위암으로 전위절제술을 시행한 환자에서의 비타민 B12 결핍성 빈혈

전위절제술 후에 생기는 비타민 B^^12 결핍성 빈혈은 빈도에 비해 크게 주목받지 못하고 있었으나, 최근 조기위암 진단의 증가 및 위암치료법의 발달로 전위절제술을 시행받은 환자의 장기생존율이 증가됨에 따라 이들 합병증에 대한 관심이 증가되고 있다. 위궤양으로 전위절제술을 받은 환자는 대개 5년에 비타민 B^^12 결핍성 빈혈이 최고의 발생빈도를 보이나 위암으로 전위절제술을 받은 환자는 위궤양에 비해 큰 수술범위, 항암화학요법, 암전이 등으로 양성질환으로 부분 흑은 전위절제술을 시행받은 환자와 체내 비타민 B^^12 의 변화가 다르다. 소화성 궤양으로 위절제술을 시행받은 환자를 대상으로 한 비타민 B^^12 결핍에 대한외국의 연구는 다수 있으나 위암으로 전위절제술을 시행받은 환자를 대상으로 한 비타민 B^^12 결핍에 대한 연구는 극히 드물며, 국내에서는 전위절제술을 시행받은지 7-10년 후에 내원한 환자에서 발견된 비타민 B^12 결핍성 빈혈의 산발적인 증례보고만 있을 뿐, 전위절제술 후 발생한 비타민 B^^12 결핍성 빈혈에 대한임상연구는 전무한 실정이다.

이에 본 연구에서는 한국인에서 위암으로 진단받고 전위절제술을 시행받은 환자들의 임상적, 생화학적, 혈액학적 지표를 조사하여 이들의 비타민 B^^12 혈청농도의 감소경향 및 조기에 비타민 B^^12 결핍성 빈혈이 발생한 환자군의 특징을 검토하고, 이를 토대로 비

타민 B^^12 결핍성 빈혈발생의 예방, 진단 및 적절한 보충요법의 시기 결정에 도움이 되고자, 1983년 6월부터 1994년 10월 사이에 위암으로 연세대학교 의과대학부속 신촌세브란스병원에서 전위절제술을 시행받은 환자 중 혈청 비타민 B^^12를 측정한 91예(159회)를 대상으로하여 혈청 비타민 B^^12 의 감소경향을 알아보고, 비타민 B^^12 결핍성 빈혈 조기발생군의 특성을 파악하기 위해 전위절제술 후 13개월에서 48개월 사이에 비타민 B^^12 혈청농도를 측정한 환자 59예를 선정하여 비타민 B^^12 결핍성 빈혈 조기발생군과 비타민 B^^12 결핍성 빈혈이 발생하지 않은 군으로 나누어 검토하여 아래와 같은 결과를 얻었다.

1) 91예 환자의 전위절제술 후 중앙 추적기간은 31.9개월(범위: 0-135개월)이었으며 비타민 B^^12 혈청농도의 중앙값은 158.94 pg/mL(범위: 20-806.2 pg/mL)이었다. 전위절제술 후 추적기간과 비타민 B^^12 혈청농도 사이의 상관식은 y=403.3×e**-0.09x+175.93×e**-0.02x(r**2=0.804)이었으며 수술 후 20개월부터 비타민 B^^12 혈청농도는 200 pg/mL미만으로 감소하는 경향을 관찰할 수 있었다. 그러나 혈색소는 비타민B^^12 혈청농도의 감소경향과는 다르게 수술 후 60개월까지는 거의 큰 변화가 없다가 그 후 완만한 감소를 보이고 있었으며 상관식은 y=-0.004x**2×0.0237x+11.07(r**2=0.067)이었다.

2) 대상환자 59예 중 비타민 B^^12 결핍성 빈혈 조기발생군(GroupⅠ)은 20예, 비타민 B^^12 혈청농도는 200 pg/mL 미만이나 비타민 B^^12 결핍성 빈혈이 없는 군(GroupⅡ)은 21예, 비타민 B^^12 결핍이 아닌 군(GroupⅢ)은 18예였다. 남자 31예(52.5%), 여자 28예(47.5%)로 남녀 비는 1.1:1이었으며, 평균 연령은 50.2세(범위:26세-66세)였다. 59예 환자의 전위절제술 전 위암증상의 기간은 GroupⅠ은 18.2주로 GroupⅡ의 9.6주에 비해 유의하게 길었으나(p<0.01) 전위절제술 후의 기간은 세 군에서 차이가 없었다(p=0.16). 각 군간의 비타민 B^^12 혈청농도는 GroupⅠ은 126.7 pg/mL, GroupⅡ는 96.0 pg/mL, GroupⅢ는 330.4 pg/mL로 GroupⅠ과 GroupⅡ는 GroupⅢ에 비해 유의하게 낮은 비타민 B^^12 혈청농도를 보였으며(p<0.01) GroupⅠ과 GroupⅡ간에는 유의한 차이가 없었다(p=0.05).

3) 검사실 소견에서 혈색소는 GroupⅠ이 10.9 g/dL인데 반해 GroupⅡ는 12.6 g/dL, GroupⅢ는 11.3 g/dL로 Group ll는 Group I의 혈색소에 비해 유의하게 높은 값을 보였으나(p<0.01) Hct, MCV, MCH, MCHC 등의 적혈구지수는 세 군간의 유의한 차이가 없었다. 철결핍

성 빈혈이 있는 경우 비타민 B^^12 결핍성 빈혈은 7예(7/10)로 철결핍성 빈혈이 없는 군(7/20)에 비해 비타민 B^^12 결핍성 빈혈의 발생빈도는 높았으나 철결핍성 빈혈유무에 따른 비타민 B^^12 결핍성 빈혈의 발생은 유의한 차이가 없었다(p=0.12).

4) 59예를 항암화학요법군, 비항암화학요법군으로 나누었을 때 같은 평균 전위절제술 후 기간에 비타민 B^^12 혈청농도는 201.8 pg/mL, 127.7 pg/mL으로 평균혈색소는 항암화학요법군과 비항암화학요법군에서 유의한 차이가 없었으나(p=0.64) 항암화학요법군의 비타민 B^^12 혈청농도는 비항암화학요법군과 같은 정도로 감소되어 있지 않았다(p=0.01).

5) 13-48개월 사이의 비타민 B^^12 결핍성 빈혈발생을 예측할 수 있는 인자는 동반질환, 전위절제술 전 위암의 증상기간이었으며 동반질환이 있는 경우 동반질환이 없는 경우에 비해 비타민 B^^12 결핍성 빈혈 조기발생의 위험은 14.3배 증가되며 전위절제술 전 임상

증상의 기간이 1주 길어질 때마다 조기 빈혈발생의 위험은 1.1배씩 증가되었다.

이상의 결과로 위암으로 전위절제술을 시행받은 환자는 수술 후 20개월부터 비타민 B^^12 혈청농도가 200 pg/mL 미만으로 감소하는 경향을 보임을 알 수 있었다. 그러나 전위절제술을 시행받은 환자는 시간의 경과에 따라 지속적으로 비타민 B^^12 혈청농도가 감소하며, 비타민 B^^12 결핍시 생기는 신경학적인 손상이 비가역적임을 고려해 볼 때, 간경변이나 과도한 알콜복용의 동반질환이 있거나 전위절제술 전 위암의 증상기간이 긴 경우는 조기에 비타민 B^^12를 보충하여야 할 것으로 생각된다. 위암으로 전위절제술을 시행받은 환자에서는 혈색소, MCV 등의 적혈구지수가 비타민 B^^12 혈청농도를 반영할 수 없으며, 비타민 B^^12 혈청농도는 항암화학요법시 세포의 비타민 B^^12양을 정확히 표현할 수 없으므로, 세포내 비타민 B^^12양을 정확히 반영하는 검사법을 개발하여 전향적 연구를 시행하는 경우전위절제술을 시행한 환자들의 적절한 비타민 B^^12의 보충시기 결정 및 비타민B^^12 결핍에 의한 합병증 발생의 예방에 도움이 될 것으로 생각된다.

Vitamin B^^12 deficiency anemia after gastrectomy for treatment of stomach cancer has not received enough clinical emphasis when considering it's prevalence, but recently many physicians have become more interested in it due to the increased long-term survivors after gastrectomy.

Patients who had undergone total gastrectomy of stomach cancer are different from patients who had undergone total or partial gastrectomy of peptic ulcer in clinical features and laboratory findings including seam vitamin B^^12 concentrate, which is due to factors of operation method, postoperative anti-cancer chemotherapy, tumor metastasis. Many physicians have reported slight differences in the onset time of the clinical manifestations of vitamin B^^12 deficiency anemia after partial

gastrectomy, because vitamin B^^12 absorption is different in degree of gastrectomy, postoperative intestinal complication etc. But incidence after total gastrectomy is peaked at 5 years. In foreign literatures there are many studies done on vitamin B^^12 deficiency after gastrectomy due to peptic ulcer, but few studies have been done about vitamin B^^12 deficiency anemia after total gastrectomy due to stomach cancer. In Korea, there are few case reports of vitamin B^^12 deficiency anemia patients who were incidentally found at 7 to 10 years after undergoing total gastrectomy and there are no collective studies to find out clinical features of vitamin B^^12 deficiency anemia after total gastrectomy.

The objective of our study is to find out the decremental character of serum vitamin B^^12 concentrate and features of patients with vitamin B^^12 deficiency anemia occurring earlier than 48 months after total gastrectomy. We studied 91 patients who underwent total gastrectomy and select 59 patients who evaluate serum vitamin B^^12 concentrates within 48 months after total gastrectomy to characterize the features of vitamin B^^12 deficiency anemia group occurring earlier than 48 months after the operation.

1) In 91 patients, the mean postoperative duration is 31.9 months(range:0-135months) and mean serum vitamin B^^12 concentrate 158.94 pg/mL(total 159 times, range:20-806.2 pg/mL). Correlation equation is y=403.3×e**-0.09x +175.93×e**-0.02x (r**2=0.804). The postoperation duration that serum vitamin B^^12 concentrate falls below 200 pg/mL is 20 months.

2) Among 59 selected patients, there are 20 cases of vitamin B^^12 deficiency anemia group(GroupⅠ), 21 cases of vitamin B^^12 deficiency(GroupⅡ) and 39 cases of non-vitamin B^^12 deficiency group(GroupⅢ). Male:Female ratio is 1.1 :1(31:28)and mean age is 50.2 years old(range: 26-66 years old). There are no age or postoperation duration difference between groups. But GroupⅠ and GroupⅡ shows lower serum vitamin B^^12 concentrate than GroupⅢ(126.7/96.0 pg/mL Vs 330.4 pg/mL, p<0.01). Preoperation symptom duration is longer in groupⅠ than GroupⅡ(18.2/9.6 weeks, p<0.01). There is no significant difference in R8C indices and biochemical laboratory results except hemoglobin(10.9 g/dL, 12.6 g/dL, and 11.3g/dL in GroupⅠ, GroupⅡ and GroupⅢ, p<0.01). Splenectomy, anti-cancer chemotherapy did not have a significant influence on the incidence of vitamin B^^12 deficiency anemia occurring within 48 months. 11 cases had associated diseases with vitamin B^^12 deficiency anemia. There were 9 alcoholics and 2 liver cirrhosis. Associated diseases had significant influence on the incidence of vitamin B^^12 deficiency anemia(p<O.01) within 48 months.

3) Among the 30 cases who could measure iron profile, 10 patients had an iron deficiency anemia. There is a higher incidence of vitamin B^^12 deficiency anemia in iron deficiency anemia group(7 cases in 10 cases, 70%) than non iron deficiency

anemia group(7 cases in 20 cases, 35%) but no statistical significance(p=0.12).

4) Among 59 cases, vitamin B^^12 deficiency anemia occurred in 13 out of 40 patients who had undergone anticancer chemotherapy. When comparing this incidence(32.5%) to that of the non-chemotherapy group(7 case, 36.8%), anti-cancer chemotherapy did not have a significant influence on the incidence of vitamin B^^12 deficiency anemia(p=0.972), but seam vitamin B^^12 concentrate is higher in anti-cancer chemotherapy group(201.83/127.66 pg/mL, p=0.01)and serum iron and TIBC

shows the same feature(92.O㎍/dL, 295.0㎍/dL 49.l㎍/dL, 315.3㎍/dL: p=0.048; p=0.03) at same mean postoperation duration(27.8/27.7 months, p=0.97).

5) The independent predictor of vitamin B^^12 deficiency anemia within 48 months are preoperation symptom duration(p=0.004) and associated disease(p=0.002). But anti-cancer chemotherapy, splenectomy, cancer stage did not have a significant influence on the development of vitamin B^^12 deficiency anemia occurring within 48 months after operation.

In conclusion, vitamin B^^12 should be replenish for at least 19 months after total gastrectomy due to stomach cancer. Associated disease and preoperation symptom duration are siginificant predictors for the occurrence of vitamin B^^12 deficiency anemia within 48 months after operation. To prevent vitamin B^^12 deficiency anemia and neurologic defect, and to promote better quality of life in total gastrectomized patients with stomach cancer, prospective studies are needed with new markers to determine the exact intracellular vitamin B^^12 body store which is not affected by chemotherapy.
