뇌동맥류성 지주막하 출혈후 혈중 전해질 농도 변화에 대한 임상적 고찰

동맥류성 지주막하 출혈 후에 발생되는 전해질 불균형과 뇌혈관 연축과의 관계는 아직 연구가 미비한 상태이다. 따라서 저나트륨혈증을 치료하기 위하여 수액과 전해질을 공급하여야 하는지, 또는 수액제한이 적절한지는 아직도 논란의 대상이 되고 있어서 의사로 하여금 진퇴양란에 빠지게 한다.

이에 저자는 1985년 1월 1일부터 1986년 12월 31일까지 2년간 뇌동맥류성 지주막하출혈로 전주 예수 병원 신경외과와 연세대학고 의과대학 부속 세브란스 병원 신경외과에 입원, 치료받은 환자에서 전해질 불균형의 발생, 치료, 예후를 후향적으로 관찰하여 다음과 같은 결과를 얻었다.

1. 전해질 불균형의 발생율과 delayed ischemic deficit의 발생율은 시상하부와 인접부위의 뇌동맥류 출혈에서 높으며, 양자는 서로 비슷한 시간에 시작되고 같이 나타나는 경향이었다.

2. 전해질 불균형의 발생과 뇌동맥류연축의 유발요인과는 밀접한 관계가 있어서 Hunt & Hess의 분류에 의한 등급이 높을수록, Fisher 등급이 높을수록, 또한 기저조 출혈이 있을 때 전해질 불균형의 빈도가 높았다.

3. 저나트륨 혈증의 치료로서 일상적인 수액공급과 나트륨보충을 하였는데, 예후가 전해질 불균형이 없는 집단과 같고, 저나트륨 혈증의 기간이 철저한 수액제한으로 치료한 보고들의 것과 다르지 않았다.

이상의 결과로서 뇌동맥류 파열 후 발생되는 전해질 불균형은 뇌혈관연축과 임상적으로 상관관계가 있다고 사료되며, 저나트륨혈증의 치료방법으로 수액공급과 나트륨보충도 치료에 도움이 된다고 생각된다.

Author analyzed 173 patients diagnosed as and treated for aneurysmal subarachnoid hemorrhage at the Department of Neurosurgery in Presbyterian Medical Center and Yonsei University, Severance Hospital from January 1, 1985 to December 31, 1986.

Result of the analysis is summarized as follows;

1. The electrolyte imbalance and delayed ischemic deficit have significant relationship. They higher incidences in reptured aneurysms adjascent to hypothalamus, onset at similiar time and have tendency to accompany each other.

2. The precipitating factors of vasospasm and electrolyte imbalance have significant relationships. The incidence of electerolyte imbalance is higher in the group of high Hunt & Hess grade, high Fisher grade, presence of basal cistern hemorrhage.

3. Inspite of ordinary fluid and sodium suppliment, the hyponatremia group have the same prognosis as that of normal electrolyte concentration, and no longer duration of hyponatremia than that of strict fluid restriction groups that reported by other authors.
