99mTc-Pertechnetate 섭취곡선을 이용한 갑상선 기능에 대한 연구

(99m)**Tc -Pertechnetate를 사용하여 컴퓨터 처리를 이용한 전산역동학적 방법으로써 갑상선 섭취곡선지표(Thyroid Uptake Slope Index)를 구한 후 이들을 면역혈청학적 검사치인 Triiodo thyronine (T^^3 ), Thyroxine (T^^4 ), Free Thyroxine (Free T^^4 )을 중간 매체로 하여 기존의 (131)**I-24 시간 섭취율과 비교 분석함으로써 보다 간편, 신속, 정확한 갑상선 기능판별의 방법 및 그 유용성을 제시하는데 그 목적이 있다.

대상은 갑상선 섭취율 및 주사를 시행한 기능항진군 40명, 정상기능군 50명 및 정상대조군 20명으로 다음과 같은 결과를 얻었다.

1. 정상군과 기능항진군의 (99m)**Tc -Pertechnetate 섭취곡선지표는 각각 4.87±2.26, 27.67±9.56으로서 기존의 (131)**I-24 시간 섭취율이 각각 29.22±10.23, 71.45±15.51인 것과 비교해 볼때 두 군 사이의 의의있는 차이로 기능판별이 용이하였다.

2. (99m)**Tc -Pertechnetate를 이용한 섭취곡선지표와 면역혈청학적 검사치인 T^^3, T^^4, Free T^^4 와의 상관비율은 (131)**I-24 시간 섭취율과 그들과의 관계와 별 차가 없었으며, 특히 (99m)**Tc -Pertechnetate 섭취곡선지표와 (131)**I-24 시간 섭취율과의 직접적인 상관비도 0.898로서 (99m)**Tc -Pertechnetate를 이용한 섭취곡선지표의 갑상선 기능판별의 신빙성은 매우 높은 것으로 사료된다.

3. 갑상선 양엽의 기능에 현저한 차이가 있을때 양엽을 따로 분리하여 평가할 수 있는 장점이 있다.

4. 전처치 없이 빠른시간에 기능과 형태를 정확하게 파악할 수 있으며 노약자 및 항갑상선제재로 치료중인 환자의 반복검사에도 적용할 수 있는 장점이 있다.

By using (99m)**Tc -Pertechnetate, we evaluated the thyroid function of 110 persons with uptake slope index (U.S.I.) which was calculated by computerized dynamic flow study.

Also, we compared our result of U.S.I, with those of established (131)**I-24 hr uptake % in given materials by comparative analysis of their correlation with the hormonal values of T^^3, T^^4, Free T^^4. The results were as follows:

1. The U.S.I. of euthyroidismal group and hyperthyroidismal group were 4.87±2.26, 27.67±9.56 respectively. The (131)**I-24 hr uptake % of above groups were 29.22±1O.23 and 71.45±15.51. So, the differentiation of the two groups could be done more easily by using (99m)**Tc -Pertechnetate U.S.I. than by using

(131)**I-24 hr uptake %.

2. The correlation rates between (99m)**Tc -Pertechnetate U.S.I. and other laboratory hormon levels, T^^3, T^^4, Free T^^4, are almost parallel with those between (131)**I-24 hr uptake % and the values of T^^3, T^^4, Free T^^4.

Also, the direct correlation rate between (99m)**Tc -Pertechnetate U.S.I. and (131)**I-24 hr uptake % was 0.898.

So, the method of thyroidal function evaluation by (99m)**Tc -Pertechnetate U.S.I. is very reliable.

3. The (99m)**Tc -Pertechnetate U.S.I. is very helpful to evaluate the functions of each lobe respectively in cases of having obviously different radioactivity between both lobes.

4. (99m)**Tc -Pertechnetate used dynamic thyroid function study and scan can be performed during the short time without preparation and especially helpful in debilitating patient under antithyroid drug therapy, who need repetitive follow up examination.
