소아의 뇌막염에서 뇌척수액내의 유산량에 관한 연구

소아에 있어서 각종 뇌막염의 정확한 감별진단을 즉각적으로 하기란 어렵고 여러 가지 임상소견과 뇌척수액의 변화에 대한 주의깊은 관찰로도 진단이 의심스러워 어려움이 따르게 된다.

그러나 뇌척수액의 소견이 뇌막염의 진단에 중요한 것은 주지의 사실이며 화농성 및 결핵성 뇌막염 환자에서 뇌척수액의 특징적 소견으로 당량의 감소의 정도는 병의 경중 및 혈당량의 변화에 따라 다르다(Petersdorf. 1959., 李哉昇, 1971.)

Cantarow(1959)는 뇌막염에서 뇌척수액내의 당량감소는 해당작용의 촉진으로 생성되며 그 종산물인 유산이 뇌척수액내 축적되나 정상에서는 없거나 극 미량 검출된다고 하였다. Levinson(1971년)은 뇌막염환자에서 뇌척수액내의 pH는 정상이하로 감소하고 뇌척수액

의 알카리도(度)의 감소로 산성증이 생기며 당의 해당작용으로 생긴 종산물로서 유산이 수막염균성 뇌막염환자와 결핵성 뇌막염 환자에서 뇌척수액내에 축적된다고 하였다.

이에 저자는 이들 측정치의 진단적 가치를 알기 위하여 세브란스병원 소아과에 입원한 75명의 뇌막염 환자와 건강한 고아원아동 25명을 대상으로 유산량을 검사하였고 대상아동을 4군으로 나누어 제1군은 정상아, 제2군은 비세균성 뇌막염 환아, 제3군은 화농성 뇌막염 환아, 제4군은 결핵성 뇌막염환아로 구분하였다.

이들의 결과를 보면

A. 제1군(정상아군): 25예로 뇌척수액내 유산량은 평균치 5.8mg/dl이고 혈액내 유산량은 7.1mg/dl이었다. 이들간의 상관관계는 없었으며 뇌척수액내의 당량과 유산량과의 상관관계도 없었다.

B. 제2군(비세균성 뇌막염군): 25예로 척수액내 산량은 평균치 8.8mg/dl로 정상아군에 비하여 의의있는 증가를 보였다. 혈액내 유산량의 평균치는 7.4mg/dl로 정상에 비하여 증가는 없었으며 뇌척수액내 유산량에 대한 혈액내 유산량의 상관관계는 의의있게 나

타났다. 그러나 뇌척수액내의 유산량과 당량은 상관관계가 없었다. C. 제3군(화농성 뇌막염군): 20예로 뇌척수액내 유산량은 평균치 36.4mg/dl로 정상아군에 비하여 현저한 증가를 보였고 혈액내 유산량도 평균치 26.2mg/dl로 의의있는 증가를 보였으며 뇌척수액내 유산량과 혈액내 유산량은 상관관계가 있었다. 뇌척수액내의 당량과 유산량과의 관계는 역상관 관계를 보였다.

D. 제4군(결핵성 뇌막염군): 30예로 뇌척수액내 유산량은 평균치 25.6mg/dl로 정상아군에 비하여 의의있는 증가를 보였으나 화농성 뇌막염군의 유산량보다 적은 중증도의 증가를 보였다. 혈액내 유산량도 평균치 18.9mg/dl로 의의있는 증가를 보였고 뇌척수액내 유

산량과는 상관관계가 있었다.

E. 12예의 추적검사는 결핵성 뇌막염 8예와 화농성 뇌막염 4예로 뇌척수액내 유산량의 치료기간 동안의 변화는 병세의 호전과 함께 정상으로 환원됨을 볼 수 있었다. 화농성 뇌막염예에서는 치료후 3일째의 뇌척수액내 유산량은 치료전 유산량의 반으로 감소하였고

치료 6일경에 정상 범위내로 환원되었다. 결핵성 뇌막염예에서는 치료후 5일경부터 감소하여 20일경에는 유산량이 정상으로 환원됨을 관찰할 수 있었다.

이상의 결과로 뇌막염에서 뇌척수액내 유산량 측정은 뇌막염의 진단과 감별진단에 지침이 될 수 있으며 병의 경중 및 호전여부를 알 수 있는 방법으로 추천할 수 있다.

Prompt, precise differentiation between various forms of meningitis-aseptic, bacterial and tuberculous-is critical difficult problem for the practicing pediatrician.

The present investigation was carried out to assess the diagnostic usefullness of lactic acid levels in CSF. Fresh sample of CSF and blood were collected from 75 infant and children with meningitis and 25 normal children.

CSF lactic acid, CSF sugar, blood lactic acid and blood sugar were estimated from them. These subject were divided into 4 group: Group 1 (Normal-25 cases), Group 2 (aseptic meningitis-25 cases), Group 3 (purulent meningitis-20 cases), Group 4 (T.B. meningitis-30 cases).

The results were as follows:

1. In normal group, CSF lactic acid was 5.8 mg/dl and blood lactic acid was 7.1mg/dl. The ratio of CSF Lactic acid to blood lactic acid was approximately 0.8.

2. In Group 2. (aseptic meningitis), CSF lactic acid was 8.8 mg/dl and blood lactic acid was 7.4 mg/dl. The CSF value was slightly increased compare to normal CSF lactic acid levels. CSF sugar and lactic acid were not correlated.

3. In the purulent meningitis group, CSF lactic acid was markedly increased to a significant level compare to the normal value. The mean was 36.4 mg/dl. Blood lactic acid also increased in linear fashion with CSF lactic acid. The ean value of blood lactic acid was 26.2 mg/dl.

4. In T.B. meningitis, There was moderate increased of lactic acid significantly different from normal values. The mean was 25.8 mg/dl. Blood lactic acid also increased. The mean was 18.9 mg/dl. A positive linerar correlation between CSF lactic acid and blood lactic acid was noted.

5. On the 3rd hospital day, CSF lactic level was approximatel half level of the initial values in purulent meningitis.

It returned to normal levels on 6th hospital day.

In T.B. meningitis, CSF lactic acid also showed a decrease from the 5th hospital day and returned to normal values on 20th hospital day.

Concerning the above results, CSF lactic could be a possiblely be a good index in differential diagnosis of meningitis and in it's clinical course
