Abstract
[한글]
HBsAg carrier는 일반적으로 혈액등에 B형간염바이러스가 3개월 혹은 6개월 이상 지속적으로 양성으로 나타나는 경우를 말하는데 대개 증상이 없기 때문에 간기능검사를 시행하지 않는경우가 많으며 더구나 간기능검사를 시행하여 간기능이 정상인 경우에는 간의병변이 없는 것으로 간주하기 쉽다. 또한 자각증상이 없는 HBsAg carrier에서의 간침생검에 의한 병리조직학적소견은 보고자에 따라 큰차이를 보이고 있다.
이에 저자는 1984년 6월부터 1984년 10월까지 연세대학교 외과대학 부속 세브란스병원에 입원하여 이학적 검사 및 간주사상 간종대가 없고 간기능검사소견이 정상이며 과거력상 6개월 이내에 급성간염의 병력이 없고 HBsAg이 지속적으로 양성인 currier 41명을 대상으로 B형간염바이러스표지자, 연령 및 성별분포, 직업, HBsAg을 검사하게 된 동기, 복강경검사소견, 병리조직학적소견등을 관찰하여 다음과 같은 결론을 얻었다.
1. B형간염바이러스표지자는 HBsAg양성인 41예 전예에서 anti-HBc도 양성이었고 anti-HBs는 음성이었다. HBeAg과 anti-HBe 는 41예중 33예에서 시행하여 HBeAg양성이 15예 ( 45.4%), anti-HBe양성 16예( 48.5%), 양자 모두 음성이 2예( 6.1% )이었다.
2. HBsAg carrier외 연령분포는 17세에서 52세까지로 평균 32.2세이었으며 20대와 30대가 가장 많아 전체의 70.7 %를 차지하였고 남녀비는 1.73대 1로 남자에 많았으며 HBsAg carrier의 직업은 회사원이나 공무원이 23예로 가장 많아 56.1%를 차지하였다.
3. HBsAg을 검사하게 된 동기는 신체검사에서 우연히 발견된 경우가 21예로 가장 많아 51.3%이었다.
4. HBsAg carrier의 복강경검사소견은 41예중 19예 즉 46.4%에서 만성간질환의 소견을 보였다.
5. 병리조직학적소견은 41예중 14예 즉 34.1%에서 만성간질환의 소견을 보였고 병리조직학적으로 정상인 경우는 17.1% 밖에 되지 않았다.
6. HBeAg 양성 15예중 만성간질환이 7예 (46.7 %) , 비특이성 소견이 8예(53.3%)이었고 anti-HBe양성 16예중에는 만성간질환이 3예 ( 18.8 %)이었고 비특이성소견이 13예 ( 71.2%)로서 HBeAg양성군에서는 만성간질환보다 비특이성소견이 비슷한 빈도를 나타내었고 anti-HBe양성군에서는 만성간질환보다 비특이성소견의 빈도가 더 높은 양상을 나타내었다.
이상의 결과를 종합해 보면 HBs Ag carrier의 복강경소견은 46.7 %에서 만성간질환의 소견을 보였으며 병리조직학적소견은 34. 1 %에서 만성간질환의 소견을 나타내었고 병리조직학적으로 정상인 경우는 7.1 %에 불과하였다.
HBsAg 양성군에서는 만성간질환과 비특이성 및 정상소견이 비슷한 빈도를 나타내었고 anti-HBe양성군에서는 만성간질환보다 비특이성 및 정상소견의 빈도가 더 높은 양상을 나타내었다,
결론적으로 HBsAg carrier는 간기능검사치가 정상이고 간종대가 없는 경우에도 34.1 %에서 만성활동성간염 및 간경변증. 만성활동성간염 또는 만성지속성간염등이 있어 carrier의 상당수가 발견당시 이미 만성간질환으로 이행되었음을 알 수 있었으며 간기능검사치가 정상이고 간종대가 없는 carrier라 하더라도 간경변증이 없는 것으로 간주하지말고 간침생검을 시행하여 실제의 병변을 정확히 파악해야 될 것으로 생각된다.
또한 간침생검의 병리조직학적소견이 정상인 경우에도 정기적인 추적관찰을 하여 만성간질환으로의 이행여부와 시기를 파악하는 것이 carrier의 관리에 도움이 될 것으로 사료된다.

[영문]
The hepatic status of HBsAg carrier is still controversial. To clarify this issue a prospective study was performed in 41 HBsAg carriers whose liver functions were normal and who had no hepatomegaly on examination and liver scan. All HBsAg carriers underwent peritoneoscopy and liver biopsy using Vim-Silvermann needle.
Viral markers, age and sex distribution, occupations, reasons for HBsAg check, peritoneoscopic findings, pathologic findings, comparison of peritoneoscopic with pathologic findings, and pathologic findings according to HBeAg and anti-HBe status were analyzed in HBsAg carriers.
The results were as follows :
1. HBsAg and anti-HBc were positive and anti-HBs negative in all 41 carriers. HBeAg and anti-HBe were checked in 33 among 41 carriers. HBeAg was positive in 15 cases (45.4%), anti-HBe positive in 16 cases (48.5%), and both negative in 2 cases (6.1%).
2. The 3rd and 4th decades had peak incidence (70.7%) and the male outnumbered the female at a rate of 1.73 to 1.
3. In 46.7% of the carriers, the peritoneoscopic findings were consistent with chronic lover diseases.
4. In 34.1% of the carriers, the pathologic findings were consistent with chronic liver diseases.
5. Of the 15 cases with HBeAg positive, 7 cases (46.7%) had chronic lover diseases, 5 cases (33.3%) nonspecific findings, and 3 cases (30.0%) normal liver. Of the 16 cases with anti-HBe positive, only 3 cases (18.8%) had chronic liver diseases, 11 cases (68.8%) nonspecific findings, and 2 cases (1.4%) normal liver.
In conclusion, a significant number of HBsAg carriers had chronic liver diseases even though their functions were normal and they had no hepatomegaly. It is recommended that a liver biopsy be done In every HBsAg carrier although his lover functions are normal and he has no hepatomegaly.
