고압산소요법이 백서 임의형 피부판 생존율에 미치는 영향

피부판의 생존은 성형외과 영역에서 중요한 과제이며, 피부판의 괴사를 막고 생존을 증가시키기 위한 방법의 하나로 조직에 필요한 산소를 공급한다는 점에서 고압산소요법이 사용되게 되었다. 고압산소요법이 피판 생존율을 증가시키는 기전으로는 혈류내의 산소량을 증가시켜 피판 원위부에 충분한 산소를 공급하는 것과 조직으로 직접 산소가 흡수되는 것 등으로 설명되곤 있으며 최근 독성없이 피부판 생존을 증가시킬 수 있는 압력, 회수 및 노출시간에 대한 연구가 활발해지고 있다.

본 실험에서는 100 %산소 2기압에 하루 2회씩 노출시키되 매번 노출시간을 각각 3(1분, 2시간, 4시간으로 달리하고 노출일수를1일. 3일, 5일 7일로 달리하여 피부판 생존을 증가시킬 수 있는 시간대를 찾고자 하였으며, 다음과 같은 결과를 얻었다.

1. 피판 수술 후 100% 2기압산소에 2시간씩 1일2회 3일, 5일,7일간 노출시킨 실험군 및 4시간씩 1일 2회 1일간 노출시킨 실험군에서 생존율이 증가하였다( P〈 0.01 ) . 30분씩 1일2회 1일, 3일, 5일, 7일간 노출시킨 실험군 및 2시간씩 1일2회 1일간 노출시킨 실험군에서는 생존율의 의의있는 증가를 볼 수 없었다.

2 .생존율이 증가된 군돌 사이에서 3일째 이후 노출일수 증가에 의한 생존율 증가는 없었다.

3 .고압산소에 노출된 총시간이 같더라도 수술 이후에 노출된 경우에서 피부판 생존율 증가 효과가 컸다.

The survival of skin flap is important aspect of plastic surgery and to prevent necrosis and to increase the survival rate of the skin flap, hyperbaric oxygen therapy huts been introduced as a method for providing oxygen necessary to tissue.

Recently, the study about the pressure, frequency, exposure time of hyperbaric oxygen therapy to increase flap survival without oxygen toxicity has been made.

In this experiment, the random skin flap was used and the effect of hyperbaric oxygen therapy with various exposure time and day was compared. The following results were obtained.

1. In Group Ⅵ, Ⅶ, Ⅷ, Ⅸ(2 atm, 100% hyperbaric oxygen,2 hrs: 3D., 5D., 7D. ? 4 hrs: ID.). the average survival rate was significantly increased (P 〈 0.01).

There was no significant increase of flap survival rate in Group Ⅰ, Ⅱ, Ⅲ, Ⅳ, Ⅴ(2 atm, 100% hyperbaric oxygen,30min.; 10., 30., 5D., 7D., & 2 hrs; ID.). 2. There was no significant increase of flap survival after3 day-exposure period according to the increase of oxygen exposure day.

3. The effect of hyperbaric oxygen to increase the survival rate of flap was higher in groups which were exposed at immediate postop. period than in groups which were exposed later.
