하악골은 저작, 발음, 호흡 및 안면표정등의 기능에 관여하며, 여러 근육 및 연조직에 의해 두개안면골에 연결되어 있는 동적인 골로 복잡한 성장 및 발육 과정을 보인다. 1771년 Hunter는 하악골은 하악지 전연의 골 흡수와 후연의 골 첨가에 의해 두개골에 대하여

후상방으로 성장한다는 보고를 하였다. Charles(1925)는 하악골 성장 및 발육에서 과두돌기의 중요성을 처음으로 지적 하였으며, 그후 Brodie(1941), Bjork(1955), Sarnart & Robinson(1756)등 많은 학자들은 하악골 과두돌기를 하악골의 성장점으로 인식하였다. 그러

나, 1960년 Moss는 기능적 기질 이론(functional matrix theory)에서 하악골의 과두돌기, 오훼돌기, 하악각, 하악체, 치조돌기부의 독립된 성장을 주장하였고, 각각 성장에 관여하는 대표적인 소골격 단위(microskeletal unit)로 측두근, 교근, 내측익돌근, 외측익돌

근, 치아, 하치조신경 및 맥관속 등을 제시하였다. 또 1987년 Precious와 Delaire는 두개악안면 성장에 있어서 두개안면 평형을 경조직인 구조체(arohitecture)와 주변 연조직 및 경조직의 구성체(structure)에서도 얻어야 한다고 주장하면서 Moss의 하악골 5개 부위의 독립적인 성장을 지지 하였다.

저자는 Moss, Delaire등의 제안에 기초하여 한국인 정상성인 하악골의 독립성장 부위의 절대 길이와 각 부위간의 상관성 그리고 각 부위 거리 비율의 상관 관계는 어떠한지를 확인하기 위하여 한국인 정상성인 57명의 여자와, 60명의 남자, 총 117명을 대상으로 이

들의 Panoramic 방사선 사진을 이용하여 소설첨부의 계측점에서 과두돌기, 오훼돌기, 하악각, 하악체부와 하악지의 길이를 측정 하였으며 또한 각 거리의 비율과 상관관계를 조사한 바, 다음과 같은 결론을 얻었다.

가. 한국인 정상성인에서 치조돌기부를 제외한 나머지 4개 부위 중 과두돌기, 오훼돌기, 하악체부가 길이의 비율에서 상관관계를 가지며 유의한 일정한 비율을 보였다.

나. 한국인 정상 성인에서

1. 각 계측 항목의 길이를 비교한 결과 남·여 간에는 통계적으로 유의한 차이가 있었다 (p<0.001).

2. 좌우측의 서로 대응되는 항목간의 길이의 상관성을 조사한 결과 상관성(r>0.7)은 매우 높았다 (p<0.001).

3. 오훼돌기에 대한 과두돌기 길이의 비율은 여성에서 1.2302±0.110, 남성에서 1.2277±0.120, 하악체에 대한 과두돌기 길이의 비율은 여성에서 0.5233±0.047, 남성에서 0.5266±0.043 그리고 하악체에 대한 오훼돌기 길이의 비율은 여성에서 0.4282±0.044 남성에서 0.4313±0.039였다 (P>0.05).





A STUDY OF MANDIBULAR GROWTH PATTERN IN KOREAN ADULT NORMAL FACES

Over the 200 years ago, Hunter(1771) recognized that mandibular bone actually

grows in a predominantly upward and backward direction toward the base of the

skull. Charles(1925) has described the importance of the condyle in the mandibular

growth. Most authors agree that the mandibular growth occur predominantly at the

condyle(Brodie, 1941, Bjork, 1955, Sarnart & Robinson, 1956), But recent

theoretical and experimental studies have fundamentally altered our understanding

the growth process of the mandible.

In opposition to the condyle as a primary site of mandibular growth, Moss (1960,

1968) postulated the functional matrix hypothesis that the mandible is composed of

various funtional components, including an alveolar portion, the coronoid process,

the gonial region, the condyle area, and corpus portion. Growth of the mandible is

the sum of growth of all of its functioal component which has independent growth.

He propesed that the relation of funtional components has an invariable ratio. A

similar conclusion reached by Precious & Delaire(1987).

The aim of this study is the observe the ratio and correlation between the

functional components of the mandible in Korean normal adults on the basis of the

Moss and Delaire's hypothesis. The author selected 57 females and 60 males who have

normal faces in Korean adults. Using the Panoramic X-ray, the author measured the

length the condyle, coronoid process, angle portion, corpus portion from lingular

and ramal height



The result as fellows :

1. There were significant invariable radio and correlation between the ratio of

length of condyle, coronoid and corpus portion.

2. In both sexes, the absolute length of ramus, condyle, coronoid, angular and

corpus were signifiantly difference.

3. There were correlation in the length of each counterpart between right and

left side.

4. The ratio between the length of condyle and coronoid was 1.2302±0.110 in the

female group and 1.2277±0.120 in the male group. The ratio between the length of

condyle and corpus was 0.5233±0.047 in the female group and 0.5266±0.043 in the

male group. The ratio between the length of coronoid and corpus was 0.4282±0.044

in the female group and 0.4313±0.039 in the male group.
