황산구리 투여가 3'-methyl-4-dimethylaminoazobenzene에 의한 간암 발생과정에 미치는 영향

인체에서 간에 구리가 축적되는 대표적인 질환중 Wilson병의 경우 적절한 치료를 하면 중년이상이 되도록 생존할 수 있을 뿐 아니라 원발성 담증성 간경변증도 혈색소증(hemochroma-tosis)보다 빨리 간경변증으로 진행함에도 불구하고 간암종이 거의 발생하지 않는

점은 구리에 항암효과가 있을 가능성을 시사한다. 또한 간세포암종 중 예후가 좋은 것으로 알려진 fibrola-mellar 형에서는 암종세포내에 구리 및 구리결합단백이 축적된다는 보고가 있으며, 간암종 환자에서 혈청구리가 적을 때 예후가 더 좋다는 보고 등 간암종에 대한 구리의 영향에 관한 간접적 증거들이 제시되어 있음에도 불구하고 간암종 발생에 미치는 구리의 효과를 구명하기 위한 실험적 연구는 미미한 실정이다.

본 연구에서는 구리가 화학 발암제에 의한 간암 발생과정에 미치는 영향을 알아보기위하여 흰쥐에 황산구리 및 3'-methyl-4-dimethylaminoazobenzene(이하 3'-MeDAB으로 약함)을 단독 또는 병합으로 기간을 달리 투여한 후 도살시기를 달리하여 간의 무게, 구리 함량 및 조직학적 소견과 간에 발생한 병변의 bromodeoxyuridine(이하 BrdU로 약함) 표지지수를 조사하였으며, 그 결과를 요약하면 다음과 같다.

1. 간의 구리함량은 구리와 3'-MeDAB을 병합투여하고 3일 후에 도살한 군에서 의의있게 높았으나, 투약기간에 따른 차이는 없었으며, 투약 중단후 정상범위로 감소하였다.

2. 3'-MeDAB을 단독 또는 구리와 병합투여한 모든 경우에 난원형세포(oval cell)의 증식, 간세포의 과증식결절(hyperplastic nodule), 그리고 섬유성 담관증식(cholangiofibrosis) 및 암종이 발생하였다. 이들 중 암종이외의 병변들은 발생빈도에 있어서 구리병합투여 여부에 따른 차이를 보이지 않았으나, 구리병합투여시 간의 과립, 결절 및 종괴형성과 간의 무게 증가가 뚜렷하게 적었고 암종의 발생도 적었다.

3. 3'-MeDAB 단독 또는 구리 병합투여시의 과립, 결절 및 종괴형성과, 간의 무게는 투약 중단후에도 더 증가하였다.

4.난원형세포, 과증식결절, 섬유성 담관증식 및 암종의 BrdU 표지지수는 구리병합투여 여부에 따른 차이를 보이지 않았다.

5. 섬유성 담관 증식의 발생빈도나 BrdU 표지지수는 투약중단 후에도 매우 높은 정도로 유지되었으나, 난원형세포의 증식과, 난원형세포 및 과증식결절의 BrdU 표지지수는 투약 중단후 감소하였다.

이상의 결과를 종합하여 보면 구리가 흰쥐에서는 간에서 빨리 대사되어 소실되기 때문에 3'-MeDAB에 의한 발암과정을 완전히 억제하지 못하지만 전암단계 병변들의 발생을 지연시키며, 일단 발생한 병변들의 성장속도에는 효과가 없다고 생각한다.



[영문]

Excess copper is accumulated in the liver of patients with Wilson's disease and primary biliary cirrhosis which are known to progress to liver cirrhosis in their early course. However, hepatic carcinomas develops exceptionally in these copper excess conditions, suggesting that copper may play an inhibitory role in the development of hepatic carcinoma. Furthermore, it has been frequently reported that the hepatic copper content is increased in hepatocellular carcinoma of the fibrolamellar type, which is known to be less aggressive than the usual type of hepatocellular carcinoma.

This study is aimed to elucidate the effect of copper on the

3'-methyl-4-dimethylaminoazobenzene(3'-MeDAB) induced hepatic carcinogenesis. Sprague-Dawley rats were used for the experiment and divided into 4 groups according to 3'-MeDAB and copper administration: Ⅰ. normal control, Ⅱ. copper only, Ⅲ. 3'MeDAB only, Ⅳ. 3'-MeDAB plus copper. Animals of the groups Ⅲ and Ⅳ

were administered intraperitoneally in a dose of 0.5 mg, twice a week. Animals were sacrificed at different intervals. Liver weight, hepatic copper content and the gross and microscopical changes of the liver were examined and compared between the

experimental groups. Cell kinetics of the various lesions developed in the hepatic carcinogenesis was studied by applying the immune-histochemical method for bromodeoxyuridine(BrdU).

The hepatic copper content was significantly increased in animals given copper but returned to normal value after cessation of administration. 3'-MeDAB administration caused oval cell proliferation and produced hyperplastic nodule, cholangiofibrosis and carcinoma of tIne liver. Simultaneous administration of

copper did not alter the incidence of the lesions induced by 3'-MeDAB, except for carcinoma. The liver weight and the hepatic nodules and masses of animals given 3'-MeDAB and copper were smaller than those given 3'-MeDAB only for same periods.

The development of carcinoma was less frequent as well in animals given 3'-MeDAB and copper. The liver weight as well as the granularity, nodularity and mass formation continued to increase after cessation of 3'-MeDAB administration. Copper did not affect the BrdU labelling indices of the hepatic lesions induced by

3'-MeDAB. The oval cell proliferation and the BrdU labelling indices of oval cell and hyperplastic nodule were decreased, but the incidence of cholangiofibrosis and its BrdU labelling index were still elevated after cessation of 3'-MeDAB administration.

These findings indicate that copper could delay the development of 3'-MeDAB induced hepatic lesions, but not suppress, since copper does not stay long to accumulate in rat liver and that copper could not affect the proliferation of 3'-MeDAB induced hepatic lesions once developed.
