으로 의미있는 차이가 관찰되었다(p=0.001). 관상동맥협착의 정도에 따른 TF의 농도는 두 혈관 질환(312.7±136.0pg/m1), 단일혈관(301.3±136.4 pg/m1)

및 세 혈관 질환(295.0±135.7 pg/ml) 별로 유의하게 차이가 있었다(p=0.006). 혈장 TFPI의 농도도 환상동맥질환군(85.2±48.6ng/m1)이 정상대조군(65.0±29.0 ng/ml)에 비해 의미있게 높았다(p=0.015). 관상동맥질환군에서 TFPI의 평균 농도는 급성심근경색증(100.1±77.0 ng/m1), 불안정형협심증(78.1±27.2 ng/m1), 안정형 협심증(77.1±32.8 ng/71) 순으로 높았으나 통계학적으로 유의한 차이가 없었으며(p=0.179), 관상동맥협착의 정도에 따른 차이도 없었다(p=0.258). 관상동맥질환군에서 혈장 TF의 농도와 나이, 혈중 콜레스테롤 ,중성지방, 고밀도지단백 콜레스테롤, lipoprotein(a), C-반응성 단백(CRP) 및 섬유소원과는 통계학적으로 외미있는 상관성이 없었다(p>0.05). 혈장 TFPI외 경우 CRP와 상관성이 관찰되었으며(r=0.350, p=0.01), 나머지 지표와의 상관성은 없었다. 환자군에서 성별에 따른 혈장 TF 및 TFPI 농도의 차이는 관찰되지 않았다(p=0.66, p=0.45). 유세포 분석기를 이용하여 전처리를 하지 않은 정상인 말초혈액단구에서 TF 항원을 측정한 결과 TF가 검출되지 않았으며, 관상동맥질환자의 혈장으로 처리한 경우 TF 항원의 발현 양성율은 34.6±10.7%(relative fluorescence index:9.6±3.6)로서 정상인 혈장으로 처리한 경우인 23.2±10.2%(relative fluorescence index:4.9±3.5)에 비해 유의하게 높았다(p=0.001). 또한 말초혈액 단구의 PCAf를 측정한 결과, 관상동맥질환군에서 정상대조군보다 의미

있게 활성도가 높았다(PCA 1.28±0.22 vs 1.12±0.12, p<0.001). 말초혈액 단구에서 TF mRNA의 Northern 분석 결과, 양성대조로서 맣초혈액 단구에 LPS를 처리한 경우 TFmyNA band가 진하게 관찰되었고 전처리를 하지 않은 정상 말초혈액 단구에서는 TF mRNA band가 관

찰되지 않았다. 관상동맥질환자의 혈장으로 전처리를 한 경우 정상대조군보다 TF mRNA band가 진하게 관찰되었다.

이상의 결과를 종합해 볼 때, 관상동맥질환은 TF가 증가한 응고항진상태이며, 아울러 관상동맥질환자의 혈장으로 말초혈액 단구에서 TF 발현이 유도됨을 관찰하였다. 이는 단구를 통한 TF의 발현이 관상동맥내 혈전 형성의 한 유발인자로 작용할 수 있음을 시사하며 향후 TF 발현 유도물질 규명 및 이에 대한 치료제 개발 등이 더욱 연구되어져야 할 것으로 생각된다.

Tissue factor(TF) plays a central role in the extrinsic pathway of coagulation by binding coagulation factor Ⅶ/Ⅶa. Tissue factor pathway inhibitor(TFPI) is the activated factor X dependent inhibitor of TF-induced coagulation. TF antigen was

identified in the smooth muscle cells, foam cells and monocytes of human atherosclerotic plaques. TF is not normally expressed on intravascular cells in the resting state but it can be induced in monocytes and endothelial cells by various inflammatory mediators and immune complexes. This suggests that TF expressed by peripheral monocytes may play a role in the hypercoagulability of the coronary artery disease(CAD).

The aims of the present study were to investigate 1) whether the plasma levels of TF and its specific inhibitor TFPI are correlated with the various clinical conditions of the CAD, 2) whether TF is expressed on peripheral blood monocytes by treatment with plasmas from patients with CAD.

Plasma levels of TF and TFPI were measured in 65 CAD patients and 22 healthy controls by ELISA method, and TF induction on monocytes in vitro in the presence of these plasma samples were assessed by flow cytometric analysis using anti-human TF and CDl4. We also examined procoagulant activity in peripheral blood mononuclear cells(PBMC) by plasma recalcification assay. TF mRNA expression by PBMC was also examined using Northern analysis.

Significantly elevated levels of both TF and TFPI were found in CAD patients, compared to those of 22 healthy controls (303.6± 134.1 vs. 187.3± 108.7pg/ml, p=0.001; 85.2 ± 48.6 vs. 65.0± 29.0 ng/ml, p=0.015, respectively). Unstable angina patients showed higher TF plasma level (360.9± 166.9 pg/m1) than that of

patients with acute myocardial infarction (AMI) (288.5± 131.4 pg/m1) and stable angina (262.5±77.2 pg/ml). TFPI plasma levels in AMI were higher(100.1 ±77.0 ng/ml) than those in unstable angina (78.1 ± 27.2 ng/ml) and stabl e angina (77.1 ±32.8 ng/ml). Plasma TF levels were not correlated with age, total cholesterol,triglyceride, HDL-cholesterol, lipoprotein(a), fibrinogen, or C-reactive protein. A positive correlation, however, was observed between TFPI and CRP (r=0.35, p=0.01).

Normal monocytes incubated in CAD plasmas than those incubated in control plasmas were found to show increased TF antigen expression (23.2± 10.2 vs. 34.6± 10.7, p<0.001) on flow cytometric analysis. Peripheral blood mononuclear cells(PBMC) also expressed more procoagulant activity(PCA) when incubated in CAD plasmas than in control plasmas(1.28±0.22 vs. 1.12±0.12, p<0.001). In Northern analysis, TF transcripts(3.1 & 2.3 kb) were absent in untreated PBMC, while TF mRNAs were detected in PBMC treated with lipopolysaccharide(LPS) and human thrombin. The

intensity of bands was higher in PBMC treated with plasma from patients with coronary artery disease(CAD) than that of normal control.

In conclusion, plasma levels of TF increased in patients with CAD, and TF antigen expression was higher when PBMC were incubated with plasmas of CAD patients than with plasmas of healthy control. These data suggest that hypercoagulability in CAD may be associated with elevated TF circulating levels and TF expression on monocytes may play an important role in the thrombogenesis in CAD.
