Down 증후군은 가장 흔한 상염색체 이상으로 정신지체가 가장 문제가 되며 삼염색체성(trisomy)의 기전을 알고자 하는 노력이 과거 20여년간 진행되어 왔으나 아직 정확한 병태 생리를 알지 못하고 있는 실정이다. 그간의 연구는 주로 세포유전학적인 방법을 통하여 실시되었으나 최근 21번 염색체의 다형성 표식자의 개발과 분자유전학적인 방법의 발달로 extra chromosome 21의 근원이 부모와 각 부모의 생식세포 형성과정에서 감수분열중 어느 단계의 이상에서 비롯된 것인지에 대한 연구가 진행되고 있으며 그 신빙성이 높은 것으로 알려져있다.

본 연구에서는 분자유전학적인 방법으로 우리나라 Down증후군 환아의 extra chromosome 21이 부모중 어느쪽에서 유래되었는지, 또한 생식세포의 감수분열 Ⅰ, Ⅱ단계중 어디에서 비분리(nondisjunction)가 일어났는지를 밝히고, Down 증후군 환아를 출생하였을 당시의 산모의 나이와 본 연구에서 나타난 trisomy 21의 근원과의 관계에 대하여 조사함과 동시에 우리나라에서 Down 증후군을 진단하는데 있어 분자유전학적인 방법의 유용성을 정립하는데 도움이 되고자 하였다.

연구 방법으로 염색체 검사를 통해 진단된 Down 증후군 환아와 부모를 대상으로 DNA 다형성 표식자(D21Sll)를 이용하여 중합 효소 연쇄 반응을 시행하였으며,6% acrylamide gel 전기영동 및 autoradiograph를 시행한 후 나타난 alelles의 위치와 강도를 판독하여 비분리의 근원을 결정하였다.

염색체 검사를 통해 확진된 36명의 다운 증후군 환아의 염색체형은 47,XY,+21이 21명, 47,XX,+21이 15명이었고 mosaicism이나 전위형 다운 증후군는 없었다.

환아와 부모 모두를 대상으로 연구할 수 있었던 경우는 19가족 이었으며, 환아와 어머니만 대상이 되었던 경우가 17가족 이었다. 다운 증후군 환아를 출생하였을 당시의 어머니의 평균 나이는 30.8±4.2세 이었으며 아버지의 평균나이는 33.6±7.3세 이었다.

D21Sll 염색체 표식자를 이용하여 연구를 시행하는 과정에서 연관이 없는 104명의 우리나라 사람에서의 D21Sll 표식자에 대한 heterozygosity는 79.4% 이었으며 전부 8개의 각기 다른 크기의 alleles를 관찰할 수 있었다.

전체 36 가족 중에서 27 가족(75.077)에서 비분리가 부모중 어느쪽에서 일어났는지를 알 수 있었고 나머지 9 가족(25.0%)에서는 정확히 구별할 수 없었다. 구별이 가능하였던 27 가족중 24명(88.9%)이 어머니에서 비분리가 일어났으며 3명(11.1%)에서는 아버지가 비분리의 근원임을 알 수 있었다.

어머니에서 근원이 되었던 24명의 환아중 16명에서 감수분열단계 중 비분리의 근원을 알 수 있었는데 감수분열 1단계의 비분리에 의한 경우가 9례(57.3%),2단계에 의한 경우가 7례(42.7%) 이었다. 아버지에서 유래된 3례 모두 감수분열 Ⅱ단계에서의 비분리로 인한

것이었다.

이상의 결과를 종합해 볼 때 Down 증후군을 진단하는데 분자유전학적인 방법이 유용하리라 여겨지며, 부모중 extra chromosome 21의 근원은 어머니에서 비롯된 경우가 아버지보다 월등히 높은 것을 알 수 있었고, 감수분열 단계중 비분리의 근원은 아버지에서 유래된 경우 모두 감수분열 Ⅱ단계가 근원이었으나 어머니에서 유래된 경우는 정확히 구별하기 어려운 경우가 있었는데, 이는 여러가지 중심체 혹은 중심체 근접 다형성의 DNA 표식자들을 같이 이용한다면 해결할 수 있으리라 사료된다.

Down syndrome, the most common single cause of mental retardation, is usually due to meiotic nondisjunction leading to trisomy 21. In order to understand the mechanisms of meiotic nondisjunction, several studies have been carried out to determine the origin of the extra chromosome 21 in Down syndrome. These include chromosome heteromorphisms by different staining techniques. More than 1000 individuals and their parents have now been examined. Cytogenic heteromorphism studies indicate that 70-80% of cases of trisomy 21 are maternal in origin and that 20-3O% are paternally derived. Recently, recombinant DNA technology permitted the use of chromosome 21 specific DNA probes for the analysis of the origin of nondisjunction by DNA polymorphisms. Molecular studies indicate a maternal error rate of about 94%, significantly higher value than that reported by the cytogenetic studies. From these result it has been suggested that the cytogenetic appoarch has an unacceptably high error rate and that it no longer has practical value in studies of chromosome 21 nondisjunction. We have studied DNA polymorphisms at loci on the long arm of chromosome 21, in order to determine the parental origin of a extra chromosome and the meiotic stage of nondisjunction in 36 families with free trisomy 21 and to establish the molecular genetic diagnosis in Down syndrome.

A total of 36 families, each with a propositus with chromosomally diagnosed free trisomy 21, were included in the study. The D21Sll locus showed heterozygosity of 79.4% in the 104 unrelated Korean. Eight alleles were observed. The parental origin of the extra chromosome 21 could be determined in 27 of 36 cases. Maternal origin was 88.9%(24/27) and paternal origin 11.1%(3/27). Among the informative cases, the meiotic error occurred at the first maternal meiosis in 9 (57.3%) and second

meiosis in 7 (42.7%) of 16 cases. All paternal meiotic error occurred in the second meiosis.

Further experiments with DNA polymorphic markers that map at the centromere of chromosome 21 are needed in order to definitely determine the meiotic stage of nondisjunction in trisomy 21.
