성인 비만 남성에게 체지방율이 폐기능 및 산소섭취량에 미치는 영향

비만증은 동맥경화증, 고혈압, 허혈성 심질환, 당뇨병, 뇌졸증 등의 질병과 밀접한 관련이 있다. 비만에 의해 발생되는 생리적 변화는 흉곽과 복강의 지방 축적으로 호흡의 기계적 기능의 증가 및 호흡시 산소 소비는 증가하나, 호기예비량(expiratory reserve volume: ERV)은 감소하여 동맥혈 내의 가스의 확산이나 폐의 기계적인 기능에 이상을 초래한다.

일반적으로 비만이 심한 사람이 체중 감량 후에는 호기예비량과 폐활량이 개선되고 산소섭취량, 심박출량, 혈액량이 의의 있게 감소됨이 알려져 있다. 그러나 운동 중 최대 산소섭취량과 폐기능을 측정한 결과 비만인이 정상인에 비하여 최대 산소섭취량이나 폐기능에서 통계적으로 의의있는 차이가 없다는 연구도 있다. 또한 비만 여자에서는 체질량지수(body mass index: BMI)가 증가하면 폐기능이 감소한다는 연구와 함께 비만 남성에서는 관련이 없다는 주장도 있었다.

본 연구에서는 30세 이상 69세까지의 성인 남자 87명을 대상으로 체지방 측정계로 체지방 백분율(body fat %)을 측정한 다음, 체지방율 20% 미만을 대조군으로, 체지방을 20-25% 사이는 비만 Ⅰ군, 체지방율 25% 이상은 비만 Ⅱ군으로 분류한 후 체지방을 증가에 따른 안정시 폐기능 및 운동중 심박수와 산소섭취량의 차이를 비교 분석하여 다음과 같은 결과를 얻었다.

1. 폐활량(VC), 최대환기량(MVV)은 대조군에 비해 비만 Ⅰ군과 비만 Ⅱ군이 유의하게 낮았으며, 비만 Ⅱ군이 비만 Ⅰ군보다 더욱 낮은 결과를 보였다(P<0.01). 호기예비량(ERV)은 비만 Ⅱ군이 대조군과 비만 Ⅰ군에 비해 유의하게 낮았다(p<0.01).

2. 노력성폐활량(FVC), 노력성호기량 0.5초치(FEV^^0.5), 노력성호기량 1.0초치(FEV^^1.0), 노력성호기량 3.0초치(FEV^^3.0)는 비만 Ⅰ군과 비만 Ⅱ군이 대조군에 비해 유의하게 낮았으며, 비만 Ⅱ군이 비만 Ⅰ군보다 더욱 낮은 결과를 보였다(P<0.01).

3. 노력성호기량 1초율(%FEV^^1)도 대조군에 비해 비만 Ⅰ군과 비만 Ⅱ군이 유의하게 낮았다(p<0.05).

4. 최대호기유속(PEF)과 폐활량의 75%, 50%, 25% 지점에서의 호기유속인 FEF^^75%, FEF^^50%, FEF^^25%, 그리고 최대흡기유속(PIF)이 비만 Ⅱ군이 대조군에 비해 유의하게 낮았다(p<0.05, 0.01).

5. 운동중 산소섭취량의 변화는 비만 Ⅱ군이 대조군 보다 운동 3분, 5분, 7분에 높았고(p<0.05, 0.01), 회복기에는 비만 Ⅰ군과 비만 Ⅱ군이 대조군보다 유의하게 높았다(p<0.01).

이상의 결과를 종합하여 보면, 체지방율이 높은 군에서 안정시 폐기능이 떨어지고 운동중 심박수와 산소섭취량이 증가하는 결과를 보이며, 이는 체지방을 증가가 폐용량을 감소시키고, 흉벽의 용압율을 감소시키며, 낮고 불규칙적인 호흡이 폐포의 환기량을 감소시킨 결과로 해석된다. 따라서 성인 비만인들이 체지방율을 감소시키면 폐기능을 향상시킬 수 있을 것으로 기대된다.

[영문]

The purpose of this study was to investigate the relationships of percent body fat on pulmonary function and oxygen consumption in adult men. The subjects consisted of 87 adults without diseases between 30 and 69 years old.

Subjects were divided into three groups : (1) control group of 33 men with less than 20% body fat, (2) obese group Ⅰ of 28 men with more than 20% and also less than 25% body fat, (3) obese group Ⅱ of 26 men with more than 25% body fat.

The results are as follows.

1. Vital capacity, maximal voluntary ventilation were significantly lower in obese groups(P<0.01). Expiratory reserve volume(ERV) were significantly lower in obese group Ⅱ than that of in control and obese group Ⅰ(p<0.01).

2. Forced vital capacity(FVC), FEV^^0.5sec, FEV^^1.0sec, and FEV^^3.0sec were significantly decreased in obese groups(p<0.01), and they were lower in obese group Ⅱ than in obese group Ⅰ.

3. FEV^^1.0% were significantly lower in obese groups than in control group(p<0.05).

4. Peak expiratory flow(PEF), peak inspiratory flow(PIF), FEF^^75%, FEF^^50%, FEF^^25% were significantly decreased in obese group Ⅱ compared to that of in control group(p<0.05, p<0.01).

5. Oxygen uptake at 3 minute, 5 minute, and 7 minute during exercise were significantly higher in obese group Ⅱ than that of in control group(p<0.05, p<0.01). Oxygen uptake at rest were also significantly higher in obese groups(p<0.01).

In this study, when the percent body fat increased, pulmonary function decreased. However during exercise, oxygen uptake significantly increased in obese groups. Thus the reduction in expiratory reserve volume causes abnormalities in lung mechanics, and that increase oxygen consumption of breathing in obese groups.
