관상동맥 협착증의 치료법으로 경피적 관상동맥 확장성형술이 널리 시술되고 있으나 스텐트 삽입술이 도입된 이후에도 여전히 20%의 재협착율을 보이고 있다. 재협착의 여러 기전 중 혈관 평활근 세포의 증식은 주요 기전의 하나로 알려져있다. 재협착을 억제하기 위한 다양한 전신 약물 요법이 시도되었으나 동물모델에서만 성공을 거두었을 뿐 임상시험에서는 모두 실패하였다. 약물의 국소 전달법은 원하는 부위에 고농도의 약물을 전달할 수 있는 방법으로서 전신적 약물 투여에 따른 부작용을 최소화할 수 있어 중재술 후 재협착의 예방에 이용될 수 있는 하나의 대안으로 개발되고 있다. 본 연구는 동맥경화의 발생을 억제하며 혈관 평활근 세포의 증식 및 이주를 억제한다고 알려져 있는 에스트로겐을 약물 전달체인 리포좀을 이용하여 흰쥐 경동맥 풍선 손상 모델에서 국소 투여하여 신생내막 증식에 미치는 영향 및 그 기전을 알아보고자 하였다.

약물 전달체로서 리포좀을 개발한 후, 이를 17β-에스트라디올(17β-estradiol)과 결합시켜 17β-에스트라디올-리포좀 복합체(이하 E**2 -리포좀)를 제조하여 실험에 이용하였다. B**2 -리포좀은 egg phosphatidylcholine, cholesterol, polyethyleneglycol-phosphatidylethanolamine, 17β-에스트라디올을 혼합하여 최종 리포좀내 지질 농도가 10mg/ml, 17β-에스트라디올 농도가 66μM, 크기는 200nm 이하가 되도록 제조하였다. 실험 동물로

는 350±30g의 Sprague-Dawley 흰쥐를 사용하여 생리적 식염수 투여군 22마리(1군), 리포좀 투여군 46마리(2군), E**2 -리포좀 투여군 46마리(3군)의 세 군으로 나누었다. Ketamine(80mg/kg)을 복강 내로 투여하여 마취시킨 후 좌경동맥에 2F Fogarty 풍선도자로 손상을 가하고, 손상을 가한 동맥의 원위부 및 근위부를 일시 결찰하여 해당 약물을 0.2ml씩 3분에 걸쳐 동맥내강으로 투여하였다. 세 군 모두 손상 후 7일(4마리), 14일(6마리), 21일(12마리)에 관류고정으로 희생시켜 양측 경동맥을 적출한 후 H-E 염색 및 Lawson's elastic-van Giesen 염색을 시행하였다. 형태계측 검사로는 가장 증식이 많이 된 부위를 택하여 광학현미경하에서 측정하여 중막면적, 신생내막면적, 내막면적 대 중막면적의 비(내막/중막 비)를 구하였다. 또한 손상 후 1일, 3일, 7일, 14일에 2군과 3군에서 각 4마리씩 희생시켜 5-bromo- 2'-deoxyuridine(BrdU) 염색 및 proliferating cell nuclear antigen

(이하 PCNA) 염색법으로 신생내막 및 중막의 표지지수(labelling index)를 구하였다.

세 군 모두에서 유의한 염증반응이 관찰되지 않았으며, 생리 식염수 투여군과 리포좀 투여군 사이에는 손상 후 7일, 14일, 21일에 중막 및 신생내막 면적에 유의한 차이가 없었다. E**2-리포좀 투여군에서는 리포좀 투여군에 비하여 7일, 14일, 21일에 내막/중막 비가 각각 17%, 30%, 34% 감소하였다(각각 p<0.05). 손상 후 3일에는 리포좀 투여군보다 E**2 -리포좀 투여군에서 중막의 BrdU 표지지수 및 PCNA 표지지수가 낮았다(P<0.05). 손상 후 7일에는 E**2-리포좀 투여군에서 신생내막의 면적은 적었으나(P<0.05) 양군간에 중막 및 신생내막의 BrdU 표지지수 및 PCNA 표지 지수에 유의한 차이가 없었다.

이상의 결과를 종합하여 볼 때 새로이 개발된 nanomemeter 크기의 E**2-리포좀은 국소전달시에 흰쥐 경동맥 조직과 적합성이 있었으며, 풍선 손상을 가한 경동맥에 E**2-리포좀을 국소 투여한 결과 손상 후 초기에 중막의 혈관 평활근 세포의 활성화 및 증식을 억제하여 이후에 발생하는 신생내막의 형성을 간접적으로 억제하는 효과가 있어 향후 동맥의 풍선손상 후 발생하는 신생내막 증식의 억제에 이용될 수 있으리라 생각된다.

[영문]

Objective: Restenosis remains the major limiting factor in longterm efficacy of nonsurgical intervention of coronary artery obstructive disease as it occurs at least 20% even after the introduction of stents. Vascular smooth muscle cells play key roles in the development of atherosclerosis and neointimal formation after endothelial injury. Estrogen has been reported to inhibit migration and proliferation of vascular smooth muscle cells in vitro and in vivo. All the attempts to prevent restenosis by systemic administration of drugs known to be effective in animal models have been foiled in human trials. Sustained local delivery rather than systemic administration represents a potential alternative because it can achieve higher tissue drug levels at the site of balloon injury avoiding systemic side effects. We have investigated the effect and mechanism of nanoparticulate sustained-release carrier system using liposome incorporating 17β-estradiol(E**2) on neointimal formation in rat carotid artery balloon injury model.

Methods: 17β-estradiol benzoate, egg phosphatidylcholine, cholesterol, polyethylene glycolphosphatidylethanolamine were mixed to produce E**2-liposome formula where the final concentration of lipids and E**2 were 10mg/ml and 66μM.

The size of the particle was less than 200nm confined by scanning electron microscope. Rat carotid artery balloon injury model was used with Sprague-Dawley rats weighing 350±30g. To investigate the biocompatibility of liposome and the effect of E**2 -liposome, rats were divided into 3 groups. The saline group(n=22) received 0.2ml of normal saline, liposome group(n=46) received 0.2ml of plain liposome and E**2 -liposome group(n=46) received 0.2ml of E**2 -liposome. 1) Rats from the three groups were sacrificed with perfusion fixation at 7(n=4), 14(n=6), and 21(n=12) days after injury, respectively. Morphometric analysis was performed for calculating medial area, neointimal area and I/M (intimal area/medial area) ratio. 2) The other rats from liposome and E**2-liposome groups received 100mg/kg of 5-bromo-2'-deoxyuridine(BrdU) at 25, 9 and 1hr before sacrifice at 1(n=4), 3(n=4), 7(n=4), and 14(n=4) days after injury. BrdU and proliferating cell nuclear antigen(PCNA) stains were performed for these samples to elucidate a mechanism of inhibitory effect of E**2 .

Results: 1) There was no increase in the neointimal area in liposome group compared with saline group at 7, 14, 21 days after injury, respectively. 2) There was 17%, 30%, and 34% reduction of I/M ratio in E^^2 - liposome group compared with lipsome group at 7, 14 and 21 days after injury, respectively. 3) BrdU stain revealed that at day 3, labelling index(LI) of media was lower in E^^2 - liposome than in liposome group(p<0.05), and at day 7, LI of neointima was not significantly different between the two groups despite smaller neointimal area in the E^^2 - liposome group. 4) PCNA stain revealed simlar but slightly higher LI of intima and media at each time, confirming the result of BrdU stain.

Conclusions: Nanoparticulate liposome formula apears to be biocompatible. Local Intraluminal infusion of E^^2 - liposome formula after balloon injury of rat carotid artery significantly decreased neointimal formation. The mechanism seems to be the inhibitory effect on the proliferative response of smooth muscle cells in media at an early stage of injury. This formula apears to show potential for clinical applications in the prevention of neointimal formation followeing balloon angioplsty.
