정신보건 정보시스템의 모델개발 : 지역사회 정신보건센터 중심으로

정신질환과 관련하여 질병의 원인을 밝혀내기 위한 연구가 전 세계적으로 지난 80여 년간 꾸준히 진행되고 있다. 선진외국의 경우 1960년대부터 과거의 입원, 수용 위주의 정신질환 관리체계에서 탈피하여 지역사회 중심의 정신질환 관리체계로 전환하고 있다. 우리

나라의 경우는 1960년대부터 정신질환자 관리사업을 보건사회부의 정책사업에 포함하기 시작하였으나, 현재 정신질환자의 대부분이 질적으로 낮은 수준의 보호시설에 수용되어 있는 수준이다. 이에 경기도에서는 1997년부터 지역사회 정신보건센터 또는 보건소 내에

정신보건과를 신설하여 시범사업을 실시하고 있다.

따라서 본 연구의 목적은 국내외의 문헌고찰을 통하여 지역사회 정신보건 사업에 관한 전산화 현황을 분석하여 현재 보건소 정신보건 사업을 대상으로 한 시스템 분석 및 설계를 통하여 정신보건 정보시스템의 모형을 제시한다. 또한 국외에서 개발한 정신보건모형

을 참조하여 향후 개발할 정신보건 시스템의 모형을 제시하기 위함이다.

이를 위하여 본 연구는 국내외의 관련 문헌고찰을 통하여 지역사회 정신보건사업에 관한 현황 및 업무를 분석하였다. 이것은 시스템 분석 및 설계 단계에서 자료흐름도, 계층적 입출력 모형, 개체-관계성도로 작성되어 전체적인 시스템의 구성요소 및 그들간의 연관관계를 모형화하고 데이터를 형식화 한다.

정신보건 정보시스템은 윈도우 95 환경에서의 프로그램 개발도구인 Visual Foxpro 5.0을 이용하여 개발되었다. 전체적으로 본 시스템은 Barry(1996), Kenneth(1997) 등 외국 주요 연구의 정신과 진료와 관련된 임상정보시스템 (Clinical Information System, CIS)

을 참고로 하여 환자의 정신상태, 사회력 등 정신질환의 증상평가, 목표설정, 치료계획수립, 치료과정 및 평가 등의 순서에 의해 진행된다. 특히 우울증과 관련하여는 1993년 시도된 Kenneth의 연구를 바탕으로 하여 Beck, CES-D, SCL-90R 등의 검사에 대한 자동평가

및 그래픽 지원이 가능하게 함으로써 환자상태의 변화추이를 쉽게 관찰할 수 있도록 하였다. 본 연구에서 구축된 정신보건 정보시스템은 총 22개의 DB와 40개의 화면으로 구성되어 있다. 정신보건 정보시스템의 향후 발전방향으로는 현재 개발된 프로그램의 개선모형,

보건소 정보시스템과의 통합모형, 인터넷을 이용한 정신보건사업, 지역사회 정신보건 네트워크의 구성 등을 제시하였다.

현재의 정신보건사업은 광역형 정신보건사업과 보건소형 정신보건사업의 두가지 제도가양립됨으로 인해 지역사회 정신보건사업의 기획, 평가, 정보관리체계개발, 위기개입, 프로그램의 개발 등에 대한 투자가 중복되거나 혼선을 빚고 있다. 또한 지역사회를 대상으로 한 정신보건사업의 표준안이 확립되지 않은 상태이다. 따라서 이와 같은 정신보건사업의 제한점을 극복함으로써 지역사회 중심의 정신보건사업을 효율적으로 수행하기 위한 정보체계의 개발이 가능하며 계획성 있는 사업이 계속되어질 것이다.

A study to find out the cause of diseases related to mental illness has been widely conducted throughout the world for the past 80 years. Since the 1960's, developed countries have been shifting their focus from a system of hospitalization and accommodation to a more efficient system that puts its focus in the community. In Korea, mentally ill patients are accommodated in poor quality shelters even though the management of them was included in the social policy of the Public Health Department since the 1960's. Thus, community health center and public health center were established in Kyunggido in 1997 on a demonstration basis The purpose of this study is to show a mental health information system model through literature reviews of the current status of computerization for a community mental health project. In addition, a newly developed model of mental health system that is referred from a mental health model made in a developed country will be presented.

To complete this project, present conditions and operation of the community mental health business was analyzed using Korean and foreign literatures. This formats the data and modelizes the relationship between Data Flow Diagram (DFD), Hierachy Plus

Input-Process-Output (HIPO), and Entity-Relationship Diagram (E-R Diagram) that were constructed in the process of system analysis and planning.

The mental health information system was developed with Visual Foxpro 5.0 which has the ability to make programs in Windows 95. The stability and sociality of a mentally ill patient are first analyzed, then a goal is established. Then a plan for a proper treatment is written, and the patient is treated and analyzed in such process. Clinical Information System (CIS) that is related to the main studies of Barry (1996) and Kenneth (1997) was used as a reference guide to make such process.

A study of Kenneth done in 1993, especially about Schizophrenia, made it possible to observe the change in conditions of the patient easily through a possible automatic evaluation and their graphics of Beck, CES-D, and SCL-90R. The mental health

information system built in this study consists of 22 databases and 40 screens. For future development in the system, the following ideas for improvements are mentioned: an improved model of the constructed program, a model of the system integrated with

the public health center's, a new mental health system using internet, and networking the community health center.

The existing mental health activities are not very well organized in areas of investment, planning, evaluation, development of a management system due to an overlapping system in a wide area and community mental health center. In addition,

a standard suggestion of the mental health activity that puts its object on the community, is not confirmed yet. Therefore, a development of an information system that is effective in community health centers will be possible and a plan with a

bright future will be made continuously as the limits of mental health activity is overcome.
