상피성 난소암은 높은 CA 125 양성율을 나타내며 그 농도의 증감은 상피성 난소암의 진행 및 퇴행과 깊은 연관성이 있음이 보고되고 있다. 본 연구에서는 부인과 환자 중 상피성 난소암을 포함하여 양성 및 악성 질환 92예를 대상으로 수술 전에 CA 125의 농도를 측정하여 CA 125의 양성 하한선을 35 U/ml로 설정하였을 때 상피성 난소암 13예 중 10예(76.9%)가 양성 반응를 보였으며 자궁경부암 15예, 자궁내막암 1예에서는 전혀 양성율을 보이지 않았으며 부인과 양성질환 63예 중 4예(6.3%)에서 양성으로 나타났다. CA 125의 양성 하한선을 65 U/ml로 설정하였을 경우 상피성 난소암의 13예 중 6예(46.2%)에서 양성으로 나타났으며 부인과 양성 질환 모두에서 음성으로 나타났다. CA 125의 양성 하한선을 35 U/ml로 설정하였을 때 상피성 난소암을 진단하는데 있어 특이도는 93.7%, 민감도는 76.9%이었다. 병리조직학적 분류에 따른 상피성 난소암의 CA 125에 대한 양성율은 장액성 낭성암 8예 중 6예(75.0%), 점액성 낭선암 3예 중 2예(66.7%), 자궁내막양선암 2예 중 2예(100%)에서 양성으로 나타났으며 암의 기별에 따른 CA 125에 대한 양성율은 암이 진행됨에 따라 양성율이 높은 경향을 보였다. 상피성 난소암 환자에서 수술 전 CA 125 농도와 수술 후 임상 경과와의 연관성을 보면 수술 전 CA 125의 65U/ml 이하인 경우 65U/ml 이상인 경우에 비하여 수술 후 임상경과에 있어 유의한 차이가 있었다. 즉 수술 전 CA 125의 농도가 높을수록 수술 후 암이 지속 또는 진행되는 경향을 보였으며 항암화학제에 대한 반응이 좋지 않았다. 수술 전 CA 125 농도가 35 U/ml 이상이었던 상피성 난소암 환자에서 CA 125의 농도가 두배 이상 증가 혹은 50% 이상 감소하였을 경우 CA 125 농도가 유의하게 증감한 것으로 간주하여 수술 1주일 후 CA 125의 농도 변화와 수술 후 임상경과와의 연관성을 조사하여 통계학적으로 분석하여 보면 수술 전후 CA 125의 농도 변화와 수술 후 임상 경과와는 연관성이 없었다. 이상의 연구결과로써 상피성 난소암은 높은 CA 125 양성율을 보이며 특히 비점액성 상피성 난소암의 조기 진단에 임상적으로 유용하다고 사료되며 상피성 난소암 환자에서 수술 전 CA 125의 농도는 암의 파급 정도와 연관성이 있으므로 예후 판단에 이용될 수 있으리라고 생각된다.

CA 125 is an antigen associated with human epithelial ovarian cancers. Increase in CA 125 levels has been found in over 80% of patients with epithelial ovarian cancer. Moreover, concentrations of CA 125 have been correlated with tumor progress. In the present study, serum CA 125 levels were evaluated preoperatively

in 92 patients with gynecologic diseases and one week postoperatively in 10 patients with epithelial ovarian cancer. Ten of 13 patients (73.9%) with epithelial ovarian cancer had serum CA 125 levels greater than 35 U/ml preoperatively. In 4 of

63 (6.3%) patients with benign gynecologic diseases, CA 125 was greater than 35 U/ml. If the upper limit of normal was set at 65 U/ml, none of 63 patients with benign diseases had elevated serum CA 125 levels, whereas 6 of 13 (46.2%) epithelial ovarian carcinoma patient sera remained positive. Among 15 patients with

cervical carcinoma and 1 patient with endometrial carcinoma, none had serum CA 125 levels greater than 35 U/ml. Using a cutoff level of 35 U/ml, the results of the preoperative assay can be characterized by a 93.7% of specificity and 76.9% of sensitivity to detect epithelial ovarian cancer. among 13 patients with epithelial ovarian carcinoma, 6 of patients (75%) with serous cystadenocarcinoma, 2 of 3 patients (66.7%) with mucinous cystadenocarcinoma and 2 of 2 patients(100%) with endometrioid carcinoma had elevated serum CA 125 values if the upper limit of

normal was set at 35 U/ml. There was an increasing incidence of elevated levels of CA 125 in relation to a higher tumor stage. The relationship between the preoperative serum CA 125 levels and the clinical course of th epithelial ovarian carcinoma showed that if the preoperative value was less than 65 U/ml, the disease

regressed in all of 7 patients. When the preoperative value was greater than 65 U/ml, the disease regressed in two of 6 patients. High preoperative values of CA 125 appeared to predict a poor response to treatment and a high recurrence rate. Serum CA 125 levels were measured at one week after cytoreductive surgery in 10 patients with epithelial ovarian carcinoma who had preoperative CA 125 levels in excess of 35 U/ml. If a doubling or halving of antigen levels is considered to be a significant change, these alterations in CA 125 levels after one week of

cytoreductive surgery did not correlate with the clinical course. It is, therefore, suggested that CA 125 antigen is useful for early diagnosis of nonmucinous epithelial carcinoma and preoperative values have prognostic significance in predicting response to treatment.
