병원폐기물의 처리실태와 적정관리에 관한 연구

전국민 의료보험 시대를 맞아 의료기관을 이용하는 환자의 수가 증가하여 의료활동에 수반되어 발생하는 감염성 의료계 폐기물의 적정처리가 사회문제화 되고 있다.

우리나라에서는 도시 폐기물을 비롯하여 산업 폐기물의 위생적인 처리방안은 많은 연구가 있었으나, 현재까지 감염성 의료계 폐기물의 적정관리에 관한 구체적인 조사연구가 없는 실정이다.

본 연구에서는 병원폐기물의 처리 실태를 파악하여 적정관리 방안을 제시하기 위해 1990년 1월부터 4월까지 서울시 6개 병, 의원을 대상으로 병원 폐기물의 종류 및 배출량을 조사하고 배출량과 종사자수, 병상수, 연면적과 상관관계를 분석하여 다음과 같은 결과를 얻었다.

1. 조사대상에서 배출하는 의료기관별 적출물 배출량은 1차 의료기관이 60.5kg/월(4.4%), 2차 의료기관이 277.5kg/월(20%), 3차 의료기관이 1,047.2kg/월(75.6%)이었으나, 총 병원폐기물 중 적출물이 차지하는 비율은 0.8%이었으며, 종사자수당(인) 의료계폐기물의 발생원단위는 2.8-6.9kg/day/인으로 조사되었다.

2. 의료기관별 산업폐기물 배출량은 1차 의료기관이 77.6kg/월(1.6%), 2차 의료기관이 1,400kg/월(28.3%), 3차 의료기관이 3,461,3kg/월(70.1%)이었으나, 총 병원폐기물 중 산업폐기물이 차지하는 비율은 2.8%이었으며, 병상수당 의료계폐기물의 발생원단위는 4.1-6.9kg/day/bed으로 조사되었다.

3. 의료기관별 일반폐기물 배출량은 1차 의료기관이 4,570.9kg/월(2.7%0, 3차 의료기관이 30,264.5kg/(월18.1%), 3차 의료기관이 132.566.3kg/월(79.2%)이었으나, 총 병원폐기물 중 일반폐기물이 차지하는 비율은 96.4%이었으며, 연면적당(m**3) 의료계폐기물의 발생원단위는 0.07-0.15kg/day/m**3으로 조사되었다.

4. 병원폐기물 발생량(C4)과 종사자수(C1)와의 상관관계〔C4(t/월=0.0909C1 + 4.43, r=0.99〕, 병상수 (C2)와의 상관관계 〔C4(t/월)= 0.17C2 - 3.71, r=0.092〕, 연면적(C3)과의 상관관계 〔C4(t/월)= 0.00241C3 + 3.37, r=0.998〕는 높았으나 Mann-Whitney U test에서는 폐기물 발생량과 연면적과는 통계학적으로 유의한 차이가 있었다. (P<0.05).

5. 폐기물 처리방법은 의료기관내에 자체처리장이 없어 중간적환장에 보관한 후 전체 위탁처리 하는 것으로 나타났다.

6. 병원폐기물의 유해성에 따른 발생장소별 분리수거율이 낮았다.

7. 폐기물 관계법규의 미비로 감염성 폐기물(적출물)은 제도적으로 산업폐기물에 포함 되지 못하고 있었다.

8. 병원 폐기물의 제도적인 유통과정을 통제 할 수 있는 복수관리전표 체제가 없었다.

9. 병원폐기물을 적정관리 하기 위하여 행정관청의 제도적인 관리방안과 병원의 기술적 관리방안에 따라 감염성 폐기물의 적정처리 기준을 설정할 수 있는 행정적인 기준이 필요하다.

이상의 결과로 보아 병원 폐기물의 적정관리 기본은 분리배출에 영향이 있는 것으로 나타났으며 발생원으로부터 최종 안정화 처리에 이르기 까지 제도적으로 위생적인 적정관리 대책이 요구될 뿐만 아니라 감염성 폐기물의 안정화 처리에 대해 보건학적인 측면에서 이 분야에 더 많은 연구가 필요하다고 본다.

With the bignning of the medical in surance, hospital wastes by medical activities has been increased and its increment is proportional to the number of patients utilizing the medical institution. The proper treatment of wastes. is now become as one of the great environmental problem in Korea.

There has been various kinds of surveys on the sanitary treatment of urban wastes and industrial wastes, but when it comes to infectious medical wastes, there has hardly been any research on the proper management of them.

For proposing a devices for the proper management with grasping the present status of treating medical wastes, survey was conducted from Jan. to April in 1990 to identify the sorts and quantities of the medical wastes and data were analyzed to correlated between the waste quantities and the number of employees, sickbeds and the total floor area at six hospitals in Seoul.

The results are as follows:

1. The amount of wastes generated in a month is 60.5 kg (4.4%) in the medical clinic, 277.5 kg(20%) in the medium sized hospital, 1047.2 kg/month(75.6%) in the large hospital. The exhaust amount accounts for 0.8% of the total medical wastes. The unit amount of the hospital wastes was calculated as 2.8-6.9 kg/day per medical personnel.

2. The amount of industrial wastes generated in a month is 77.6 kg(1.6%) from the 1400kg (28.3%) from the 3461.3 kg(70.1%).

The industrial wastes accounts for 2.8% of the total medical wastes.

The unit amount of the hospital wastes was calculated as 4.9-6.9 kg/day per sickbed.

3. The amount of general wastes generated in a month is 4570.9 kg(2.7%) for the 30264.5 kg(18.1%) for the 132566.3kg(79.2%). The general wastes accounts for 96.4% of the total medical wastes. The unit amount of the hospital wastes per unit area (m**2 ) was calcuated as 0.07-0.15 kg/day.

4. The correlation coeflicient between the amount (C^^4 ) of the medical waste generated and the number of employees (C^61 ) is 0.99 〔C^^4 = 0.0808 C^^1 + 4.43, r = 0.99〕 and the correlation betwen the C^^4 and the number of sickbedgs is also

0.992 〔C^^4 = 0.17C^^2 - 3.71, r = 0.992〕.

Finally, the correlation between the C^^4 and the total floor area is 0.998 〔C^^4 = 0.00241C^^3 + 3.37, r = 0.998〕 but there are the statistical difference (P<0.05) between the amount of waste gererated and the total floor area according to the Mann-Whitney U test.

5. All the hospitals investigated disposed their wastes to the outside for the treatment because the hospital had no plants for their own treatment.

6. Harzardous wastes discharged from the hospitals which must be separated from the general wastes were not collected separately.

7. Infectious wastes are not included in the industrial wastes because of a loophole of the environmental law and regulations.

8. There were no effective system that can manage wastes discharged from the hospitals.

9. The institutional countermeasure for treating wastes is wgent for the hospitals to manage properly the medical wastes.

As a result, the classification of wastes from the hospitals is the first-step to solve the problem that make it possible to manage the wastes from the origin.

The stabilization step to prevent adverse effects from wastes is stronghy required for the oltizen's health and also continuous research for stabilizing treatments of the infections wastes is necessary.
