날로 증가하고 있는 소규모 사업장은 대규모 사업장에 비해 작업조건이나 재정구조가 취약하고 보건관리를 제대로 받지 못하고 있어, 정부에서는 이들 사업장을 대상으로 막대한 예산의 국고지원사업을 실시하고 있다. 그러나 실제 소규모 사업장 근로자가 대규모 사업장 근로자에 비해 재해율은 높은 반면, 직업병 발생률은 유의한 차이를 보이지 않고 있다.

따라서 본 연구는 대규모 사업장 근로자와 소규모 사업장 근로자의 주관적인 건강상태와 의료이용실태를 비교함으로써 사업장 규모에 따른 건강수준의 차이를 평가해 보고자 하였다. 경인지역 1개 산업보건대행기관에서 1997년 9월부터 11월까지 보건관리대행 및 특수건강진단과 일반건강진단을 받는 500인 이상의 대규모 사업장 근로자 203명과 50인 미만의 소규모 사업장 근로자 133명을 대상으로 CMI 증상 점수와 의료비용을 자기기입식 설문조사를 통해 조사하였다.

수집된 자료는 SPSS/Win program을 이용하여 두 집단의 인구사회학적 특성과 직장관련 특성, 작업환경평가 점수, CMI 증상 점수 및 의료이용실태비교는 χ**2 , t-test를 실시하였고, CMI 증상 점수 및 의료비용의 영향 요인을 파악하기 위하여 상관분석과 ANOVA, 다중회귀분석을 실시하였으며, 다음과 같은 결과를 얻었다.

첫째, 대규모 사업장 근로자의 작업환경평가 점수는 20.19점, 소규모 사업장 근로자의 작업환경평가 점수는 17.67점으로 대규모 사업장 근로자에 비해 소규모 사업장 근로자가 근무작업환경을 양호하다고 평가하였다.

둘째, 대규모 사업장 근로자의 CMI 중상 점수는 61.58점, 소규모 사업장 근로자의 CMI 증상 점수는 57.17점으로 대규모 사업장 근로자에 비해 소규모 사업장 근로자의 주관적 건강상태가 양호하였다.

셋째, 대규모 사업장 근로자와 소규모 사업장 근로자의 일반질병유병율은 각각 12.6%, 11.7%로 통계적으로 유의한 차이가 없었다.

넷째, 결근 근로자 비율, 평균 결근일수 및 입원 근로자 비율은 대규모 사업장 근로자가 20.2%, 1.54일, 7%이었고, 소규모 사업장 근로자는 21.8%, 0.94일, 6.8%로 두 집단간에 통계적으로 유의한 차이가 없었다.

다섯째, 대규모 사업장 근로자와 소규모 사업장 근로자의 연간 의료비지출액이 각각 1인당 81,000원, 44,000원으로 대규모 사업장 근로자가 소규모사업장 근로자보다 의료비 지출이 많았다.

여섯째, 근무시간이 길수록, 작업환경평가 점수가 높을수록, 그리고 남자근로자보다 여자 근로자가 CMI 증상 점수가 높았다.

일곱째, CMI 증상 점수가 높을수록 연간 의료비용을 많이 지출하였다.

이상의 결과를 종합해 보면, 대규모 사업장 근로자가 소규모 사업장 근로자보다 CMI 증상 점수와 의료비용이 유의하게 높았으나, 다중회귀분석을 통해 혼란변수인 연령, 성별을 통제한 결과 대규모 사업장 근로자와 소규모 사업장 근로자의 건강수준은 차이가 없었다. 그러나, 소규모 사업장 근로자가대규모 사업장 근로자보다 CMI 증상 점수가 낮고 의료비 지출이 낮은 경향이 있어, 소규모 사업장 근로자들의 작업환경이 열악하고 건강상태가 좋지 않다는 기존의 생각과 차이가 있었다. 따라서 이러한 원인을 파악하고 효율적인 보건관리사업을 진행하기 위하여 소규모 사업장 특성에 관한 심도 깊은 연구가 이어져야 할 것이다.

Working conditions, financial structure and the health services offered in small scale industries(SSI) are inferior to those offered by large scale industries(LSI). Thus, the government is spending funds from the national treasury to provide health care far workers in SSI. Workers of SSI have a higher prevalence rate of occupational injuries than do workers of LSI. On the other hand, there is no difference in the prevalence rate of occupational diseases between workers of the two different settings. Therefore, this study was performed to compare the health status of workers between SSI rind LSI. The instruments used for the study were the revised Cornell Medical Index(CMI) and a questionnaire of medical costs for workers. The subjects for the study were workers from Kyungin industrial complex, 133 workers in SSI and 203 workers in LSI. Data were analyzed with SPSS/Win program using chi square and t-test for the demographic characteristics of the two groups, CMI scores and use of medical facilities, ANOVA and correlations to analyze relationships between variables, and regression analysis to determine variance.

The results of this study are as follows;

1. Concerning the work environment, it was found that workers in LSI evaluated the environment at 20.19 and those in SSI, at 17.63, the lower score indicating a more positive evaluation.

2. CMI score showed that workers in LSI load a mean score of 61.58 while those in SSI had a mean score of 57.17, again the lower score indicating a positive evaluation of subjective health status.

3. There was no significant difference in the workers' prevalence rate of occupational disease in LSI which had a rate of 12.6% and those in SSI, 11.7%.

4. The percentages of absentees, the average days absent from work and the number of workers admitted to hospitals over the past year were 20% workers, 1.54 days and 17% workers, respectively for workers in LSI and 21.8% workers, 0.94 days and 6.8% workers, respectively for those in SSI.

5. There was a significant difference between the average yearly medical costs far the two groups, with workers in LSI having a higher average cost of 81,000 won, and workers in SSI having an average cost of 44,000 won.

6. It was found that working hours, work environment, sex of worker contributed significantly to the variance in the CMI scores, with longer working hours, lower environmental scores contributing to higher CMI scores. Women had higher CMI scores

than men.

7. CMI scores contributed to the variance of medical costs, the higher CMI scores workers had, the higher medical costs they spent.

There was no significant difference between the two groups for CMI scores and medical costs in the regression analysis after controlling the sex and age of workers. It led to a conclusion that there was no difference in the health status of the two groups. However, workers in SSI as compared to workers in LSI, had lower CMI scores, lower medical casts per year even though the work environment and health status were described as less favorable. Therefore, in-depth research is needed to understand the reasons for these differences and to provide efficient health care services to workers in SSI.
