최근에 활성산소가 다른 염증성 질환에서와 같이 췌장염에서도 병태생리에 관여한다는 여러 근거가 제시되고 있다. 활성산소가 급성췌장염의 병인으로 작용하는 기전으로는 호중구를 활성화시키고 혈관내피에 흡착토록 하여 순환장애를 유발하며, 활성화된 호중구는

myeloperoxidase, elastase 및 peroxidase등의 효소와 prostaglandins, leukotrienes 및 활성산소를 재분비하여 염증을 유발하는 것으로 알려져 있다. 따라서 활성산소를 제거하거나 또는 생성을 억제시켜서 췌장염을 치료할 수 있을 것으로 기대된다. 기존의 췌장염의 치료를 위한 실험 및 임상연구에서는 superoxide dismutase(SOD), catalase, dimethyl sulfoxide(DMSO), allopurinol, selenium등의 항산화물질을 시도하여 왔으나 연구자에 따라 상반된 결과를 보고하기도 하였다. 본 연구는 췌장염의 병인으로 활성산소의 역할을 규명하고, 투여 후 세포 내로의 투과가 비교적 용이한 rebamipide, N-acetyl-cysteine, allopurinol,β-carotene, α-tocopherol 및 vitamin C등 저분자항산화물질의 효과를 비교 관찰하여 췌장염의 치료제로의 개발 가능성을 규명하고자 시행하였다.

본 연구를 위한 실험적 췌장염 모델로는 흰쥐를 이용한 cerulein유발 부종성 췌장염과, 콜린결핍 에티오닌(choline-deficient ethionine-supplemented, CDE)식이를 이용한 마우스의 괴사성 출혈성 급성췌장염 모델을 사용하였다. 실험적 췌장염 유발 후 췌장조직 내

활성산소 반응물질(lipid peroxide) 및 항산화방어 체계(glutathione, glutathione peroxidase) 변동, 혈청 amylase 및 cytokine의 변동, 그리고 조직학적 변화를 검색하였다. 본 연구의 결과는 다음과 같다.

Cerulein으로 유발한 췌장염에서 대조군에 비하여 혈청 amylase치와 췌장 조직 내 지질과산화물의 함량이 증가하였으며, glutathione의 함량은 저하하였다. N-acetyl-cysteine은 cerulein 유발 췌장염에서의 혈청 amylase 상승을 억제하였으며, 췌장조직의 부종 및 공포화를 경감시켰다. β-Carotene은 cerulein 유발 췌장염에서의 혈청 amylase 치의 상승을 유의하게 감소시켰고, 췌장조직 내 glutathione의 고갈을 억제시켰다. Rebamipide와 allopurinol은 cerulein 유발 췌장염에 별다른 영향을 주지 않았다.

CDE식이는 마우스에서 췌장염을 유발하여 혈청 amylase치의 상승과 부종을 초래하였다. 혈청 TNF-α, IL-6 및 IL-1β이 증가되었으며 지질과산화물의 함량을 유의하게 증가시켰다. α-Tocopherol과 vitamin C의 병용 투여는 CDE유발 췌장염에서 췌장조직의 부종을 억

제하였으며, 지질과산화물 함량 증가와 혈청 amylase 및 염증성 cytokine의 증가를 억제하였다. Rebamipide는 혈청 amylase와 염증성 cytokine의 증가를 억제하였으나 allopurinol과 N-acetyl-cysteine 등의 다른 항산화물질은 통계적으로 유의한 결과를 나타내지 않았다.

이상의 결과로 보아 활성산소는 cerulein 유발 췌장염과 CDE식이 유발 췌장염의 병인으로 작용하며, 저분자항산화물질은 췌장염에 대한 치료제로서 가능성을 제시하여 주어 앞으로 이에 관한 임상시험 등의 연구가 필요하다고 생각한다.

The mechanisms responsible for premature, intrapancreatic activation of digestive zymogens is poorly understood, but it is generally believed that this phenomenon is a early critical event in the pathogenesis of acute pancreatitis. Recently, it is suggested that pancreatic oxidative stress is important in the early phase and in the progress of experimental human pancreatitis, but effects of antioxidants on pancreatitis are still controversial.

The purpose of the present work was to evaluate the role of oxidative stress and the efficacy of low molecular weight antioxidants(rebamipide, N-acetyl-cysteine, allopurinol, β-carotene, α-tocopherol and vitamin C) in different experimental

models of pancreatitis, those are cerulein-induced pancreatitis in the rat and choline-deficient ethionine-supplemented(CDE) diet-induced pancreatitis in the mouse. The changes of amylase, TNF- α, IL-6 and IL-1β in blood, and pancreatic histopathology and contents of glutathione, glutathione peroxidase and lipid

peroxide in the pancreatic tissue were determined. Microphotogram of pancreatic tissues showed edema and vacuolization variably in both models of pancreatitis, but necrosis and other inflammatory changes were prominent in CDE-induced pancreatitis.

Amylase and inflammatory cytokines were elevated in blood. N-acetyl-cysteine lowered the increase of serum amylase level in cerulein-induced pancreatitis.

β-Carotene lowered serum amylase and prevented the depletion of tissue glutathione in cerulein-induced pancreatitis, Rebamipide ameliorated the CDE diet-induced pan-creatitis and lowered the increase of serum amylase and cytokines. The combination of α -tocopherol and vitamin C decreased markedly amylase and cytokines in blood, and decreased lipid peroxide in pancreatic tissue, in CDE diet-induced pancreatitis.

In conclusion, oxygen free radicals may be involved in pancreatitis and low molecular weight antioxidants have protective effect on pancreatitis. Further studies are need to be applied for the clinical pancreatitis.
