Abstract
[한글]

목적 : Crouch 보행은 가장 흔하게 관찰되는 뇌성마비(Cerebral palsy) 환아의 병적 보행으로 말기 유각기(Swing phase)와 입각기(Stance phase)의 과도한 슬관절의 굴곡을 보인다. 슬관절 굴곡의 증가에 대한 치료로서 오금근육(Hamstring muscle)에 대한 연장술은 Crouch 보행의 치료에 있어서 가장 기본적인 술식으로 생각되어 왔다. 그러나 오금근육(Hamstring muscle)의 길이의 단축에 대해선 다양한 연구결과가 있었으며, 일부 저자들은 오금근육(Hamstring muscle)의 길이의 단축이 없다고 주장하기도 하였다. 이는 Crouch 보행을 단지 시상면(Sagittal plane)에서의 슬관절 굴곡이라는 측면에서만 연구를 하였기 때문이며, 정확한 오금근육(Hamstring muscle)의 차이를 측정하기 위해서는 관절의 관상면(Coronal plane) 및 횡단면(Transverse plane)에서의 차이를 고려하여야 한다. 본 연구에서는 Crouch 보행이 관찰되는 뇌성마비 환아의 보행을 3차원 보행분석과 근-건 길이분석을 통해 분석하여 근-건 길이의 변화 및 속도의 변화가 각 관절 및 보행에 미치는 영향을 정량적으로 분석하고자 하였다. 이러한 분석을 통하여 보행 중 대퇴골의 염전 및 각 관절의 회전 운동에 따른 Crouch 보행의 특성을 구명하고 각 군에서 근육의 길이 및 수축 속도를 분석하여 Crouch 보행의 치료에 필요한 객관적인 기준을 제시하고자 한다.대상 및 방법 : 2004년 9월부터 2006년 12월까지 양하지 경직성 뇌성마비로 본원에 내원한 환아 중 Crouch 보행을 보이는 환아 중 독립 보행이 가능한 환아를 대상으로 하였다. 총 62명의 환아(124례)가 연구대상이었으며 남아가 40명, 여아가 22명이었다. 환아의 검사 당시 평균 연령은 8세(3세 5개월 ~ 13세 2개월)였다. 동 연령대의 정상인을 모집하여 정상군과의 비교하였다. 보행은 삼차원 보행분석기인 Vicon 370 Motion Analysis System? (Oxford Metrics Inc., Oxford, U.K.)을 이용하여 시공간적인 지표(temporospatial data), 운동형상학적 지표(kinematic data) 및 운동역학적 지표(kinetic data)를 관찰하여 분석하였다. 근육의 길이와 수축 속도는 3차원 보행분석상에서 얻어진 운동형상학적 지표(kinematic data)를 이용하여 컴퓨터 상에서 3차원 모델을 작성하고 이에 각 근육의 해부학적 위치를 입력하여 측정하였다. 측정된 운동형상학적(kinematic data), 운동역학적(kinetic data), 근-건 길이 및 속도 변화는 보행주기의 각 2%마다 측정된 수치를 평균화 하여 각 군의 평균 보행 분석 그래프를 작성하였다. 개개의 환자에서 입각기 중 초기 접지기의 최고값, 최저값, 평균값을 구하였으며, 입각기 와 유각기 중의 평균치를 구하였다. 대퇴골 및 경골의 염전등 회전 변형은 전산화단층촬영을 이용하여 각각의 환자의 개별적 결과를 얻었다. 환아에서 절대 근육의 길이 및 속도의 측정은 불가능하기에 본 연구에서는 보행 근-길이율(percent muscle length)과 보행 근-길이속도(percent muscle velocity) 를 측정하였다. 보행 근-길이율(percent muscle length)은 휴지기의 길이에 대한 보행 중 근육 길이의 변화의 비로 정의하였다. 보행 근-속도율(percent muscle velocity)은 근육의 해부학적 길이 변화 속도와 최대 수축 속도에 대한 비로 정의하였다. 통계 분석은 SPSS 11.0 for window version을 이용하였다. 뇌성마비 군과 정상군의 비교 시 two sample t-test를 시행하였다. 두 군간 비교 시는 two sample t-test를, 세 군간 비교 시는 ANOVA(analysis of variance)를 시행 후 Bonfferoni method를 이용하여 다중 비교를 시행하였다.결과 : 정상군과 비교하여 볼 때 뇌성마비 환아는 입각기(Stance phase)시 시상면(Sagittal plane)에서 골반 및 고관절 및 슬관절이 굴곡된 보행을 보였으며 고관절은 외전(Abduction)되어 있었고, 슬관절의 내회전(Internal rotation)으로 내족지 보행(Intoeing gait)을 보였다. 보행 중 족관절은 족저 굴곡되었으며 횡단면상(Transverse plane) 내족지 보행을 보였다. 장요근(Iliopsaos), 대퇴직근(Rectus femoris) 및 하퇴 삼두근(Gastroc -soleus)의 비례 근-길이율(Percent muscle length)의 감소를 보였다. 대퇴골 염전이 30°이상 증가한 군에서 고관절 굴곡 및 내회전이 증가하였다. 골반이 내회전된 경우는 중둔근(Gluteus medius)의 길이가 외회전시보다 짧고 정상 범위 보다도 감소하였으나, 고관절이 내회전 된 경우는 중둔근(Gluteus medius), 내전근(Adductor)의 길이가 정상범위 내에서 증가되어 있었고 슬관절이 내회전 된 경우는 중둔근(Gluteus medius), 내전근(Adductor) 및 오금근육(Hamstring muscle)이 모두 외회전시보다 짧았으나 정상범위 이내였다.정상과 차이가 나타나지 않는 오금근육(Hamstring muscle)과 단축된 하퇴 삼두근(Gastroc -soleus)을 고려할 때, 족관절 굴곡-슬관절 신전 조합의 불균형에 의해 초기 입각기의 슬관절 굴곡의 증가와 중기 입각기의 슬관절 굴곡이 나타난 것으로 생각된다. 모든 군에서 장요근(Iliopsaos)은 정상보다 짧게 나타났다. 따라서 내회전에 관계없이 Crouch 보행에서는 장요근(Iliopsaos)에 대한 연장술이 항상 고려되어야 할 것으로 생각된다. 따라서 하퇴 삼두근(Gastroc -soleus)에 의한 족관절 족저 굴곡의 교정과 장요근(Iliopsaos)에 의한 골반 전방 경사 증가의 교정이 가장 중요한 연장술이라고 생각된다. 중둔근(Gluteus medius), 내전근(Adductor) 및 오금근육(Hamstring muscle)의 상호작용에 의해 관절의 회전운동이 나타나며 골반의 내회전이 증가할 시에는 중둔근(Gluteus medius)의 연장술이 고려되어야 할 것으로 생각된다.결론 : Crouch 보행의 수술 시 장요근(Iliopsaos) 및 하퇴 삼두근(Gastroc -soleus)의 경직이 고려되어야 하며, 장요근(Iliopsaos)의 연장술은 관절 회전에 무관하게 항상 고려되어야 한다. 대퇴골 전염전의 증가 시 장요근(Iliopsaos)의 경직은 상대적으로 교정이 되는 것으로 생각되며, 관절의 회전에는 중둔근(Gluteus medius), 내전근(Adductor), 오금근육(Hamstring muscle)이 상호 관여하는 것으로 생각된다.

Objectives : Crouch gait, the most common pathologic gait in children with cerebral palsy, is a knee flexion gait affecting the terminal swing and stance phase. The first choice for the treatment of crouch gait is usually lengthening of the hamstring, but conflict has arisen in the field due to differences in the appropriate length of the hamstring muscle. This conflict can be boiled down to differences in data collection in that many authors restrict the measurement of crouch gait to the sagittal plane. To estimate the exact length of the hamstring muscle, researchers must not restrict measurements to the sagittal plane, but also include the coronal and transverse plane motions. To properly measure the muscle-tendon complex’s length and velocity to other joints requires study and analysis using a 3 dimensional gait analysis during real gait, instead of static position. The purpose of this study was to clarify crouch gait according to the femoral torsion and rotation at each joint from the data of 3 dimensional gait analysis during motion and to establish objective guide lines for the treatment of crouch gait.Materials and Method: Between Sep. 2004 and Dec. 2006, patients with spastic diplegia who could walk independently were included. Patients who had undergone selective posterior rhizotomy, other orthopaedic surgery and botulinum injection within 6 months were excluded. 62 patients (124 cases) were selected, 40 males and 22 females. The average age at the time of evaluation was 8 years (from 3 years 5 months to 13 years 2 months). 15 normal children between 5 and 15 years old were also included in the study for comparative data. Temporospatial data, kinematical data and kinetic data were obtained using the Vicon 370 Motion Analysis System (Oxford Metrics Inc., Oxford, U.K.). The torsion of femur and tibia was obtained from computed tomography of each patient. We used percent muscle length and velocity to estimate muscle length and velocity changes, since it is impossible to calculate the actual muscle length and velocity due to muscle length being different in each patient. The definition of percent muscle length is the ratio of muscle length during resting phase to muscle length during gait. The definition of percent muscle velocity is the ratio anatomic length speed to maximum contraction speed. The gait analysis graph of kinematics, kinetics, percent muscle length and velocity was created using the mean value obtained from gait cycle every 2%. The data was analyzed using SPSS 11.0 for Windows. Two sample t tests were used to collect two sets of variables. ANOVA tests were performed for three variables followed by the application of the Bonfferoni method. Statistical significance was set at P<0.05.Results: Comparison of cerebral palsy patients to the normal group shows flexed pelvic, hip and knee joint during stance phase in the former; hip adduction and internal rotation of knee is also observed. During stance phase, ankle plantar flexion and in-toeing gait are evident. The percent muscle length of iliopsoas, rectus femoris and gastro-soleus muscle is shorter than in the normal group. Hip flexion and internal rotation are increased when femoral rotation is more than 30°. When pelvis is internal rotated, the percent length of gluteus medius is especially shorter than the normal group. When hip is internally rotated, the percent length of gluteus medius and adductor muscle is increased, but within normal range. When the knee is internally rotated, the percent length of gluteus medius, adductor and hamstring are shorter than external rotated knee position but within normal range. Evaluation of the hamstring muscle and shortened gastrosoleus reveals that increased knee flexion during initial stance and mid stance phase occurs due to an imbalance of the ankle plantar flexion-knee extension coupling. Percent length of iliopsoas muscle is always shorter than normal group. Irrespective of rotation level, lengthening surgery of iliopsoas muscle might be essential. Lengthening surgery of gluteus medius muscle could also be considered when pelvis is internally rotated, even though the corresponding effect of gluteus medius, adductor and hamstring muscle to other joints is variable.Conclusion: : Spasticity of iliopsoas and gastro-soleus muscle might be considered before correctional treatment of crouch gait. Irrespective of rotation level, lengthening surgery of iliopsoas muscle is always an option. Lengthening surgery of gluteus medius muscle could be considered when pelvis is internally rotated. When femoral anteversion is increased, the length of iliopsoas is shorted perhaps due to skeletal deformity and rotation of joint is affected by gluteus medius, adductor and hamstring respectively.
