한국 폐경기 골조송증 환자의 칼슘섭취 및 장내 칼슘흡수에 관한 연구

폐경기 여성의 골조송증 위험인자는 폐경자체가 중요한 것으로 알려져 있고 칼슘섭취량 및 장내흡수의 결함 등이 지적되고 있지만, 국내에서는 이에 대한 연구가 드문 실정이다.

이에 저자는 한국 폐경기 여성에서 대조군 12예(56±2.7세, 폐경 후 5±4.1년)와 이중 양자 흡수기(Dual photon absorptiometry)로 진단된 골조송증 환자 7예(54±3.2세, 폐경 후 6±2.24년)를 대상으로 회상 방법(recall method)에 의한 칼슘섭취량 및 45Ca를 이용한 장내 칼슘 흡수를 조사하여 다음과 같은 결과를 얻었다.

식이를 통한 일일 칼슘섭취량이 환자군(59%)에서 대조군(86%)보다 의의있게 낮았다 (P<0.005). 경구 투여한 45Ca은 양군 모두 120분에 혈청 최고 방사능치를 나타내었다. 45Ca의 경구 투여 후 혈청 45Ca 방사능치는 환자군이 각 시간대 모두 저하되어 있었고 120분에 가장 낮았다.(P<0.05). 환자군 중 1예에서 전형적인 칼슘의 흡수장애를 보였다. 또한 1시간대의 혈중 45Ca의 방사능치와 척추와 대퇴골 골밀도의 상관 관계는, 대조군에서만 대퇴골의 골밀도와 약간의 정상관 관계가 있었고 (r=0.45), 나머지 군에서는 상관 관계가 없었다.

이상의 결과로 폐경기 골조송증 환자군에서는 대조군에 비해 1일 식이를 통한 칼슘섭취량과, 45Ca 경구 투여 후 혈청 45Ca 방사능치가 저하되어 있는 경향을 보였다.

In postmenopausal osteoporotic women, the menopause itself is the risk factor for osteoporosis. And in these patients, low dietary calcium intake and disturbance in intestinal calcium absorption have thought to be risk factors for osteoporosis.

Author surveyed the amount of daily calcium intake and serum radioactivity of orally administered 45Ca in 7 cases of postmenopausal osteoporosis and age matched 12 control subjects. The results obtained are as follows.

1. Amount of daily dietary calcium intake was significantly lower in osteoporotic group than in control group(p<0.005).

2. Orally administered 45Ca reached peak serum activity at 120 min.

3. Serum radioactivity of 45Ca was lower in osteoporotic group than in control group at 120 min.

4. There was 1 case of calcium malabsorption in osteoporotic group.

5. The 1hr serum 45Ca level showed moderate positive correlation with femur bone density in control group, but no correlation with other bone density in both groups.

Based upon these results, in postmenopausal osteoporosis, the amount of dietary calcium intake was significantly lower, and the serum 45Ca activity, after oral administration of 45Ca, showed lower tendency than in control group.
