베체트 증후군의 혈액내 임파구와 면역글로부린에 관한 연구

Abstract

[한글]베체트 증후군은 만성적으로 여러 장기를 침범하는 증후군이며 원인불명의 반복적인 염증반응을 특징으로 한다. 발병기전에 대해 여러 가지로 논의되고 있으나 특히 면역학적 측면이 최근 활발히 연구되고 있다.

저자는 베체트 증후군의 면역학적 연구의 일환으로 검사당시의 주증상 발현양상에 따른 혈액내 임파구와 면역글로부린에 대한 연구를 시행하였다.

연구대상은 세브란스병원 베체트 증후군 특수진료실에 내원한 불완전형 이상의 129명의 환자와 베체트 증후군의 가족력이 없는 건강한 정상 한국인 19명을 대상으로, E-rosette법에 의한 총 T임파구 정량, 단일클론항체법에 의한 OKT^^4와 OKT^^8 세포의 정량, EAC-r

osette법에 의한 B임파구 정량, PHA에 의한 임파구 전환능 시험(Lymphocyte transformation test), 전기영동법에 의한 감마글로부린 정량과 면역확산법에 의한 IgG, IgA, IgM, C3, C4 정량을 시행하여 다음과 같은 결과를 얻었다.

1. 대상환자는 남자 53명, 여자 76명으로 1:1.5의 비율이었으며, 완전형 32명, 불완전형 97명으로 1:3의 비율이었다. 대상환자의 평균연령은 35.3세로 남녀간의 평균연평의 차이는 없었다.

2. 대상환자의 평균 총 T임파구와 OKT^^4세포의 수는 각각 71.4±7.7%와 41.6±13.5%로 정상대조군의 73.6±6.1%, 60.7±7.2%에 비해 감소하였고, OKT^^8세포와 B임파구의 수는 각각 24.2±11.8%, 18.4±5.5%로 정상대조군의 22.0±5.7%, 10.4±1.7%에 비해 증가하였

으며 OKT^^4/OKT^^8 비는 2.1±1.3으로 정상대조군의 2.7±0.6에 비해 저하되어 모두 통계적 유의성을 보였다(P<0.05). PHA에 의한 임파구 전환능은 control stimulation index에 비해 1.2±1.5의 비율로 증가되어 있었다. 그러나 T 및 B임파구의 면역학적 변화는 검사 당시의 주증상 갯수나 주증상 발현현상에 따르는 차이나 일정한 경향을 보이지 않았다.

3. 혈청내 면역글로부린 및 보체의 정량에 있어 대상환자의 평균치는 모두 정상범위내에 있었으나, 검사당시 주증상 갯수나 발현양상에 따라 보았을 때는 주증상 수가 3개 이상인 경우 감마글로부린, IgG , IgA , IgM, C3 ,C4 모두에서 평균치에 비해 낮은 양상을 보였고, 이 중 IgA와 C3는 주증상이 4개인 군에서 통계적으로 유의한 감소를 나타냈다.

4. 검사당시의 각 주증상별 유, 무에 따르는 T 및 B임파구 정량 및 임파구 전환능 시험 결과는 유의한 변화가 없었다.

5. 검사당시의 각 주증상별 유, 무에 따르는 혈청내 면역글로부린및 보체 정량에서는 안증상이 있을때가 없을때에 비해 모든 측정치가 감소하였고 이 중 감마글로부린, IgG는 현저히 감소하여 통계적 유의성이 있었다.

이상의 결과로 보아, 베체트 증후군에서 세포매개면역은 주증상 개수, 주증상 발현양상, 각각의 주증상 유, 무에 의해 크게 영향받지 않으나 체액면역의 경우는 이들에 의해 영향을 받을 것으로 사료되었다.

[영문]Behcet's syndrome is a chronic multisystemic disorder which is characterized by a relapsing inflammatory process of unknown etiology. Many hypotheses have been brought up for the explanation of the cause. However, immunologic aspect is under

the most active research currently.

In our investigation, various immunologic aspects of 129 patients with complete and incomplete type of Behcet's syndrome were taken under consideration : quantitations of total T-cells by E-rosette method and B-cells by EAC-rosette method, helper/inducer T and suppressor/cytotoxic T-cell by monoclonal antibody technique(OKT^^4, OKT^^8), lymphocyte transfomation test with PHA, quantitation of serum r-globulin by electrophoresis and quantitations of serum IgG, IgA, IgM, C3, C4 by immunodiffusion.

The results are as follows :

1. The male to female sex ratio was 4:6, the complete to incomplete type ratio was 1:31,the mean age was 35:3 years, and there was no significant difference in the mean age of both sexes.

2. The means of the whole patients' total T-and helper/inducer T-cells were significantly decreased and the means of the suppressor/cytotoxic T-and B-cells were significantly increased when compared to that of the controls. However, no significant difference of definite tendency was present in relation to the number of type of major criteria at the time of the test.

3. The means of the whole patients' serum r-globulin, IgG, IgA, IgM, C3, and C4 were all within normal limits. However, when more than 3 major criteria were present at the time of the test, all the means were significantly decreased. Among the tests performed, the means of the IgA and C3 were significantly decreased when 4 major criteria were present.

4. Quantitations of total T-cells, B-cells, OKT^^4 cells and OKT^^8 cells and lymphocyte transformation test with PHA showed no significant differences according to the activity of each major symptom.

5. Levels of the serum immunoglobulins, C3, and C4 were decreased in patients with ocular lesions and the decrease of r-globulin and IgG had statistical significance.

From the results, the cell meditated immunity in Behcet's syndrome is not influenced by the number or patterns of four major symptoms or the activity of each major symptom at the time of the test. However the humoral immunity is considered to be influenced by the parameters.
