관절연골의 자기공명영상과 조직학적 소견의 상관관계

Abstract
[한글]

본 연구의 목적은 관절연골의 자기공명영상에서 나타나는 층파 조직학적 층을 인위적으로 만든 지표를 사용하여 비교 및 분석하고 이 층들이 자기공명영상의 인공물에 의한 것이 아님을 밝히는 것이다.

소의 무릎연골과 사람의 거골연골의 표면에 5개의 'V'모양의 지표를 만든 후 1.5T (CE Horizon)를 사용하여 고해상자기공명영상을 얻었다. T1강종(TR/TE: 300/14) 및 고속 스핀에코 T2 강조(TR/TE: 2000/53) 영상을 얻었으며 사용된 영상화 변수는 격자크기 512×256, 시야 4×4cm, 절편두께 2mm, 신호 획득 횟수(number of excitations) 20. 자기공명영상은 1) 주파수 암호화(frequeney endcoding)방향 전환 2) 위상 암호화(phase encoding) 정상(×2, ×½) 전환 3) 위상 암호화 수 (128, 160, 192, 224, 256)를 변화시킨 영상을 얻었다.

H&E, trichrome, alcian blue와 safranin 0 염색을 시행한 후 자기공명영상과 조직학적 표본을 1:1로 비교하였다.

조직학적 접선층 (tangential)과 이행층 (transitional)은 자기공명영상의 T1 고신호강도와 T2의 저신호강도층에 해당되었다. 조직학적 방사층(radial)은 T1강조영상에서 중간신호강도 및 T2강조영상에서 고신호에서 저신호강도로 변하는 층에 해당되었다. 소의 T1강조영상의 방사층의 하부에 해당되는 부위에 저신호 및 중간신호 강도의 띠가 보였다.

석희화 연골층(calcified cartilage)층은 모든 경우 저신호강도로 보였다.

위상 암호와 경사를 변화시켜 얻은 영상의 결과 표면의 저신호강도 층은 전혀 변화되지 않았으며 이는 이 층이 실존하는 층이면 자화 (susceptibility) 또는 화학 전이 (chemical shift) 인공물에 의한 것이 아님을 알 수 있었다. 위상 암호화 수를 증가시킴에 따라 자기공명영상의 층은 더욱 확실하여져 이는 절단 (truncation) 인공물에 의해 나타난 것이 아님을 의미하였다.

주파수 암호화 방향의 전화에 따른 자기공명영상의 각 층은 변화를 보이지 않았다.

결론적으로 자기공명영상의 층 구조는 자화, 화학 전이 또는 절단 인공물에 의한 것이 아니며 인위적으로 만든 지표를 사용하여 조직학적 층과 상관시킬 수 있었다.

[영문]

The purpose of this study was to correlated the laminae of different signal intensities(SI) on MR images with the histologic layers using artificially made landmarks and also to rule out MR aritfacts as the cause of the laminae.

Five 'V' shaped landmarks of different depths were made on the surfaces of bovine patella and human talar cartilage. Both T1WI (TR/TE:300/14) and FSE T2WI (TR/TE:2000/53) were obtained on a 1.5T (Horizon,GE) system. Imaging parametrs were; matrix size 512*256m FOV 4*4cm, slice thickness 2mm, number of excitations(NEX) 20. Images were obtained with 1) changed frequency encoding direction 2) changed read out gradient strength(x2, x1/2) 3) varied number of phase encoding steps (128,160,192,224,256). H&E, trichrome, alcian blue and safranin O

staining were performed and 1:1 correlation of the histologic tangential and transitional zones correlated with the superficial high and low SI on T1 and T2WI. The radial zone correlated with the lamina of intermediate SI on T1WI and the lamina with gradually decreasing SI from high to low on T2WI. An additional well defined low and intermediate SI bands on the bovine T1Wi correlated with the lower radial zone. Calcified cartilage layers were of low SI on both T1 and T2 studies.

Images with altered read out gradient strength showed no difference in thickness of the superficial low SI layer proving that it is a true layer and not caused by susceptibility or chemical shift artifacts. The laminae became more distinct as the

phase encoding steps were increased. The laminated appearance was not caused by truncation artifacts.

No alterations of the laminae were noted by changed frequency gradient direction.

In conclusion the laminated appearance is not caused by susceptibility, chemical shift or truncation artifacts and can be correlated with the histologic layers using artificial landmarks.
