Comparison of interproximal soft tissue height for single implants and contra-lateral natural teeth

Abstract

[한글]치주질환으로 인하여 발치를 할 경우 이미 소실된 치조골로 인하여

향후 임플란트 수술 시 임플란트가 깊게 식립될 가능성이 있다. 본

연구의 목적은 소구치와 대구치 부위에서 치주질환으로 인하여 발치

후 반대측 자연치와 비교하여 깊게 매식된 단일 임플란트 수복물

주위의 치간 유두-치조골간 수직 고경 (IS-STI)과 반대측 자연치아 부위의 동일 고경 (IS-NT)을 비교하는 것이다.

소구치와 대구치 부위에 상부 보철물 연결 후 1 년 이상 기능한

단일 임플란트가 존재하며 반대측 부위는 치주수술 후 건강한

상태를 유지하고 있는 25 명의 환자를 대상으로 연구를 진행하였으며 발치의 이유는 모두 치주염이었다. 방사선 촬영을

통하여 임플란트와 반대측 자연치아 부위에서 치간유두-치조골간

수직고경을 측정하였다. 또한, 인접한 자연치아의 골 수준과

임플란트 변연골의 골 수준과의 수직적 길이 차이와 반대측 자연치

사이의 골 수준의 길이 차이를 측정하였다. Wilcoxon’s signed

rank test 를 사용하여 각 군간 차이를 검정하였다.

임플란트와 인접한 자연치아와의 변연골 수준의 수직 길이와

반대측 자연치아 사이 변연골 수준의 수직 길이와의 차이는

통계적으로 유의하였으며 이는 임플란트가 깊이 매식되었음을

의미한다. 반면 IS-STI 와 IS-NT 는 통계적으로 유의하지 않은

차이를 보였다. 이는 임플란트 주위의 치간유두-치조골간

수직고경이 반대측 자연치아와 차이를 보이지 않음을 의미한다.

이상의 결과를 통하여 소구치와 대구치 부위에서 임플란트가 비록

치주염으로 인해 치조골이 파괴된 상태에서 깊게 매식되더라도

치간유두-치조골간의 수직적 길이는 반대측 자연치와 큰 차이를

보이지 않았다는 결론을 얻었다.

[영문]The objective of the present study is to compare the interproximal soft tissue dimensions adjacent to a single implant restoration in the premolar-molar position with those adjacent to the contra-lateral natural teeth.Twenty-five patients with a single tooth implant on premolar-molar position and healthy contra-lateral teeth were enrolled in this retrospective cross-sectional study. The reason for the extraction was periodontitis. Radiographs were used to measure the shortest distance from the tip of the papilla to the crestal bone in single implants and contra-lateral teeth. A Wilcoxon signed ranks test was used to examine the differences between the interproximal soft tissue dimensions adjacent to the single tooth implant restorations (IS-STI) to those of the contra-lateral natural teeth (IS-NT). We found no significant difference between the mesial IS-STI and mesial IS-NT (3.28±0.83 / 3.31±0.62; p=0.861), or between the distal IS-STI and distal IS-NT (3.09±0.72 / 3.3±0.63; p=0.263). In conclusion, the interproximal soft tissue height adjacent to single implant supported restorations in the premolar-molar position is similar to that adjacent to the contra-lateral natural teeth.
