복막투석환자의 자가관리 이행정도에 관한 조사연구

문화와 의료의 발달과 함께 급성질환의 발생은 감소되고 있는 반면에 만성질환의 발생은 증가하고 있으며 최근 대부분의 사망원인이 되고 있다.

만성신부전은 환자가 치료를 이행하지 않는 경우 회복이 지연되고 질병을 악화시키게 되어 생명을 위협하므로 평생동안 치료이행과 자가관리를 계속 수행하여야한다.

이에 본 연구는 복막투석요법의 자가관리 중 무균적인 용액교환절차 및 도관출구관리의 이행능력을 향상시키기위한 교육프로그램개발의 기초자료를 제공하기 위하여 첫째 복막투석환자의 응액교환절차 및 도관출구관리의 자가관리 이행정도를 알아보고, 둘째 인구사회학적 특성 및 질병 제특성에 따른 자가관리 이행정도의 차이를 파악하고, 셋째 용액교환절차 및 도관출구관리의 자가관리 불이행현상을 파악하였다.

연구방법은 서술적 조사연구로서 1992년 4월 1일부터 4월 30일까지 4주 동안 G회사의 투석상담실에 등록되어 있는 서울시 거주자 30명을 대상으로 임의표출한 후 연구자가 직접 가정방문하여 면접, 관찰을 통해 자료를 수집하였다.

연구도구는 연구자가 문헌을 참고로 제작한 용액교환절차 및 도환출구관리 이행여부를 파악하기위한 Checklist를 사용하였으며, 이행하고 있지않은 문항에 대해서는 개방형 질문으로 그 이유를 대답하도록 하였다.

수집된 자료의 분석을 위하여, 서술적 통계량, 비모수 통계분석 중 Mann - Whitney test, Kruscal - Wallis test의 통계기법을 사용하였으며, 자가관리 이행현상을 묘사하기위해 내용분석기법을 참조하였다.

연구결과를 요약하면 다음과 같다.

1. 자가관리 이행정도는 평균 0.69점으로 보통이상의 자가관리 이행정도를 나타내었다.

이행정도가 가장 낮은 문항은 용액교환절차 중에서의 '마스크를 착용한다'가 평균 0.4점, 도관출구관리 중에서 '도관출구부위의 청결을 위해 매일 샤워한다'가 평균 0.07점으로 나타났다.

2. 자가관리 이행정도에 영향을 미치는 대상자의 특성으로는 성별, 직업, 주거사항, 경제상태, 손조작 능력장애, 시력장애 등 특성구분간의 평균간에 유의한 차이를 나타내지 않았다.

3. 자가관리 불이행현상에서는 대상자의 자가관리 불이행의 이유가 무균적인 개념에 대한 지식부족, 낮은 동기부여수준, 병원에서의 교육부족, 인식부족, 열악한 주거환경, 신체적 능력의 결함, 경제적인 부담 등으로 나타났다.

교육자로서 간호사는 용액교환절차 ��도관출구관리의 이행점수가 낮은항목부터 우선순위로 하여, 자가관리 불이행의 원인이 되는 사항을 재검토·확인한 후 자가관리 이행능력을 정확하게 사정하여 대상자에게 필요한 구체적인 도움을 제공함으로써 치료적인 자가관리 요구를 극소화시켜서 복막투석에 따른 자가관리를 잘 이행할 수 있도륵 도와야 하겠다.

This study was undertaken to provide preliminary data for the development of an education program for self-care performance promotion by CAPD patients which centers around the dialysate exchange procedure and catheter exit site care.

The purpose of this study was to evaluate self-care performance level and to describe self-care noncompliance phenomena in the dialysate exchange procedure and catheter exit site care.

The research design utilized in this study was a descriptive survey design.

A purposive sample of 30 patients with CAPD resident in Seoul was obtained from registered clients at dialysis service room in G-Company.

The data were collected from April 1 to April 30, 1992, through interview in which a questionaire was used and observation in which a checklist was used.

The instrument used in this study was a self-care performance scale for the dialysate exchange procedure and catheter exit site care which was prepared by the researcher.

The data were analyzed by using descriptive statistics, Mann - Whitney test and Kruscal - Wall is test with SPSS/PC+

The results are summarized as for lows:

1. The average level of self-care performance was 0.69, showing a relatively high level of self-care performance. The lowest score was for the item "wear a mask" with an average of 0.40 for the dialysate exchange procedure and "shower or wash the exit site every day" with an average of 0.07 was lowest for the catheter exit site care.

2. Client characteristics such as sex, occupation. socio-economic status, handling ability and visual impairment were not significant factors influencing the self-care performance level.

3. The reasons why self-care was net done were a knowledge deficit about the concept of aseptic care, low level of motivation, insufficient education at the hospital, a lack of understanding, inadequate housing, deficit in physical ability and economic hardship.

From the result of this study, if self-care performance in CAPD patients is to be promoted, the nurse as an educator must reinforce and teach knowledge and skills on items which showed lower scores on the self-care scale and support concrete helping

services and coping strategies to deal with therapeutic self-care demands.
