난관불임술후 자궁외임신 발생의 위험요인

Abstract

난관불임술후 자궁외임신 발생에 영향을 주는 요인을 밝히기 위하여 1983년도 대한 불임시술협회에 접수된 자궁외임신 414예와 서울특별시 ·경기지역 대조군 479예를 대상으로 본 연구를 실시하여 다음과 같은 결과를 얻었다.

1. 자궁외임신 발생의 비교위험도를 본 결과 도시거주부인이 농촌거주부인보다 1.321배로 높았고, 국민학교 교육수준이하가 중학교 교육이상보다 1.924배로 높았으며, 3명이상의 자녀를 가진 부인이 2명이하보다 2.008로 높았는데, 이들은 각각 95% 신뢰구간에서 통

계적으로 유의하였다.

2. 전기소작법 사용후에, 기타 방법을 사용한 경우보다 비교위험도가 5.756으로 높았으며, 95 % 신뢰구간에서 통계적으로 유의하였다. 이 비교위험도가 순수히 시술방법의 차이에 기인한 것인지 또는 confounder 들의 영향에 의한 것인지를 검토한 결과

가. 피임경험이 있는 군에서는 전기소작법에 의한 자궁외임신 위험이 기타 방법보다 8.488배 증가되었고, 피임경험이 없는 군에서는 6.439배 증가되었다. 조기비교위험도는 피임경험 표준화 비교위험도 7.560배 증가되었다. 조비교위험도는 피임경험 표준화 비교위험도 7.560(5.756/7.560) 23.9%나 축소 산출하고 있다.

나. 과거 인공유산 회수의 증가에 따라 전기소작법에 의한 비교 위험도는 점점 증가되어서 인공유산의 경험이 없는 군에서는 전기소작법후 자궁외임시 위험이 4.200배, 1∼2회 경험이 있는 군에서는 4.982배 증가되었으나, 3회이상 인공유산의 경험이 있는 군에서는 10.254배 증가되어 전기소작법에 의한 자궁외임신 위험이 2배이상으로 높게 증가되었다.

The risk factors of the ectopic pregnancy subsequent to tubal sterilization

To identify the risk factors of ectopic Pregnancy following tubal sterilization,

414 cases of ectopic pregnancy afters tubal sterilization and 479 randomly selected

controls were analyzed.

The results were as fellows;

1. The relative risk of ectopic pregnancy were 1.3 among urban women as compared

to rural women.

The corresponding relative risks were 1.9 for the women with primary education or

lower than the women with Secondary school education or higher, and 2.0 for the

women who had two children or less.

2. A) The relative risk was significantly higher as 5.8 after tubal coagulation

than after other methods.

This association was confounded by the past experience of contraception,

underestimating the adjusted risk by 23.9%.

B) The relative risk was also confounded by the past experience of induced

abortion.
