인간의 하악골은 하악궁(mandibular arch)에서 유래되며, 하악궁의 발생과정에서 막대기 모양의 멕켈연골(Meckel's cartilage)이 양측으로 나타나고, 이 연골은 전방으로는 하악궁의 정중부까지, 후방으로는 중이의 추골(malleus) 및 침골(incus)까지 뻗어 있다. 발생이 진행되면서 멕켈연골의 전방 부분은 흡수되고, 후방 끝부분은 골화되어 추골와 칩골이 되며, 중간 부분은 퇴화되어 없어지지만, 이 부분의 연골막은 접형하악인대(sphenomandibular ligament)를 형성한다. 따라서 멕켈연골이 어떻게 발생하고, 퇴화되는지를 안다

는 것은 하악골과 그와 관련되는 구조물들의 형성을 이해할 수 있고, 또한 하악골의 발생과 연관되어 나타날 수 있는 기형 등을 이해하는데 도움이 될 것으로 생각된다.

이에 한국인 배자와 태아에서 멕켈연골의 발생과 퇴화 과정을 조직학적 및 미세해부학적 방법으로 조사하여 멕켈연골이 하악골의 발생 때 직접 관여하는지, 멕켈연골의 퇴화 과정에서의 잔유물인 접형하악인대의 발생 과정을 연구함으로써 멕켈연골과 하악골 발생

과 연관해서 나타날 수 있는 악골기형과 이에 따른 증상을 이해하는 이론적 배경을 얻고자 본 연구를 시행하여 다음과 같은 결과를 얻었다.

1. 하악골의 골화는 발생 19기에 하악궁의 전방부와 멕켈연골의 외측에서 처음으로 관찰되었다.

2. 하악골이 골화되어 길이가 성장할 때 처음으로 골화가 일어나는 부분은 연골과 근접해 있었으나, 나머지 부분은 연골과 떨어져 있었으며, 하악골체 부분에서 멕켈연골 자체의 골화는 관찰되지 않았다.

3. 멕켈연골은 발생 17기에 처음으로 출현하였으며, 멕켈연골은 발생 9주에 처음으로 퇴화되기 시작하였고, 하악골체 부분과 접형하악인대에서는 발생 29주 이후에 연골을 관찰할 수 없었다.

4. 멕켈연골의 중간 부분은 퇴화가 진행됨에 따라 섬유성 조직으로 대치되면서, 소설(lingula)에서 인고실렬(squamotympanic fissure)에 이르는 접형하악인대가 되었다.

이상의 결과를 보아, 멕켈연골의 전방부는 하악골이 처음으로 골화되기 시작하는 부위와 근접해 있었으나 하악골이 골화되면서 하악골의 내측에서 흡수되어 소실되는 일시적 구조물로 생각되었다. 한편 멕켈연골의 후방부는 연골내골화(endochondral ossification)

를 거쳐 추골과 침골을 이루었으며, 하악골과 추골 사이의 중간 부분은 태아기 동안 퇴화되어 접형하악인대가 됨을 알 수 있었다.

[영문]

The development and fate of the human Meckel's cartilage were studied by microscopic examination. Nine human embryos from stage 16 to stage 23 and twenty-three human fetuses from 9 weeks to 29 weeks of fetal age were used in this study. The embryos had been staged according to Carnegie's stages. The age of fetuses were assumed according to crown rump length and foot length.

The results were as follows:

1. The intramembranous ossification of the mandible was first observed in the region of ventral ends and lateral to the Meckel's cartilage at stage 19.

2. The ossifying mandible and the Meckel's cartilage were close to each other in the region of initial ossification, but fairly separated in the other portions during mandibular ossification and growth in length. The ossification of the Meckel's cartilage itself was not observed.

3. First evidence of Meckel's cartilage was seen in stage 17. The Meckel's cartilage began to degenerate at 9th week and disappeared completely in the mandibular body and sphenomandibular ligament after 29th week of development.

4. In the middle portion of the Meckel's cartilage, catilage converted into sphenomandibular ligament after degeneration of cartilage components and it extended between lingula and squamotympanic fissure.

In summary, the ventral part of Meckel's cartilage has a spatial relation with ossifying mandible, but the cartilage does not seem to play a role in the ossification of mandible. The Meckel's cartilage is considered to be temporary structure which is absorbed at the inner side of the ossifying mandible. The dorsal end of Meckel's cartilage developed into malleus and incus through the endochondlal ossification. During the fetal periods, the middle part of Meckel's cartilage between lingula and malleus degenerated and developed into sphenomandibular ligament.
