간경변증에 동반된 특발성 세균성 복막염의 재발빈도 및 예견인자

특발성 세균성 복막염(Spontaneous bacterial peritonitis : SBP)은 간경변증의 중요한 합병증이며 복수가 동반된 간경변증의 10-15%에서 발생하며 약 50%에서 패혈증, 간성혼수, 위장관 출혈, ��등으로 사망한다.

최근에는 특발성 세균성 복막염에 대한 높은 인식과 조기진단 및 적절한 항생제의 투여로 사망률이 현저히 줄었다. 사망률의 감소는 궁극적으로 특발성세균성 복막염의 재발빈도를 증가시킬 것으로 예상되어 이에 대한 관심이 필요할 것으로 생각된다.

특발성 세균성 복막염은 재발의 빈도가 많을수록 예후가 나쁜 것으로 알려져 있으며 재발을 예방하기 위한 예견인자의 규명 및 치료방법의 확립에 대해 많은 연구가 진행되고 있다. 그러나 국내에서는 아직 재발된 특발성 세균성 복막염에 대한 연구 보고가 없다.

이에 연구자는 연세대학교 원주의과대학부속 원주기독병원 내과에 내원한 복수가 동반된 간경변증 312예 가운데 특발성 세균성 복막염 68예를 대상으로 처음 발병된 특발성 세균성 복막염 군과 재발된 특발성 세균성 복막염 군으로 나누어 특발성 세균성 복막염의 재발빈도 및 재발에 영향을 미치는 인자에 대해 알아보고자 본 연구를 시행하였다.

결과

1. 복수가 동반된 간경변증 312예 가운데 특발성 세균성 복막염은 68예(21.8%)이었으며, 이중 특발성 세균성 복막염이 재발한 경우는 17예(25%) 이었다.

2. 재발된 특발성 세균성 복막염의 중상은 복통 12예(70.6%), 38℃ 이상의 발열 9예(52.9%), 복부 반발통 10예(58.8%), 간성혼수 3예(17.6%), 저혈압 2예(11.8%), 위장관출혈 4예(23.5%)로 복통 및 발열이 가장 흔한 증상이었다.

3. 재발된 특발성 세균성 복막염 17예 가운데 16예는 1회, 1예는 2회 재발하였다. 재발된 17예 가운데 8예(47.1%)는 6개월 이내, 11예(64.7%)는 1년 이내, 14예(82.4%)는 1년6개월 이내에 재발을 하였다.

4. 재발된 특발성 세균성 복막염의 입원시 사망률은 41.1% 이었고 17예 가운데 12예가 사망하여 사망률은 70.6% 이었다.

5. 재발된 특발성 세균성 복막염 환자의 간기능 상태는 Child's class A가 2예(3.9%), B가 17예(33.3%), C가 32예(62.7%) 이었고 처음 발병된 특발성 세균성 복막염도 이와 유사하며 의미있는 차이는 없었다(p>0.05).

6. 처음 발병된 특발성 세균성 복막염과 재발된 특발성 세균성 복막염의 임상양상 및 생화학적 검사소견을 비교하였을 때 유의한 차이는 없었으며 재발을 예측할 수 있는 인자는 없었다(p>0.05).

이상의 결과로 간경변증에 동반된 특발성 세균성 복막염의 발병률은 21%로 재발률은 25% 이었고 재발과 관련된 예견인자는 발견할 수 없었다. 따라서 간경변증에 동반된 특발성 세균성 복막염의 경우 재발을 조기 진단하기 위해서는 모든 환자를 대상으로 치료후 첫

6개월간 세심한 경과 관찰이 필요할 것이다.

Spontaneous bacterial peritonitis (SBP) is a frequent and severe complication of cirrhosis. In the most recently published studies, the prevalence of SBP among hospitalized cirrhotics with ascites has been estimated to be around 10-15%, the mortality rate related to this complication being more than 50%. SBP is thought to appear as a consequence of the impaired defensive mechanisms against infection present in cirrhotic patients, such as depressed reticuloendothelial system phagocytic activity, impaired leukocyte function, reduced serum compliment levels, and low antibacterial activity of ascitic fluid. It has proposed that ascitic fluid opsonin capacity is directly correlated to ascitic protein concentration and that this explains an observed predisposition to infection in patients with low ascitic fluid protein concentration. This present study aims to investigate the frequency of the recurrence of SBP in a large series of cirrhotic patient who recovered from the first episode of SBP and to identify any possible predictors of recurrence SBP. We reviewed records of chart in 312 consecutive cirrhotics with ascites patients treated in our hospital between January, 1988 and August, 1995. The incidence of SBP was 21.8%(68cases) and showed 80.9% in male, 19.1% in female. Seventeen(25%) of the 78 patients included in the study after the resolution of their first episode of SBP developed one or more episodes of SBP during follow-up. SBP recurred once in 16 of these patients, twice in 1 patients. The cumulative probability of SBP recurrence was 47.1% at 6 months, 64.7% at 12 months, and 875 at 18 months of follow-up. This study reveals that neither ascitic fluid total protein nor the severity of liver disease(Child's class) predicts the occurrence of recurrent SBP. We conclude that the occurrence of recurrent SBP is unrelated to the type of liver disease, and severity of liver disease did not predict the presence of recurrent SBP. Also, ascitic fluid total protein ≤ 1.0 g/dl, prothrombin time ≤45% may not be a sensitive predictor of recurrent SBP.
