종합병원 간호인력의 적정기준에 관한 고찰 : 서울시내 300병상이상 종합병원을 중심으로

오늘날 국민의료가 중요시되고 있으며, 또한 1977년 7월 의료보험도임 이후 병원의료에 대한 수요증가로 인력의 증원은 필요불가결하다. 이에 병원의 진료사업에 있어 질·양적으로 커다란 영향을 주고 있는 간호부서는 적절한 인력배치 및 활용을 함으로써 병원의 목적, 즉 적절한 환자치료에 만전을 기할 수 있어야 한다. 본 연구는 종합병원 간호인력의 적정기준의 고찰을 위해 서울 시내 300병상 이상 13개 종합병원 전수중 조사 가능했던 11개 종합병원을 대상으로 하여 1976.1∼1979.10 사이의 의료연보 및 기획실, 인사과, 간호과의 통계자료와 소정의 면접지를 작성하여 대상 종합병원의 간호부장, 간호과장 또는 그 대리인(간호행정 업무 담당자)을 대상으로 하여 연구자 스스로 면담하여 자료를 수집하였으며, 평균치, 백분율로 자료를 처리하였다.

연구결과를 종합하면 다음과 같다.

1. 단순변수비에 의한 간호인력의 수요측정에 대한 비교(1979년 10월 현재)

첫째 환자수 : 간호원수는 조사대상 실제간호원수가 입원, 외래를 포함한 총 간호원수이므로 입원·외래를 포함한 환자수에 대한 법정간호원수와의 비율을 볼 때 실저간호원수 대 법정간호원수는 1.3:1로 법정간호원수 보다 실제간호원수는 조금 확보된 상태였다.

둘째 의사수 : 간호원수는 의사 1명에 간호원 1.3명으로 1971년 허정의 연구(1:2)에 적용하면 간호원수가 부족한 상태로 나타났다.

셋째 병상수 : 간호원수는 병상수 2·3병상에 간호원 1명으로 1975년 수련병원 진료실적(대한병원협회자료)에서와 비교 (3:1)할 때 병상수에 대한 간호원수가 조금 확보된 상태였다.

2. 대상 종합병원 간호인력 수요측정의 적용기준을 볼 때 국·공립 종합병원인 경우는 볍정기준을 그대로 준수하는 것을 원칙으로 하고 있었으며, 환자시간수와 간호시간수 산출법 적용이 3개 병원, 경험적으로 측정이 2개 병원, 기타 통괄적 적용법이 1개 병원이었

다.

3. 의료보험실시 전·후의 단순변수비에 의한 간호원의 수요측정에 대한 비교를 보면 환자수:간호원수는 입원·외래를 포함한 환자수에 대한 비율을 볼 때 실제간호원수 대 법정간호원수는 1.2:1로 의료보험실시 전·후와 같았으며, 실제간호원수는 법정간호원수보

다 어느 정도 확보된 상태였다. (의료법 시행령 제24조 1항3호 적용)

의사수 : 간호원수는 의료보험실시 전 1:0.9에서 실시후 1:1.1로 간호원수가 조금 확보된 셈이 되나 허정의 연구(1:2)에 적용하면 간호원의 부족현상을 나타내고 있다. 병상수 : 간호원수는 의료보험실시 전 2.5:1에서 실시 후 2.3:1이었으며, 1975년 수련병원 진료실적의 3:1과 비교할 때 간호원수가 조금 확보된 셈이된다.

4. 간호인력의 전문직과 비전문직에 대한 대비에서는 국·공립종합병원이 8.1:1 로 간호전문직의 비율이 제일 높았고, 재단법인이 1.6:1로 간호전문직의 비율이 제일 낮았다.

그리고 입원실 임상과별 간호인력의 전문직과 비전문직의 대비에서 전문직 간호인력의 비중이 제일 큰 중환자실은 4.4:1이었으며, 비교적 전문직 간호인력이 적은 신생아실은 2.3:1이었다.



[영문]

Health care is most important these days, and an increase of hospital personnel is necessary because of increased demands for hospital medical care since the Korean Government initiated the Health Insurance Law on July 1st, 1977. Thus in medical examinations and treatment, the nursing service can have a great influence both qualitatively and quantitatively through good nursing personnel management and utilization, so that the purpose of a hospital, namely, the giving of proper patient care, may be carried out.

In order to research the optimal level of nursing manpower in general hospitals in Seoul, of the thirteen hospitals with 300 or more beds, eleven were selected for this study, Materials referred to include annual reports of some general hospitals (1976.1 - 1979.10), statistics of the planning, personnel, nursing, hospital affairs departments as well as an interview with director of nursing, the chief of nursing, or a substitute (supervisor of nursing administration) using an interview from constructed by the writer. The collected data was manually tabulated and presented statistically using mean and percentage distributions.

The findings of the study are as follows:

Ⅰ. The demans for nursing manpower calculated on October, 1979 by crude ratio have been studied.

1. Because the present patient to nurse ratio is calculated with patient totals including both in and out-patients, the ratio of actual number of nurses to the number required by legal standard is 1.3:1.

This means that the munber of nurses actually exceeds the legal requirements (in-patient-nurse ratio : 5:2, out-patient-in-patient ratio :12:1)

2. The present doctor-to-nurse ratio is 1:1.3. When compared to the ideal ratio set by Huh Jung et al. in his research in 1971, which was 1:2, the present situation would indicate a shortage of nurses.

3. The present bed-to-nurse ratio is 2.3:1, showing an increase in nurse over the findings of a study of medical care in teaching hospitals published in the Journal of The Korean Hospital Association in 1975. The ratio reported there was 3:1.

Ⅱ. The ratios vary according to the methods used to measure nursing manpower demands. National and public general hospitals calculate ratios according to set legal standards.

Three private general hospitals use the method based on patient-hours and nursing-hours. Two general hospital calculate according to the nursing needs in the particular clinical situation. And one general hospital used an over-all evaluation

computation method.

Ⅲ. Crude ratios between patients and nurses, doctors and nurses, beds and nurses, before and after the establishment of the Health Insurance Low are compared as follows: In the first category, the actual numbers of nurses to the number required by legal standard ratio of 1.2:1 remained the same after the implementation of the Low.

In the second category, the doctor-nurse ratio showed an improvement with 1:1.1 after the Law compared with a 1:0.9 ratio before. However, this ratio still falls short of the recommended standard of 1:2 of Huh Jung et al, 1971.

The bed-nurse ratio before Health Insurance was 2.5:1, but improved after the Health Insurance Law to 2.3:1, showing an increase in the supply of nurses.

However the ratio recommended in the 1975 study reported in the Journal of The Korean Hospital Association of 3:1 would indicate the need for even more nurses.

Ⅳ. In measuring the demands for nursing manpower, the professional-non-professional nurses ratio was calculated.

The ratio of 8.1 :1 (professional to non-professional) in national and public general hospitals was the highest compared with that of private (foundation)general hospitals, 1.6:1, which was the lowest.

The in-patient - nurse ratios in the various clinical departments were calculated and compared.

In all hospitals, the highest ratio 4.4:1 was in the Intensive Care Units (I.C.U.). while the lowest, 2.3:1, was in the Nurseries.
