척수수막류 환아의 종족변형에 대한 전경골근 건 이전술

척수수막류 환아에서 관찰되는 종족변형(calcaneus deformity)은 제 5 요추 및 제 1 천추 신경근 결함에 의한 족근관절의 굴곡과 신전근력의 불균형에 의해 초래되며 현재 주로 추천되는 대표적인 연부 조직 수술로 족근관절에 대한 단순 전외방 유리술(anterolateral release)과 전경골근의 종골로의 후방이전술(tibialis anterior tendon transfer to calcaneus through interosseous membrane)을 들 수 있다. 이 중 전경골근 이전술은 단순 건절제술에 비해 술식이 광범위하고 복잡하며 특히 치료 결과가 일정치 않다는 점 등이 이 술식의 단점으로 제시된 바 있다. 역동적 족저압 측정과 3차원 보행분석으로 전경골근 후방이전술의 결과를 분석하고, 술식의 제한점과 적응증을 제시하고자 하였다.2000년 3월부터 2003년 12월까지 수술을 시행받은 16명, 27족을 대상으로 하였다. 수술 당시 평균 연령은 7년 4개월이었고, 평균 추시 기간은 25개월이었다. 3족에서는 종골 후방전이 절골술을 시행하였다. 술 전과 술 후 12개월에 역동적 족저압 측정과 3차원 보행분석을 시행하였다.술 후 입각기 족관절의 운동범위는 호전되었으나 족관절의 운동역학, 슬관절과 고관절의 운동역학, 운동형상학적 변화는 관찰되지 않았다. 종골부 비례추진력은 83%에서 54%로 감소하였다. 술 전 35도 미만의 강직된 슬관절 운동범위를 보인 10족에서는 종골부 비례추진력이 술 전 90%에서 75%로 감소하여 술 전 81%에서 45%로 감소한 군보다 저조한 결과를 보였으며, 술 전후, 감소된 슬관절 운동범위와 증가된 고관절 외전, 골반회전, 골반사위가 관찰되었다.술 전 시상면에서 감소된 슬관절의 운동범위와 증가된 고관절 외전, 관상면 및 횡단면에서의 과도한 골반운동이 전경골근 후방이전술의 결과에 영향을 미치며, 고관절 외전근력이 중요한 예후인자로 생각된다

[영문]The purpose of this study were to examine the results of tibialis anterior tendon transfer for calcaneus deformity in children with myelomeningocele.27 feet in sixteen children were included for the study. The mean age at the time of the operation was 7+2 years (range, 4+11 ？ 14+9) and the mean follow up duration was 25 months (range, 15 ？ 50). Three-dimensional gait analysis and dynamic foot-pressure measurement were performed to analyze the results of index operation.There were no recurrences or aggravation of the deformity. Sagittal kinematics of the ankle was improved, and the relative impulse of the calcaneus was decreased postoperatively. Those patients with decreased range of motion of the knee in sagittal plane during gait had less improvement in relative impulse of the calcaneus than those with good range of motion of the knee. They also had increased range of abduction of the hip, and up-obliquity and internal rotation of the pelvisGood range of motion of the knee in sagittal plane, and of the hip and pelvis in coronal and transverse planes were important predictors of a functional transfer of tibialis anterior tendon in patients with calcaneal gait in myelomeningocele.
