학생들의 건강교육 프로그램 개발을 위해 요구되는 행동변화의 기준을 파악하기 위해서는 건강행동에 영향을 미치는 요인들을 찾아내는 작업이 우선되어야 한다. 이 연구에서는 초, 중학생의 건강행동에 영향을 미치는 요인을 분석하고 이들 요인간의 관계를 구명하기 위하여 서울에 위치한 국민학교와 중학교 학생을 임의로 선정하여 학생과 그 학생의 일차적 보호자를 대상으로 1990년 4월 11일부터 20일까지 자기기입식 질문서를 사용하여 조사하였다.

수집된 자료 중 분석가능한 응답 366사례에 대하여 건강행동에 영향을 미치리라고 가정된 요인들로 가설화된 모델에 따라 경로분석을 하였는데 결과는 다음과 같다.

1. 국민학교 6학년 학생이 184명, 중학교 1학년이 81명, 중학교 2학년이 101명이 분석되었다. 이들의 일차적 보호자는 76.5%가 어머니, 10.7%가 아버지, 기타가 13.8%였으며 보호자의 평균연령은 40.5세였다.

2. 학생의 질병취약성에 대한 부모와 학생의 인식은 다른 아이들과 비교해 53.8%가 '덜 아프다', 16.1%가 '자주 아프다'고 응답하였다. 학생의 건강상태에 대해서는 전체 응답자의 75.5%가 건강이 보통 이상이라고 인식하였고, 건강에 대한 염려는 '자주 걱정한다'가 32.5%, '때때로 걱정한다'가 39.1%, '거의 걱정하지 않는다'가 28.4%였다.

3. 학생의 건강행동과 보호자의 건강교육은 측정가능한 척도값의 범위가 24-120점이었는데, 학생의 건강행동은 83.9±11.1, 보호자의 건강교육은 93.2±11.5의 분포를 보였다.

4. 경로분석 결과 학생의 건강행동에 영향을 미치는 각 변수들의 인과적 총효과는 학생의 내적건강통제위가 0.382로 가장 컸으며, 학생의 건강관심도가 0.363, 일차적 보호자의 평상시 건강교육이 0.271, 지가된 건강상태가 0.130, 학생의 외적건강통제위가 0.127,

건강염려도가 0.121, 보호자의 내적 및 외적건강통제위가 각각 0.083, 0.035이고, 보호자의 사회경제적 지위가 0.031, 학생의 연령은 -0.020이었다.

5. 학생의 건강행동에 대한 직접적인 효과는 학생의 건강관심도가 0.363으로 가장 컸으며, 그 다음이 학생의 내적건강통제위로 0.305, 보호자의 평소건강교육이 0.163이었다.

6. 학생의 연령, 일차적 보호자의 사회경제적 지위, 건강염려도, 지각된 건강상태는 일차적 보호자의 건강교육과 학생의 내적건강통제위, 건강관심도를 경유하여 간접적으로 학생의 건강행동에 영향을 미쳤다.

결론적으로. 연구에서 설정한 가설들이 모두 채택되어, 보호자가 평소에 건강교육을 많이 할수록 학생의 건강행동 이행수준이 높았고, 건강에 대한 관심이 높은 학생의 건강행동 이행수준이 높았으며, 내적 건강통제위 성향이 강한 학생의 건강행동 이행수준이 높았음을 알 수 있었다.

결국 학생의 건강행동에 영향을 미치는 요인들 중 가장 중요한 것이 학생의 내적건강통제위, 학생의 건강관심도 그리고 보호자의 평소 건강교육이었고, 건강교육프로그램을 개발할 때 이와 같은 연구 결과를 감안한다면 학생들에 대한 효율적인 건강교육에 도움이 되리라 생각한다.

앞으로 이 연구에서 고려하지 못한 일차적 보호자 이외의 가족, 친구와 같은 사회적 구성원들과 관계망을 통한지지(social network support), 학교교사에 의한 교육과 같은 여러 요인들을 포함하여 좀 더 구체적이고 체계적인 연구와 이들 요인간의 관계를 구명하여 건강교육 교과과정에 반영하는 방안을 모색하는 연구가 필요하다.



[영문]

This study was designed to identify factors affecting children's health behavior. Analysed data were collected through a selfadministered questionnaire survey conducted in April 1990 given to elementary and middle school children and their primary caretakers. The subjects were composed of 366 pairs of child-caretakers.

Data were analysed through the method of path analysis. The results were as follows:

1. Children of the 6th grade of elementary school numbered 184, the 1st grade of middle school 81, and the 2nd grade of middle school 101. Primary caretakers were mothers(75.5%), fathers (10.7%), and others (13.8%); the average age of primary caretakers was 40.5 years old.

2. Children's and primary caretakers' cognition of disease vulnerability was 'less vulnerable' in 53.8% and 'more vulnerable' in 16.1%; 75% of respondents perceived their health as good; ;and the level about anxiety health was 'often anxious' in 32.5%, 'sometimes anxious' in 39.1%, and 'seldom anxious' in 28.4%.

3. The levels of children's health behavior and primary caretakers' everyday health education were measurable in the range of 24-120; children's health behavior was 83.9±11.1(mean±SD), and caretakers' health education was 93.2±11.5.

4. According to path analysis, the total effects on children's health behavior were 0382 in children's internal health locus of control, 0.363 in children's health concern, 0.271 in primary caretakers' health education, etc.

5. Direct causal effects on children's health behavior were 0.363 in children's health concern, 0.305 in children's internal locus of control, and 0.163 in primary caretakers' health education.

6. Children's age, primary caretakers' socio-economic status, level of anxiety about health, and perceived health status affected the children's health behaviors via primary caretakers' health education, children's internal health locus of control and children's health concerns.

In conclusion, the hypotheses which were proposed in this study were all accepted. Therefore, children who were better health-educated by their primary caretaker, who were more concerned with their health, and who had a higher internal health locus of control practiced more health behavior.

Thus, the most significant factors affecting children's health behavior were the children's internal health locus of control, children's health concern, and primary caretakers' health education. Consideration of these results in developing health

education programs could enhance the performance of health education.
