외상이나 화상 등 감염이외의 원인에 의하여 패혈증과 유사한 임상증상이 유발되는 현상을 전신성 염증 반응 증후군 (systemic inflammatory response syndrome, SIRS) 이라 한다. 전신성 염증 반응 증후군을 유발하는 병태 생리에는 다양한 싸이토카인이 관여하는데, 그 중에서도 tumor necrosis factor-alpha (이하 TNF-α)가 중요한 역할을 하는 것으로 알려져 왔다.

그러나, 외상이 전신성 염증반응 증후군의 발생에 중심적 역할을 하는 TNF-α의 발현에 직접적인 연관이 있는지는 아직까지 잘 알려져 있지 않다. 본 연구에서는 외상 초기에 내원한 환자에서 혈청 TNF-α를 측정하여 전신성 염증반응 증후군의 발생과 혈청 TNF-α와의 관계를 알아보고자 하였다.

1998년 1월부터 1998년 3월까지 연세대학교 원주의과대학 원주기독병원 응급센터로 내원한 다발성 외상 환자 중 외상 받은 후 2시간이 경과하지 않은 21예의 환자를 대상으로 하였다. 과거력 상 내분비 질환, 악성 질환, 자가면역 질환, 류마티스성 관절염, 장기이

식의 과거력, 패혈증, 18세 이하이거나 70세 이상인 경우, 경한 외상에 의해 Injury severity score (이하 ISS) 점수가 3점인 예는 연구대상에서 제외되었다. TNF-α의 정상 값은 과거력 상 특이 병력이 없는 정상인 15예의 혈청을 채취하여 구하였다.

모든 환자에게서 혈압, Glasgow coma scale (이하 GCS), revised trauma score (이하 RTS), ISS를 조사하였다. TNF-α의 측정을 위하여 6 ml 의 혈액을 채취하여 분당 2,500 회의 속도로 5분 동안 원심 분리하여 혈청을 분리한 후 -70℃에 냉동보관 하였다. TNF-α는 monoclonal anti-human TNF-α antibody를 이용한 "샌드위치 모양"의 효소결합면역흡착검사를 이용하였다.

연구 대상은 전신성 염증 반응 증후군이 발생한 군 (이하 SIRS 군)과 전신성 염증 반응 증후군이 발생하지 않은 군 (이하 non-SIRS 군)으로 나누었으며, 두 군간 임상적 자료와 혈청 TNF-α를 비교하여 다음과 같은 결과를 얻었다.

1. 연구대상자 전체 21예(남 13예, 여자 8예)중 SIRS 군이 10예(48%)였으며, non-SIRS 군은 11예(52%)였다.

2. 외상 후 SIRS 군에서 혈청 TNF-α 농도는 정상 (8 ± 2 pg/ml) 보다 외상 후 3일까지 지속적으로 증가 (내원시 14.5 pg/ml, 외상 12시간 후 31.1pg/ml, 외상 24시간 후 19.2 pg/ml, 외상 2일 후 21.2 pg/ml, 외상 3일 후 17.1 pg/ml) 되어 있었다.

3. SIRS 군에서의 혈청 TNF-α 농도(31.1 pg/ml)는 외상 후 12시간에서만 non-SIRS 군(13.4 pg/ml)보다 증가되어 있었다 (p < 0.01).

4. ISS와 혈청 TNF-α 농도는 상관관계가 없었으나 (R=0.4, p > 0.05), 중증 외상환자의 혈청 TNF-α 농도가 경증의 외상 환자에서 보다 높았다 (p < 0.01).

이상의 결과로 다발성 손상환자에서 외상 후 혈청 TNF-α의 지속적인 상승이 전신성 염증 반응 증후군의 발생과 연관이 있는 것으로 생각되며, 외상의 중증도가 혈청 TNF-α 발현과 연관이 있는 것으로 생각된다.

[영문]

The systemic inflammatory response syndrome, as defined recently by critical-care specialists, may result from various etiologies including infection, burn, or trauma. It is well known that cytokines plays a major role during development of systemic inflammatory response syndrome. Tumor necrosis factor-α (TNF) have been suggested as the most important cytokine in triggering and manifest systemic inflammatory response syndrome. However, during the development of systemic inflammatory response syndrome caused by etiologies other than infection, the exact role of TNF-α remained unknown.

The purpose of this study was to determine whether TNF-α is associated with the development of systemic inflammatory response syndrome caused by multiple trauma. Twenty one patients with multiple trauma within 2 hours after traumatic insult were enrolled in this study. Multiple blood sample were serially drawn to measure serum TNF-α level on admission, 12 hours, 24 hours, and every day until 5 days after injury. Serum TNF-α was measured by ELISA ("Sandwich type"). Blood samples of fifteen volunteers were used as a reference value for serum TNF-α.

Among 21 trauma patients, systemic inflammatory response syndrome was developed in 10 patients. Clinical characteristics and serum TNF-α levels were compared between SIRS group and non-SIRS group.

The results were as follows :

1. Serum TNF-α levels of SIRS group were persistently elevated above reference value until 3 days after on admission. Peak serum TNF-α level was observed 12hours after admission.

2. Serum TNF-α level at 12 hours after admission was higher in SIRS group than non-SIRS group (p < 0.05).

3. There was no significant correlation between injury severity score and TNF-α levels on the regression analysis, all patients with ISS higher than 16 had SIRS. No one had SIRS among patients with ISS less than 16.

In conclusion, the result of this study suggests that persistent elevation of TNF-α and degree of injury severity are associated with the development of systemic inflammatory response syndrome in multiple trauma.
