가족의 흡연과 어린이의 급성 호흡기질환 중 대표적 증상인 기침과의 관계를 규명하고자 강원도 원성군 내 4개면(소초, 지정, 귀래, 신림)에서 1985년 2월 18일∼7월 6일에 거주한 가구의 0∼14세 어린이 3,651명을 면접조사하였다.

14세이하 어린이 중에 흡연가족이 없는 어린이는 17.0%였으며 흡연자가 1명 있는 어린이는 63.4%, 2명이상이 있는 어린이는 19.6%였다. 2주간 평균 기침 유병률은 12.3%, 연간 기침일수는 22.7일이었다.

위험변수(흡연변수)는 가구 내 흡연자 수, 동거 흡연자 수, 부모의 흡연 여부, 가구원의 흡연량, 동거인의 흡연량, 부모의 흡연량 등 6개 변수를 사용하였다. 많은 교란변수의 영향을 동시에 조정한 후 흡연과 어린이 기침과의 관련성을 측정하기 위한 multiple logistic analysis에서는 21개 교란변수 중 아침 가래를 뱉는 가족 유무와 기침하는 가족 유무 및 어린이 연령의 3개 변수가 최종적으로 선정되었으며 6개의 위험변수별로 이들 교란변수와 함께 6개의 모델을 추정하였다.

각 모델별로 추정된 6개 위험변수의 odds의 크기는 1.26∼3.00으로서 교란변수의 영향을 조정한 후에도 부모의 하루 흡연량이 1∼14개피인 경우를 제외하고는 통계학적으로 유의한 관련성을 보여주었으며 위험변수 중 가구 내 흡연자 수와 양-반응관계를 측정하지 않은 부모의 흡연여부를 제외하고는 양-반응관계를 보였다. 그러나 위험변수와 기침하는 가족 유무 사이에 부(負)의 상호작용이 있는 경우가 많아서 기침하는 가족이 있을 때 흡

연의 odds가 1에 가까워지는 경우가 있었다.

이 연구에서 가족의 흡연이 어린이의 급성 호흡기질환에 직접적인 영향을 준다는 결론을 도출할 수는 없겠으나 서로 간에 직접, 간접적인 원인관계가 있을 것이라는 가능성은 충분히 제시한다고 볼 수 있으며 다른 많은 연구 결과와도 부합되었다.





[영문]

A household interview survey of 3,651 children aged 0∼14 years in Won Sung country was performed from Feb.-June 1985 to assess the relationship between family smoking and prevalence of acute respiratory disease(cough) in children.

The percentage of children in families with no smokers, one smoker and two or more smokers were 17.0%, 63.4% and 19.6% respectively. Period prevalence of coughing for the past two weeks was 12.3% and the average number of days of coughing per year was 22.7.

Family smoking(risk variable) was measured by using six variables which were: number of smokers in the family, parental smoking, number of smokers in the same room, number of cigarettes consumed daily by all family members, number of cigarettes consumed daily by parents and number of cigarettes consumed daily by the same room members.

Simple cross-tabulation showed a dose-response relationship between each of the six risk variables and children's coughing.

For multiple logistic analysis, six risk variables were recoded as indicator(dummy) variables and three confounding variables were selected among 21 potential confounding variables through discriminant analysis which were: child's age, coughers in the family and morning phlegm production in the family. Then six

logistic models were estimated with each of the six risk variables and the three confounding variables.

The range of estimated relative odds of six risk variables were between 1.26 and 3.00 and four of them showed a dose-response relationship. However three risk variables(number of smokers in the family, number of cigarettes consumed daily by all family members and number of cigarettes consumed daily by parents) showed a significant negative interaction with coughers in the family. so, if there were coughers in the family, the relative odds of these three risk variables were reduced to around 1.

This study does not offer a conclusion that family smoking directly contributes to the development of acute respiratory disease in children. However, this study does suggest the possibility of direct or indirect causal relationship and also

supports the findings of others that cigarette smoking by adults contributes to the development of acute respiratory disease in children of families who smoke.
