녹내장 여과수술의 성공률을 높이기 위해서 항대사제를 수술후에 결막하주사하는 방법이 쓰이고 있으나 이 방법이 수술의 성공률을 높일 수는 있어도 여러가지 부작용과 잦은 결막하주사로 인한 환자의 불편함이 큰 단점으로 알려지고 있다. 따라서 부작용을 줄이고 결막하주사 횟수를 줄일 수 있는 운반체계가 필요하게 되었다. liposome은 중앙의 액체핵을 주변의 지질이 들러싼 형태의 지질소포체로서 지질막이 파괴된 후에야 중앙의 액체가 배출될 수 있다. 본 실험에서는 강력한 항대사제로 알려진 cytarabine을 multivesicular liposome에 포장하여 이를 녹내장 여과수술후에 결막하주사하여서 녹내장 여과수술의 창상치유에 미치는 영향과 부작용 그리고 결막하주사 횟수를 줄일 수 있는지를 알아보고자 하였다.

백색가토에서 녹내장 여과수술을 시행한 후에 생리식염수화 cytarubine 2.5mg을 매일 1회씩 7일간 결막하주사하였고, cytarabine 2.5mg과 multivesicular liposome 포장 cytarabine(cytarabine 2.5mg을 포함)을 수술직후와 수술후 3일째에 결막하주사하고 수술후 1주, 2주, 4주, 8주에 안압측정과 수술부위 조직의 광학 및 전자현미경 검사 그리고 부작용을 관찰하였다.

1. 생리식염수를 수술후 매일 1회씩 7일간 결막하주사한군은 수술후 1주만에 수술부위가 막혔으며 안압의 유의한 하강이 없었다.

2. Cytarabine을 수술후 매일 1회씩 7일간 결막하주사한군은 수술후 2주까지는 수술부위가 막히지 알았으며 안압의 유의한 하강이 있었다.

3. Cytarabine 을 수술직후화 수술후 3일째에 결막하주사한군에서는 수술후 1주만에 수술부위가 막혔흐며 안압의 유의한 하강이 없었다.

4 Multivesicular liposome 포장 cytarabine을 수술직후와 수술후 3일째에 결막하주사한군에서는 섬유아세포의 증식이 억제되어서 수술후 2주까지는 수술부위가 막히지 알았으며 안압의 유의한 하강이 있었다.

5. 전신적인 부작요은 없었으며 눈에 나타나는 부작용은 결막하주사 횟수가 적을수록 적었다. 이상의 성적으로 미루어 보아서 cytarabine을 multivesicular liposome에 포장하여서 주사하면 약물이 수술부위에 오래동안 남아 있어서 수술부위의 창상치유를 지연시키고, 결막하주사 횟수를 줄이며, 부작용을 줄여서 multiveaicular liposome이 항대사제의 운반체계로 좋은 방법임을 알 수 있었다.



[영문]

It has been reported previously that subconjunctival injection of antimetabolites, after glaucoma filtering surgery, may enhance the surgical outcome. Complications and administration difficulties both arise from the injections and the frequency of injections. Therefore, a better delivers system is necessary to nut only improve treatment but to assure patient's compliance.

Liposomes, which are small bilayer vesicles, have been used to entrap aqueous drug solutions for slow release. In this study, multivesicular liposome-encapsulated cytarabine was injected subcoaiunctivally to observe the effect on wound healing, it's complications, and the possibility of this method for decreasing frequenoy of injections.

In normal rabbit eyes, subconjenctival injections of normal saline were administered daily for 7 days after filtering surgery in the control group. In the next group(group I), 2.5mg of cytarabine was given in the same manner as control group. In another group(group Ⅱ) 2.5mg of cytarabine was liven immediately after surgery and 3 days after surgery. The final group(group Ⅲ) was given multivesicular liesoma-encapsulated cytarabine containing 2.7mg of cytarabine in the same manner as the Group Ⅱ. The IOP and histological change of the filtering site were evaluated at postoperative 1, 2, 3, and 8 weeks.

Complications were evaluated. The results are as follows:

1.In the control group, the filtering site become occluded within a week and IOP was not decressed significantly.

2. In the group 1, the filtering site remained open and the lOP was significantly decreased for 2 weeks.

3.In the group Ⅱ, the filtering site became occluded within a week and the IOP was not decreased significantly.

4. In the group Ⅲ, the filtering site remained open and the lOP was significantly decreased for 2 weeks

5. Thaere were no systemic complications, and intraocular complications were decreased with decreasing frequency of subconjunctival injection. of multivesicular liposome-encapsulated cytarabine delay the wound healing of the filtering surgery and reduce the frequency of injections and complications.
