한국인 성인층에서의 조절과 반복적인 안압 측정이 안압에 미치는 영향

안압의 정확한 측정은 녹내장의 진단과 치료에 중요한 검사이다. 안압측정시 안압에 영향을 미치는 요인으로 조절시의 안압변화와 반복적인 안압측정으로 인한 안압변화는 초기녹내장, 고안압증, 저안압녹내장환자에서의 진단과 치료의 기준에 오류를 초래할 수 있기

때문에 정확한 진단과 치료를 위하여서는 조절과 반복적인 안압측정이 안압에 미치는 영향을 연구 분석할 필요가 있다.

이에 저자는 1986년 5월 1일부터 1986년 9월 30일 사이에 연세대학교 의과대학 부속 세브란스병원과 영동 세브란스병원의 직원과 안과외래내원환자 중 20세이상 45세이하의 정시안환자 120명을 대상으로 조절을 이완시킨 상태와 조절을 유발시킨 상태에서 Gold-mann applanation 안압계와 Schiotz 안압계를 사용하여 안압을 반복적으로 측정함으로써 조절과 반복적인 안압측정이 안압에 미치는 영향을 알아 본 결과 다음과 같은 결론을 얻었다.

1. Goldmann applanation 안압계에 의한 반복적인 안압측정시에 조절이완상태에서는 안압의 산술평균값간에는 유의의 차이가 없어서 안압하강의 경향을 보이지 않았으나 조절유발상태에서는 유의의 안압하강을 보여 반복적인 안압측정에 의하여서가 아니라 조절에 의

하여서 안압이 하강됨을 보여 주었다.

4분간의 조절에서 최대안압하강치는 평균 3.25 mmHg이었고 조절 1분후에 평균 2.01 mmHg의 안압하강을 보여 전체안압하강 중 가장 큰 폭으로 안압이 떨어짐을 보여 주었다.

2. Schiotz안압계에 의한 반복적인 안압측정시에서도 조절이완상태에서는 안압의 산술평균값간에는 유의의 차이가 없어서 안압하강의 경향을 보이지 않았으나 조절유발상태에서는 유의의 안압하강을 보여 반복적인 안압측정에 의하여서가 아니라 조절에 의하여서 안압의 하강됨을 보여 주었다.

The Effects of Accommodation and Repeated Tonometry on Intraocular Pressure in

As accommodation and repeated tonometry were known to change the level of

intraocular pressure, sometimes they may give a misleading indication for the

diagnosis and management of early glaucoma, ocular hypertension and low tension

glaucoma.

The object of this study was to investigate the effects of accommodation and

repeated tonometry on intraocular pressure in korean adults.

Repeated tonometry was performed on 120 normal korean eyes with visual acuity of

20/20 or more under controlled condition of accommodation and relaxation using

Goldmann applanation tonometry and Schiotz tonometry.

The results were as follows

1. In repeated Goldmann applanation tonometry, a statistically significant

reduction in mean intraocular pressure was produced during accommodation but not

during relaxation. The maximum reduction of 3.25 mmHg was produced during four

minutes of accommodation and significant reduction of 2.01 mmHg was shown in first

one minute of accommodation.

2. In repeated Schiotz tonometry, a statistically significant reduction in mean

intraocular pressure was produced during accommodation but not during relaxation.

The maximum reduction of 3.71 mmHg was produced during four minutes of
