중환자실은 1943년 코펜하겐에서 Barbiturate 중독 환자 처치를 위해 처음으로 시작된 이래, 집중 관찰, 집중 치료가 병합된 특수 치료를 하는 곳으로 환자에게 고도의 질적 간호를 제공하게 된다. 이 곳은 질병의 심각성이나 특수 처치 등 집중 간호나 치료가 필요로 되는 환자를 수용하는 곳으로, 환자의 특성상 사망률도 높아지고 간호와 치료의 질을 놓이기 위해 깨끗이 소독을 한 폐쇄 병등으로 보호자를 통제하게 되므로 환자들은 소외감을 느끼거나, 자신의 질병의 심각성, 예후에 대한 두려움 등 여러 가지 부정적 경험을 하게 된다. 이런 신체적, 정서적 경험은 환자의 예후에 영향을 미치게 되므로 이를 인식하여 효과적으로 감소시킨 수 있어야 한다. 특히 중환자실에서는 질병의 심각성으로 인해 정신, 심리적, 문화적, 영적 측면의 간호를 소흘히 하게 된다. 따라서 본 연구는 이를 보강하기 위해 중환자실에 입원한 환자들이 어떤 경험을 하는지 심층적으로 알아보기 위한

사례 연구이다.

자료는 1993년 3월부터 4월까지 2달에 걸쳐 수집하였다. 연구 대상자는 Y 대학 부속 S 병원의 심혈관계 중환자실에 입원 한 경험이 있는 14명이었다. 자료는 본 연구자가 직접 심층 면담과 관찰에 의해 수집하였고, 그 외 환자의 일반적인 특성과 질병 상태에 관한 정보는 환자의 의무 기록과 수간호사와 담당 간호사로 부터 제공받았다. 자료의 누락을 막기 위해 대상자의 동의하에 녹음을 하였고, 불투명한 부분을 확인하고 추가되는 부분의 설명을 위해 필요시 2차 면담을 하였다.

자료의 분석은 Taylor와 Bogdan(1984)과 김 경동과 이 온즉(1986)의 내용분석 방법(Content Analysis)을 기초로 하였다.

자료의 분석은 자료 수집과 함께 수행하였다. 분석 과정은 연구자가 직접 대상자와 면담 후, 관찰 내용과 녹음된 면담 내용을 기록하고 서술된 전체 자료를 읽으면서 드러나는 주제(Emerging Theme)을 기록하였다.

자료 분석 결과는 중환자실 내 입원 경과에 따라 도착 당시, 안정기, 퇴원 시기로 나눌 수 있었다. 도착 당시에는 중환자실이라는 것 자체와 질병 예후에 대한 두려움, 살수 있다는 안도감, 자신의 힘으로 할 수 있는 게 없고, 수술 후의 상황이 생각했던 것보다 훨씬 더 나쁨에 무력감, 보호자도 없이 혼자 떨어져 있음으로 인한 고립감, 마취에서 깨어나면서 시간, 장소를 구별하지 못하는 지남력 상실을 경험하는 것으로 나타났다. 안정기에 들어서면 폐쇄 병실에서 혼자 지내는 것으로 인한 지루함, 답답함, 갇힌 느낌 등으로 고립감이 더욱 심해지고, 조명, 소음, 기결 조작이나 침대 이동시 부딪치는 소리 등 시각, 청각 자극으로 인한 수면 장얘, 불쾌감을 경험했다. 이 시기에는 어느 정도 적응이 되면서 요구가 많아지고, 간호사에 대한 기대도 많아지게 된다. 따라서 그런 것들의 충족 여부에 따라 친절함, 신뢰감, 쌀쌀함, 섭섭함을 경험한 것으로 나타났다. 일반 병실로 올라가는 중환자실에서의 퇴뭔 기가 되면 병실로 가서 보호자와 함께 있을 수 있다는 기대감, 몸의 움직임이 자유로워지고, 다른 사람과도 이야기도 할 수 있다는 것으로 해방감을 경험하고 있었다. 또 간호사들하고는 낯이 익어 친밀감이 생긴다고 했다. 특기할 만한 것은 대상자들이 도착 당시에 자신의 질병이나 수술 등으로 인해 심한 통증을 경험했음에도 불구하고, 면담시 신체적 고통에 관한 이기보다는 정서적, 심리적인 경험만을 중점적으로 말하는 것이었다. 이는 환자의 정서적, 심리적 경험이 신체적 경험에 비해 훨씬 더 큰 영향을 주었음을 시사하고 있다고 해석할 수 있겠다.

이상과 같은 판자들의 경험을 바탕으로 도착 당시는 환자가 있는 곳이 어딘지, 처해있는 상황이 어떤지를 설명해주어 두려움이나 고립감, 시간. 장소의 혼동을 경험하지 않도록 해주고, 안정기에 들어서면, 시간의 지루함이나 갇힌 느낌 등의 고립감이 심해지므로

가능한 환자에게는 음악을 듣거나 T.V. 등, 여가 활동을 것을 제공해야겠다. 또 시각, 청각, 촉각 자극으로 인한 수면 장애나 불쾌감을 경험하지 않도륵 조명을 조절하고, 기계음 등의 소음을 줄여주고, 기재 조작이나 환자 이동시 좀 더 주의를 기울여 부딪히는

소리가 나지 않도륵 해야 할 것이다. 충분치 못한 설명이나, 강압적이고 위압적인 말투에서 쌀쌀함, 섭섭함 등을 경험하므로 환자가 알아듣게 자세히, 부드럽게 설명을 해야겠다. 퇴원시기가 되면 환자들은 기대감, 해방감을 느끼고 있으므로 병실에 가서 할 수 있는

운동, 지켜야할 사항 등에 대해 설명을 해주는 것이 과적일 것으로 사료된다.

이상의 연구 결과를 토대로 중환자실에 입원한 환자들의 경험에 대해 깊이 있게 이해함으로써 효과적으로 중재할 수 있는 방안을 마련, 신체적, 정서적, 심리적, 영적, 문화적 간호를 위한 기초가 되리라 기대한다.



[영문]

The Intensive Care Unit was first established for the treatment of barbiturate addiction in Copenhagen in 1943. An intensive care unit groups critically ill patients under the care of specially trained staff members who ideally have both the e여cation and personality to give attent ion to minute physio1og1ic variations.

Because of the high mortality rate and isolation from significant others, many patients have negative experiences 1ike fear, isolation and powerlessness. Especially, in the intensive care unit, because of the seriousness of the patients's illness, psychologic, emotional, cultural and spiritual nursing care may

not receive appropriate attention. So this case study was done to clarify the experiences of the patients in the cardiovascular intensive care unit.

The data were collooted by the researcher during the period from March 1, 1993 to April 30, 1993. The subjects were 14 patients in the cardiovascular intensive care unit. The data were collected through in-depth interviews, as well as observations

and examination of the medical records.

According to the stave, that is admission, stability, discharge, different types of experienced can be identified. The findings are as follows.

1. Upon arrival, most patients experience fear about the prognosis of their illness. relief that they are being treated but, powerlessness, isolation and disorientation.

2. During the stable stave, they experience isolation, sleep disturbance due to tactile, visual and auditory stimuli with adaptation to the situation interaction and requests to the nurses increase, the types of response given leads to increased

feeling of unpleasantness and discontentment.

3. At the discharge stage, expectation of recovery from the illness, freedom and friendliness with nurse were the emerging theme.

From the results of this study the following recommendations can be made. Isolation and disorientation can be reduced by diversional activity. Reducing sensory stimulations such as bright lights, noise and procedures which cause disturbance of sleeping and unpleasantness will decrease these negative aspects of admission to the I. C. U. .Instead of oppressive and unilated communication, explanations in detail can he done, so the patients do not experience discontentment. During the discharge stave, teaching rehabi1itation exercise and follow up care regimen should be included in the nursing care.

This study provides a better understanding of the experiences of patients in the cardiovascular intensive case unit and may faci1itate more appropriate physica, emotional, psychological, spiritual and cultural intervention strategies to minimize the negative experiences.
