재활서비스의 개선전략을 수립하기 위하여 구조화된 설문지를 이용하여 서울 시내 2개 재활병원의 재활서비스를 비교하고 다차원척도법을 이용하여 서비스에 대한 인식을 분석하였다. 연구자료는 두병원을 모두 이용한 경험이 있는 환자나 보호자 82명을 대상으로 1997년 4월 1일부터 5월 10일 까지 자기기입식 방법으로 설문으로 설문 조사하였으며 주요 결과는 다음과 같다.

첫째, 인구사회학적 특성에 따라 재활서비스의 만족도 차이를 비교하여 남성이 여성보다 만족도가 높았으나 다른 항목들에서는 통계학적으로 유의한 차이를 보이지 않았다.

둘째, 질환별 서비스 만족도를 비교하여, 뇌성마비 환자의 재활서비스 만족도가 '진료비', '시설 및 환경', '이용성의 편리성', '설명 및 정보제공' 항목에서 뇌손상이나 척수손상환자의 만족도보다 낮았다.

셋째, 두 병원간의 재활 서비스 만족도를 8가지 평가 항목별로 비교하여, B 병원이 '시설 및 환경' '이용성의 편리성', '진료비' , '설명 및 정보제공', '전체적 평가' 항목 등 5개 항목에서, A병원은 "전문성" 항목에서 재활 서비스에 대한 만족도가 통계학적으로 유의하게 높았다. 세부항목별 비교에 있어서 A 병원이 B 병원보다 높았던 항목은 7개 세부항목인데 반해, B 병원의 경우 28개에서 A 병원보다 유의하게 높았다.

넷째, 다차원척도법을 이용하여 비교한 결과 B 병원의 뇌손상서비스가 '시설 및 환경','진료비', '설명 및 정보제공', '친절성', '진료결과' 등 5항목에서 긍정적으로 인지되고 있었으며, B 병원의 척수손상서비스는 '이용 편리성'에서, A 병원 뇌 손상서비스는 '전문성'에서 다른 긍정적으로 인지되었다.

다섯째, B 병원에서 중요하다고 평가되면서 높은 만족도를 보인 항목은 '친절성'이었던반면, 중요함에도 불구하고 만족도가 낮아 향후 개선이 요구되는 항목은 '전문성'이었다.

이 연구에서 제시된 결과들에 비추어 볼 때, 개원한지 2년 반밖에 되지 않은 B 병원은, 전체적인 만족도와 다른 여러 항목에서 A 병원보다 만족도가 높았으며, 다차원척도 분석에 따르면 '쾌적하고 친절한 병원'의 이미지가 강점으로 포지셔닝되고 있어 이를 최대

한 살려나가는 전략이 바람직할 것으로 생각된다. 반면 '전문성'에 있어서는 A 병원에 상대적으로 취약하게 평가되고 있어 '치료인력의 증원'과 '재활전문병원으로서 서비스 특성화'에 좀더 힘을 기울여야 할 것이다.

이 연구는 병원 서비스에 '경영'이나 '마케팅' 개념의 도입이 활발해지기 시작하는 시점에서 두 재활병원의 서비스를 비교하여 강점과 약점을 파악하고, 다차원척도법을 이용하여 재활 서비스에 대한 환자들의 인식을 포지셔닝했다는 점에서 연구의 의의가 있다고

본다. 그러나 앞으로 재활 서비스를 비교하기 위한 표준화된 측정도구의 개발이 이루어져아 하며, 이러한 표준화된 측정도구를 이용하여 여러 재활 병원의 다양한 재활 서비스를 비교.분석하는 연구들이 추가적으로 필요하다

[영문]

The purpose of this study was to establish the strategy to improve rehabilitation service on the basis of positioning analysis by multidimensional scaling. The subjects of this study were 82 patients and guardians who have previously received in-patient and out-patient care from both of the two rehabilitation hospitals. The data were collected from self-reporing questionnaire from April 1st to May 10th, 1997.



The main findings were as follows:

First of all, male patients were more satisfied with the rehabilitaion service than female.

Secondly, as the rehabilitatioanl service satisfactions among the patient groups of three diseases were investigated, satisfaction of cerebral palsy group were lower than brain injury and spinal cord injury group in terms of 'cost', 'equipment and environment', ' convenience' and 'explanations and information'.

Thirdly, of the 8 satisfaction items investigated between the two hospitals, 5 items (such as 'equipment and environment', 'convenience', 'cost', 'explanations and information' and 'overall satisfaction') ranked higher in B hospital, and 1 item('specialty') ranked higher in A hospital.

Forthly, Brain injury services of B hospital were highly positioned in 5 items, such as 'equipment and environment', 'cost', 'explanations and information', 'kindness' and 'outcome of care'. Spinal cord injury services of A hospital were highly positioned in 'convenience' item, and Brain injury services of A hospital were highly positioned in the ' specialty' item.

Fifthly, in B hospital, the item 'kindness' was ranked high in priority in as much as satisfactory, however the highest priority item 'specialty' showed low satisfaction in B hospital.

Considering above results, B hospital should continue to endeavour in maintaining its high esteem in 'comfortableness and convenience'. However, as 'specialty' was a weak point, they shoul change for the better by training the staffs and specialization of hospital services.

As multidimentionsional scaling can simplify the complicated components in service analysis, it can be widely used in health science area.
