슬개건을 이용한 전십자인대 재건술시 Kennedy LAD 보강술의 효과

슬관절 전십자인대 재건슬시 Kermedy ligament-augmentation device의 보강 효과를 평가하고자 1970년 1월부터 1992년 12월까지 슬관절 전십자인데 손상으로 입훤 치료 받았던 43개를 .연구 대상으로 하였다. 자가 슬개건과 Kennedy ligament-augmentation device을

이용하여 전십자인대 재건술을 시행한 27레를 실험군으로 하고 자가 슬개건만을 이용하여 전십자인대 재건술을 시헹한 16례를 대조군으로 하여 평균 18 (12-35)개월 추시하였다.

실험군에서 수술 후 슬관절의 전방 불안전성을 평가하기 위하여 Lachma test, Pivot shift test등 이학적 검사와 Strker arthrometer, Dynamic radiograph를 시헹하였으며 기능적 평가을 위하여 Lysholm score, The Hospital for Special Surgery rating scale을 측정하여 대조군과 비교하였다. 비교 결과실험군과 대조군 간의 슬관절 전방 불안전성과 슬관절 기능에서 차이가 없었다. 자가 슬개건을 이용한 슬관절 전십자인대 재건술시 Keruedy ligamerltaugmentation device의 보강은 슬관절 전십자인대 재건술 후 슬관절 전방 불안전성과 슬관절 기능에 영향을 미치지 못하였다.

Forty-three patients with anterior cruciate ligament rupture were divided into two groups. The control group consisted of sixteen knees had repair with the bone-pater far tendon graft at one, and other group consisted of twenty-seven knees had repair wi th the bone-pated lay tendon graft and a Kennedy ligament-augmentation device. There was no difference between the two groups in the preoperative characteristics. Postoperatively, all patients in two groups were managed with the same program of range-of-motion exercises and physical therapy.

The duration of fort low-up averaged one and half years. Laxity was tested by pivot shift, the Stryker arthrometer and dynamic radiographs. Subjective knee function was classified using the Lysholm score and The Hospital for Special Surgery rating form. Functional or clinical difference between the two groups were

not statistical significant. The addition of the Kennedy ligament-augmentation device did not improve the funtional results and stability of bone-patellar tendon graft reconstruction in the treatment of the anterior cruciate ligament rupture.
