1970년 1월부터 1980년 12월까지 만 11년 동안 고려병원에 입원한 장중첩증 환아 161예에 대한 임상적 관찰과 치료성적에 관한 분석을 통하여 아래와 같은 결과를 얻었다.

환자는 1세 미만이 전 예의 80.2%를 차지하였으며 그 중에서도 생후 7개월부터 9개월 사이에 발생율이 31.7%로 가장 높았다. 남녀비는 2:1로 남자가 많았으며 계절별로는 춘계에 36.6%로 가장 발생율이 높았다. 원인으로는 원인불명이 94.4%로 대부분을 차지하였으

며 발육 및 영양상태는 한국소아 체중 정상발육 표준치와 비교할 때 50백분지수 이상이 72.7%였다. 임상증상으로는 구토가 89.4%로 가장 흔히 관찰되었으며 장중첩증의 유형은 회장결장형이 71예(44.1%)로 가장 많았다. 치료는 고압바리움 관장법에 의하여 70.8%가 정

복되었고 발병 후 경과시간이 짧을수록 정복율이 높았다. 재발율은 7.5%였으며 사망은 수술후 1예(0.6%)에서 발생하였다.

Intussusception is an invagination of a segment of the intestine into an adjacant

bowel segment.

The occurrence of Intussusception in infants and children constitutes one of the

commonest abdominal emergencies and is seen most frequently in the first two years

of life.

161 cases of intussusception who were admitted to the department of pediatrics

and surgery of Korea General Hospital during the period from Jan. 1970 to Dec.

1980, were analyzed clinically and the following results were obtained.

1. In age distribution, 80.2% of the patients were under one year of life.

The peak incidence occurred between 7 months and 9 months of age.

2. The sex incidence revealed male predominance than female.

Male to female ratio was 2:1.

3. In seasonal incidence, it was higher in spring than other seasons.

4. Etiologic factors were found only in 5.6% of the patients, the causes were not

determined in remaining cases.

5. The 72.7% of the patients were above 50 percentile of norm weight value of

Korean infant and children.

6. The 4 cardinal symptoms and signs were vomiting (89.4%), mucous bloody stool

(83.9%), intermittent irritability and abdominal pain (80.1%), and abdominal mass

(67.7%).

7. In analysis of the types of intussuseption, the most common type was ileocolic

type (44.1%).

Ileocecal type was 39.1%, ileoileal type was 6.2%, and colocolic type was 5.0%.

8. In 161 cases of intussusreption, 114 cases (70.8) were reduced successfully by

hydrostatic barium reduction.

9. Recurrence rate was 7.5%.

10. Mortality rate in 161 cases of intussusception was 6%.
