지역사회 건강진단은 지역사회의 잠재적, 현시적 건강 문제의 원인 및 경향, 상태를 파악하고 그 문제 상황에 이름을 주는 단계로서 지역사회 건강상태에 대한 정확한 시각을 제공해주기 때문에 지역사회 간호 수행의 근본적인 선행조건이 된다. 특히 일차보건의료를 통해 그 활동 영역 및 역활이 증대되어 가고 있는 보건간호사들이 효율적인 건강관리 수행을 하는 데에 있어 지역사회 건강진단의 기술은 필수적이다.

그러나 지역사회 건강진단은 그 중요성이 널리 인식되어 온 것에 비해 그 동안 종종 피상적으로 다루어지거나 심지어 방관되어 왔다. 지역사회 분야에서는 현재까지 진단의 정의조차 확실히 개념화되지 않은 실정이며, 또한 그 진단 방법론에 있어서도 개인진단을 지역사회 진단에 적용할 수 있는지에 대해서도 의견이 분분한 상태이다. 더우기 우리나라 실정은 아직까지 지역사회 건강진단의 방법론에 대한 필요성 제기의 단계에 머무르고 있는 단계이다.

이에 본 연구는 지역사회 건강진단의 방법론을 개발하고자 하는 시도로 지역사회 건강진단을 위한 개념 및 모델 연구를 통해 새로운 지역사회 건강진단의 접근법을 개발하는데 주안점을 두었다

지역사회 건강진단을 위한 접근법을 개발하기 위하여, 첫째, 지역사회 및 지역사회 건강 개념의 정의를 시도하였으며, 둘째, 정의된 개념을 바탕으로 지역사회 건강진단을 위한 모델의 비교를 통한 지역사회 건강진단의 방법론을 개발하고자 하였다.

연구방법은 문헌고찰을 통해 세부목적의 순서에 따라 지역사회 건강진단을 위한 개념을 연구한 후, 이를 기초로 해서 지역사회 건강진단에 대한 모델을 분석, 비교하여 새로운 모형을 제시하는 순서로 진행하였다.

연구 결과는 다음과 같다.

지역사회 개념을 구성하고 있는 요소는 지리적 경계, 사회문화적 동질성 및 공통의 관심, 기능, 상호작용, 사회조직, 생활공간, 인구였으며, 위의 요소 가운데, 기능, 인구 요소를 필수 조건으로, 지리적 경계를 준필수 조건으로, 사회문화적 동질성, 공통의 관심을

충분 조건으로 선택하여 지역사회를 정의하였다.

지역사회 건강 개념을 구성하고 있는 요소로서 지역사회의 항상성, 평형, 적응, 기능 요소을 도출하였으며, 항상성, 평형, 적응을 포괄하는 요소로서 지역사회 기능 요소를 선택하여, 지역사회 건강을 정의하였다.

모델 연구에서는 본 연구에서 설정한 지역사회 건강의 개념을 설명할 수 있다고 생각되는 모델인 역학적 모델, 뉴만 모델, 일반체계 모델을 선정하여 지역사회 건강진단 내용을 비교하였다. 지역사회 건강진단의 요소 비교에서 각 모델의 공통점으로, 역학적 모델의 숙주 요소와 뉴만 모델의 기본구조 요소와 일반체계 모델의 하위 체계 요소가. 역학적 모델의 병인 요소와 뉴만 모델의 스트레스원 요소와 일반체계 모델의 투입 요소가, 역학적 모델의 환경 요소와 뉴만 모델의 방어선과 스트레스원의 상호관계와 일반체계 모델의 상호작용 요소가 일치하였다. 이를 기초로 하여 기본구조, 방어선(기본구조 방어선 및 환경 방어선), 스트레스원 요소를 지역사회 건강진단의 내용으로 하는 통합 모형을 개발하였다.

The purpose of this study is to establish concepts, compare models, and develop a new approach for community health diagnosis.

Community heal th diagnosis is defined as the identification of actual and potential heal th problems in communities or populations. It is a label that both describes a situation and implies an ethiology. Because the community health diagnosis gives direction to both community health nursing goals and intervention, community health diagnosis is the keystone of community health practice.

Although it is widely acknowledged that community health diagnosis is an essential precursor to community health nursing intervention, it still has ambiguous definition, unmeasurable goals, and a tenuous structure. In addition, the eclipse of community health diagnosis theory in the literature is so complete that some texts even exclude diagnosis as a stage of the nursing process applied to the community, and put it under the rubric of community assessment. Many people argue about the approaches in applying community health diagnosis: It is their opionion that nursing theory has not developed sufficiently to guide community health nurses in the application of the nursin diagnosis with in the community.

In an attempt to clarify the concepts and to develop an new approach of community health diagnosis, the sub-goals of thin study are:

1. To clarify and re-establish the concepts of community and community health.

2. To investigate models explainning community health, to compare them, and to draw common features in community health diagnosis.

3. To develop a new approach to community heal th diagnosis.

The study method and the results of the study are that:

1. The concepts of community and community health reviewed and then utilized for community heal th diagnosis.

2. The epidemiological model, Neuman model, and system model were studied and commonalities elicited.

3. A new approach for community health diagnosis was developed. In this approach, the basic components for identification of community health problems were core, community strength, and stressor factors.
