한 지역사회병원 퇴원환자의 장기요양시설 이용의사에 영향을 주는 요인

지역사회 병원의 만성병 퇴원환자들의 장기요양시설 이용의사와 이에 영향을 미치는 요인을 분석하고 이들 요인간의 관계를 구명하기 위하여 한 지역사회에 위치한 종합병원에서 1984년 1월 1일부터 1986년 12월31일가지 입원했다가 퇴원한 60세이상 만성병 환자 176명을 대상으로 1987년 4월 25일부터 5월 5일까지 면접 설문조사 하였다. 분석은 교차제표에 의한 분석과 판별분석 방법을 적용하였다. 그 결과는 다음과 같다.

1. 대상자들의 장기요양시설 이용의사는 69.9%가 이용하겠다고 하였고 가족들의 의사도 75.6%가 이용하겠다고 하였다.

2. 대상자들의 가족수가 많을수록 장기요양시설 이용의사가 통계학적으로 유의하게 높았다.

3. 대상자들의 질병은 우리나라의 만성질환 구조와 마찬가지로 뇌혈관계질환, 당뇨병, 호흡기관지계 질환이 많았고 뇌혈관계 질환자의 이용도가 높았다.

4. 대상자들의 질병치료 형태에 따라서는 입원횟수가 많을수록 이용의사가 높았으나 입원기간에 따라서는 입원기간이 짧을수록 경제적으로는 가구 소득이 높을수록 이용의사가 통계학적으로 유의하게 높았다.

5. 이용의사 유·무집단으로 대상자군을 분류했을 때 제특성들이 이용의사를 결정하는 판별정도는 입원기간이 제일 높았고, 다음으로는 가족수, 입원횟수, 총외래 방문횟수의 순으로 두집단에 대한 판별력이 높았다.

이상과 같이 이용의사에는 가족수와 입원횟수, 가구소득, 교육수준, 외래를 방문하는 정도, 입원기간 등의 요인이 중요하게 영향을 미쳤다.

The purpose of this study is to determine the factors affecting discharged patients' desires for long-term care faility utilization.

For this study an oral-interview survey was conducted from April 25 to May 5, 1987. Samples for che survey were collected from 175 discharged patients who were hospitalized in Wonju Christian Hospital from January 1, 1983 to December 31, 1986. Two-Way cross-tabulation analysis and discriminant analysis were applied.

The major findings of this study can be summarized as follows:

1. A total of 69.9 percent of the sampled persons expressed desire for long-term care faility utilization.

2. The patterns of disease in this study were similar to the general pattern of chronic disease in Korea.

3. Two-way cross tabulation analysis showed the larger the family size, the stronger the desire for long-term care facility. The higher the family income, and the less number of hospitalizations, the stronger the desires were also.

4. Discriminant analysis showed that the most powerful discriminant variables were length of hospital stay, family size and number of hospital admissions.
