자외선 조사에 따른 인체 피부의 노화현상 및 p53 표현성

[한글]

자외선 요법은 광물성 기름를 바르고 자외선 B(UVB)를 조사하는 방법과 광독성 물질인 소라렌(psoralen)을 복용하거나 도포하고 자외선 A(UVA)를 조사하는 소라렌과 자외선 A(psoralen + UVA, PUVA) 방법등이 있다. 장기적인 자외선의 노출로 인한 광노화는 극세포증, 표피세포의 이형성, 확대된 피지선등을 유발할 수 있으며 멜라닌세포의 수와 크기는 증가하며 랑게르한스 세포는 그 수가 감소하고 기능도_ 떨어진다. 또한 진피내에는 탄력섬유가 변성되고 교원섬유는 양이 감소하며 만성 염증세포 침윤과 모세혈관확장증의 소견을 보인다. PUVA는 동물실험에서 표피 유전자의 돌연변이 및 피부암을 유발시킬 수 있다고 하며 또한 단기간 PUVA 치료를 받은 환자에 비해 장기간 PUVA 치료를 받아 많은 양의 자외선에 노출된 환자에서 피부의 편평상피세포암의 발생이 증가했다는 보고가 있다.

이에 연구자는 장기간(6개월 이상 10년 이하) 자외선 치료를 받은 환자를 대상으로 피부노화정도를 알아보기 위하여 자외선 노출 정상 피부에서 조직 검사를 하여 일반 염색 및 특수 염색을 하였으며, p53 및 p21단백질의 양성 빈도와 변화양상 및 p53 유전자의 돌연변이 유무를 알아보기 위하여 면역조직화학 염색과 중합효소연쇄반응-단일가닥구조 다형검사법을 실시하여 다음과 같은 결과를 얻었다.

1. 조직학적 소견상 기저세포층에서 비전형적인 멜라닌세포가 관찰되었으나, 표피세포의 이형성, 세포배열의 극성 소실과 같은 광노화 변화는 관찰할 수 없었다.

2. 조직학적 소견상 진피에서 유두 진피층의 부종, 혈관확장 및 내피세포의 증식 그리고 멜라닌탐식세포를 관찰할 수 있었으나 탄력섬유증이나 교원섬유의 변화등은 발견할 수 없었다.

3. 자외선 치료를 받은 환자의 피부에서 치료 중단 1주후 시행한 p53 면역조직화학 염색에서 p53 양성 세포수는 비노출부위와 비교할 때 의의있게 증가하였다.

4. 자외선 치료 중단 1주후 시행한 p53 면역조직화학 염색에서 자외선 조사군 모두에서 p53양성 세포가 기저세포층에서 유극층으로 분화되어 올라갈수록 그 수가 감소하는 것을 알 수 있었다.

5. 자외선 치료 중단 1주후 시행한 p21 면역조직화학 염색반응 양상은 p53 면역조직화학 염색에서 관찰한 소견과 유사하였다.

6. 1,000J/cm**2 이상 PUVA 치료군에서 치료 중단 4개월 후에 시행한 p53 면역조직화학 염색에서 p53 양성 세포수는 의의있게 감소하였다.

7. 자외선 치료 중단 1주후 채취한 환자의 피부를 이용한 중합효소연쇄반응-단일가닥구조 다형검사법에서 p53 유전자의 돌연변이는 관찰되지 않았다.

이상의 결과로 보아 장기간의 자외선 치료는 표피세포의 이형성, 세포배열의 극성 소실과 같은 광노화 변화는 초래하지 않았으며 표피세포의 p53 과표현 역시 반응성 과정으로 사료되어 자외선 치료가 비교적 안전한 치료방법으로 생각한다.

[영문]

Ultraviolet(UV) therapy consists of application of mineral oil and irradiation of ultraviolet B(UVB) and of application or ingestion of psoralen and ultraviolet A(UVA) irradiation (PUVA). Phtotoaging due to exposure of UV for a prolonged period can induce acanthosis, dysplasia of epidermal cells and enlarged sebaceous glands. In photoaging, the number and size of melanocytes are increased and the number and function of Langerhans cells are decreased. Also in dermis elastic fibers are

denatured and the collagen fibers are decreased in amount and infiltration of chronic inflammatory cells and telangiectasia are observed. Especially, long-term treatment of PUVA is said to be mutagenic and be able to induce skin cancers in animal experiments and it is also reported that there is an increased chance of cutaneous squamous cell carcinomas in the patients exposed to large amount of UV radiation after long-term PUVA treatment.

Therefore, I investigated the degree of photoaging by hematoxylin-eosin and special stains on the irradiated normal skin of the patients undergoing long-term UV therapy (6 months to 10 years) and I also performed immunohistochemical stainings of p53 and p2l and polymerase chain reaction - sing1e-strand conformation polymorphism(PCR-SSCP) on the normal skin of the patients subject to lone-term UV therapy. The following results were obtained.

1. Under observation with the microscope, although atypical melanocytes were found in the basal layer of epidermis, the features of photoaging such as dysplasia of epidermal cells and loss of polarity of epidermal cells were not observed in the patients undergoing long-term UV therapy.

2. Edema of papillary dermis, blood vessel dilatation, proliferation of endothelial cells and melanophages were seen in dermis of the patients undergoing long-term UV therapy, However, the charateristic features of photoaging such as elastosis and decrease in collagen were not seen.

3. In p53 immunohistochemical staining performed at 1 week after cessation of long-term UV treatment, the patient group demonstrated an increased number of p53 positive epidermal cells compared to unexposed skin.

4. In p53 immunohistochemical staining performed at 1 week after cessation of long-term UV treatment, the number of p53 positive epidermal cells decreased in the process of differentiation from basal layer to spinous layer in all PUVA and

UVB-treated groups.

5. The pattern of p21 immunohistochemical staining performed at 1 week after cessation of long-term PUVA and UVB treatment was similar to that of p53 immuno-histochemical staining performed at 1 week after cessation of long-term PUVA and UVB treatment.

6. In p53 immunohistocheinical staining performed at 4 months after cessation of UV treatment in the patient group of culmulated UV dosage of more than 1,000J/cm**2, the number of p53 positive epidermal cells decreased significantly compared to that of p53 positive epidermal cells found at 1 week after cessation of UV treatment, although there was still pS3 positive epidermal cells at 4 months after cessation of UV treatment.

7. The mutation of p53 gene was not found in PCR-SSCP analysis of biopsied skin done at 1 week after cessation of long-term PUVA and UVB treatment.

These results suggested that long-term PUVA and UV therapy did not induce the changes of photoaging, such as epidermal dysplasia and loss of polarity of epidermal cells and that also p53 overexpression in epidermis of UV treated skin was a reactive process.

Therefore, I think that long-term UV therapy is a relatively safe treatment modality.
