전신성 홍반성 낭창 환자에 있어서 IL-6의 과잉 생성

전신성 홍반성 낭창은 혈관과 결체 조직등을 침범하여 악화와 호전이 반복되는 경과를 지니는 만성적, 전신적 질환으로서 그 병태 생리로는 여러 의견이 있으나 근래에 와서는 자가 면역성 질환으로 분류된 후 면역학적 연구가 급진전 되어 왔다.

이 질환에서 자가 면역 질환의 면역학적 이상소견으로는 B세포의 과도한 활동성, 고감마글로블린 혈중, DNA나 RNA 그리고 세포막 구조물에 대한 비특이적인 자가 항체 등이 있는데 이러한 면역학적 변화는 B세포의 과잉 생성으로부터 기인한다고 생각되어 지며 근자에 와서 T세포나 단핵세포 및 혈관 내피세포 등에서 형성되는 cytokine인 Interleukin-6(IL-6)가 자가 면역 질환에서 자가 항체생성에 중요한 것으로 알려지고 있다. 이에 자가 면역 질환의 대표적인 예인 전신성 홍반성 낭창 환자에서 IL-6의 혈중 농도를 측정하여 과잉 생성 여부를 확인하고 C-reactive protein(CRP), 혈중 면역 복합체, 혈소판 수 및 soluble interleukin-2 receptor(sIL-2R)와 비교 분석하였다.

1986년 7월부터 1987년 9월까지 세브란스 병원에 내원하여 전신성 홍반성 낭창으로 진단 받은 환자 22명을 대상으로 하였고 환자의 혈액을 체취하여 혈청 분리후 -70℃에 보관하였다가 검사를 시도하였으며 정상인 10명의 혈액을 채취하여 대조군으로 사용하였다.

IL-6는 Genzyme Corp.사의 ELISA kit을 이용하여 ELISA방법에 의해 각 혈청의 IL-6치를 측정하였고 CRP는 TDx 기기를 이용하여 fluorescence polarization immunoasaay 법으로 측정하였다. 혈중 면역 복합체는 solid phase C1q binding assay 방법으로 측정하였고 sIL-2R는 T cell science사의 ELISA kit를 이용하여 ELISA방법으로 측정하였으며 채혈 당시의 임상 검사 소견과 비교 조사 하였다.

혈중 IL-6치는 총환자 22명 중 11명에서 현저하게 증가되어 있었고 환자들의 평균치는 1,366pg/ml로 정상인들의 평균치인 98pg/ml보다 통계적으로 의의있게 증가되어 있었다. 혈관염이 동반되어 있는 환자에서 동반되어 있지 않은 환자보다 IL-6치가 증가되어 있었으며, 면역 복합체 수치와 혈청 IL-6치와는 직선적인 상관 관계를 보였다. IL-6치와 CRP와는 상관 관계가 없었다. 혈중 sIL-2R의 평균치는 1,864U/ml로 정상인의 평균치인 300U/ml보다 증가되어 있었으나 혈청 IL-6치와의 상관 관계는 없었다.

결론적으로 전신성 홍반성 낭창에서 IL-6치가 정상에 비해 증가되어 있으며 이것이 본 질환을 일으키는 병태 생리에 중요한 역할을 하는 것으로 사료된다.

[영문]

Systemic lupus erythematosus (SLE) is a disease affecting blood vessels and connective tissue, which are damaged by deposition of pathogenic autoantibodies and immune complexes. Although a complex disease, SLE provides a number of insights into autoimmune pathogenesis. Autoimmune disease, in general, is characterized by B cell hyperactivity which results in hypergammaglobulinemia and production of a variety of autoantibodies reactive to organ-nonspecific antigens such as DNA, RNA, and cell membrane structures. SLE patients usually show a marked increase in the number of activated and immunuglobulin-producing circulating B cells. Recently, cytokines with specific effects on immune regulation have been detected and extensively studied. One of them, Interleukin-6 (IL-6), is an activated monocyte derived factor which stimulates B cell growth and differentiation.

We investigated the senum IL-6 levels of SLE patients in an attempt to demonstrate their relationship with the patients' clinical manifestation, and the serum levels of C-reactive protein (CRP), circulating immune complexes (CICs), and soluble interleukin-2 receptor (sIL-2R).

The study subjects consisted of 22 patients with SLE who had visited Severance Hospital from July 1986 to September 1987 and 10 normal controls. The patients' sera were stored at -7O℃ and later analyzed.

The serum levels of IL-6 were measured by ELISA method with InterTest-6X Human IL-6 ELISA kit; the serum CRP levels by fluorescence polarization immunoassay; the serum CIC levels by solid phase Clq binding assay; and the serum sIL-2R levels by ELISA method.

The results were as follows:

The mean serum IL-6 level of SLE patients (1,366pg/ml) was higher than that of the controls (98pg/ml) (p<0.05). Among the SLE patients studied, the mean serum IL-6 level was higher in those with vasculitis than those without. In the SLE patients studied, a linear correlation was present between the measured serum IL-6 and CIC levels: however, no correlation was present either between IL-6 and CRP levels, or between IL-6 level and platelet count. The mean sIL-2R level of the SLE patients studied (1,864U/ml) wag higher than that of the controls (300U/ml). However, in the SLE patients studied, no correlation was present between the serum IL-6 and sIL-2R levels measured.

The high serum IL-6 level might play an important role in the pathogenesis of SLE .
