통상적 하악공 전달마취와 상악결절 접근법에 의한 하악마취의 비교연구

통상적 하악공 전달마취시의 불완전한 마취현상을 개선하고자 1977년 Akinosi가 하악신경 전달마취의 새로운 방법을 소개했으며, 이 방법은 하악신경에 근접된 주사 위치 때문에 실패율이 낮고, 한번 주사로 하치조신경, 설신경, 협신경을 마취시킬 수 있으며 마취

시술이 용이하고 특히 구강내 쳬구 접근법이므로 개구제한이 있는 환자에서는 매우 유리한 방법이다.

이에 저자는 새로운 하악신경 전달마취 방법인 상악결절 접근법의 암상적 평가를 위해, 상악 결절 접근법과 통상적 하악공 전달마취방법을 만 14이상의 105명의 환자에게 적용하여 그 마취효과를 비교, 연구한 결과 다음과 같은 결론을 얻었다.

1. 통상적 하악공 전달마취시의 마취 성공률은 88.0%이었으나 상악결절 접근법시의 마취 성공률은 91.3%이었다.

2. 통상적 하악공 전달마취시 혈액의 흡입율은 12%이었으나, 상악결절 접근법시의 흡일율은 8.7%이었다.

3. 마취의 발현시간은 통상적 하악공 전달마취시의 3분이내가 44%, 3분이상이 56%이었으며 상악결절 접근법시는 3분이내가 82.5%, 3분이상이 17.5%이었다.

4. 상악결절 접근법은 통상적 하악공 전달마취 방법보다 많은 장점이 있었으며 특히 개구제한이 있는 환자에서는 매우 유용하다.

In 1971 AKINOSI re77r1e6 a new method of mandibular nerve block using an

intraoral,closed-mouth approach.

According to his report, this technique is simple to administer, well accepted by

the patientand produce profound local anesthesia of inferior alveolar nerve,

lingual neHe and long buccalrlerve.

Especially this method can easily be employed in cases in which the i aw opening

is limited.Author feyiewed 105 Patients, OY?f the 14 yell 7ld, W?f? divided into 2

group,25 Patientsadministered the conventional inferior alveolar nerve block, as

control group and 80, experimentalgroup was utilized the tuberosity approch

technique.

The following results were obtained:

Success rate of anesthesia in the conventional technique was 88.0% while in the

tuberosityapproach technique showed 91.3%.

Rate of positive blood aspiration in the conventional technique was 12.07o, the

tuberosityapproach technique was 8.7%.

The onset of anesthesia in the tuberosity approach technique was faster than in

the con-ventional technique.

By using an intra oral, closed-mouth approach, lite tuberosity approach technique

has manyadvantages than the conventional technique, and proved especially valuable

in patient withlimited mouth opening.
